[Natalizumab in the treatment of multiple sclerosis].

AIM: To review and update the mechanism of action of natalizumab and its efficacy in the treatment of multiple sclerosis (MS). DEVELOPMENT: Natalizumab, an anti-alfa-4 integrin monoclonal humanized antibody, binds to lymphocyte surface receptors to prevent transmigration of lymphocytes to areas of inflammation into the brain tissue. Furthermore, natalizumab appears to reduce T-cell activation following their infiltration of the brain parenchyma and may contribute to T-cell apoptosis in these tissues. Two large two-year, multicenter phase III trials (AFFIRM and SENTINEL) have been completed an…

Multiple sclerosis as a vascular disease.

Multiple sclerosis (MS) has traditionally been viewed and researched as an immune-mediated disease with principal emphasis on the role of activated inflammatory cells, oligodendrocytes and astrocytes in its pathogenesis. Abnormalities of cerebral endothelial cells (CECs) is an under explored facet of MS pathogenesis and vascular abnormalities play a crucial role in formation of the MS lesions and disease progress, at least in the initial stages of disease. This review will focus on MS as a central nervous system (CNS) disease with a strong vascular constituent and examines abnormalities within…

Diagnosis and management of multiple sclerosis.

Multiple sclerosis, an idiopathic inflammatory disease of the central nervous system, is characterized pathologically by demyelination and subsequent axonal degeneration. The disease commonly presents in young adults and affects twice as many women as men. Common presenting symptoms include numbness, weakness, visual impairment, loss of balance, dizziness, urinary bladder urgency, fatigue, and depression. The diagnosis of multiple sclerosis should be made by a physician with experience in identifying the disease. Diagnosis should be based on objective evidence of two or more neurologic signs t…

Parkinsonism as a manifestation of multiple sclerosis.

We describe a 48-year-old patient, with a diagnosis of relapsing-remitting multiple sclerosis, who presented to our service with a parkinsonian syndrome that markedly improved after corticosteroid treatment. To the best of our knowledge, only 12 cases of parkinsonism have been reported from 1970 to the present, of which only 8 seemed secondary to MS, i.e., those presenting conclusive imaging evidence or unequivocal response to corticosteroids.

[The laboratory diagnosis of multiple sclerosis].

The importance of laboratory methods for multiple sclerosis (MS) diagnosis and differential diagnosis is often overestimated now. The role of several methods including MRI, evoked potentials, examination of the cerebrospinal fluid and some others methods are discussed in this review. Several conditions may in some patients mimick the appearance of MS and it is easy to understand why there is a tendency among many clinicians to embark on extensive- and expensive-laboratory investigations to establish the correct diagnosis at early stages of the disease. Disorders like cerebrovascular diseases, …

[Changes in the concept of multiple sclerosis].

The author summarizes the newest knowledge in the past 10 years that has changed our way of thinking as well as the treatment strategy of the still incurable Multiple Sclerosis. The sophisticated immunological methods have brought new insight into the inflammatory process of the central nervous system. The MR examinations and the newly developed techniques allow examining the dynamism of the disease and the underlying histological alterations. The testing of new drugs--which proved to be efficient--made it possible to follow more precisely not only the classification of the pathological proces…

Rationale for immunotherapy in multiple sclerosis.

The presumed but unspecified immune-mediated basis for the pathogenesis of multiple sclerosis (MS) has led to therapeutic attempts to modify the immune system in general and in selective ways in patients with MS. In general, antiviral, anti-inflammatory, immunosuppressant, and immunomodulatory therapies have been considered. More specifically, these treatments have involved the use of glucocorticoids; immunosuppressant drugs and physical agents such as irradiation; modifications of the immune environment with therapeutic plasma exchange and intravenous immunoglobulin; and more recently, altera…

Multiple sclerosis in pregnancy: a review.

This case history discussed multiple sclerosis that began during pregnancy, in a patient with a history of juvenile rheumatoid arthritis. The etiology, epidemiology, pathophysiology, diagnosis, clinical course, and management of MS are reviewed from the point of view of an obstetrician, with emphasis on the interaction between MS and pregnancy. The similarities and differences between MS and rheumatoid arthritis, with respect to the effect of pregnancy on these diseases, is discussed. Because of the generally beneficial effect of pregnancy on MS and rheumatoid arthritis, and the frequent exace…

The pineal gland in multiple sclerosis.

Multiple sclerosis (MS) is a chronic demyelinating disease of unknown etiology. Clinical, neurochemical, and neuroradiological data implicate the pineal gland in the pathophysiology of MS. To investigate the relationship of MS to the pineal gland further, we surveyed the prevalence of pineal calcification (PC) on CT scan in a cohort of 29 MS patients (7 men, 22 women, mean age: 40.1 years, SD = 8.9) who were admitted consecutively to a neurological service for acute exacerbation of symptoms. For the purpose of comparison, we also surveyed the prevalence of choroid plexus calcification (CPC) in…

Trauma and multiple sclerosis. An hypothesis.

An obligatory event in the pathogenesis of the multiple sclerosis plaque appears to be an increase in the permeability of the blood-brain barrier. Neuropathological observations of the brain of persons suffering from concussion after relatively minor head injury, as well as of animals subjected to experimental brain injury, have shown that alterations of the blood-brain barrier constitute a common result of such trauma. It is postulated that the alterations of the blood-brain barrier secondary to trauma of the brain or spinal cord of patients with already established multiple sclerosis may res…

Prevalence of multiple sclerosis in Saskatoon.

In an epidemiologic survey based on a search of all available medical records for 1955 to 1980 at the city's three general hospitals the prevalence in Saskatoon of probable multiple sclerosis was found to be 111/100,000 and that of combined probable and possible multiple sclerosis 134/100,000 on Jan. 1, 1977. The average annual incidence over the three decades was 4.8/100,000. These prevalence and incidence rates were two to three time greater than those reported for other Canadian cities. Of the 150 individuals with probable multiple sclerosis 87 were living in Saskatoon at the time of onset …
