Denervation of the external branch of the superior laryngeal nerve: laryngeal and phonatory features.

PURPOSE OF REVIEW: The external branch of the superior laryngeal nerve (ESLN) innervates the cricothyroid muscle of the larynx, a vocal fold tensor primarily responsible for pitch elevation. A longstanding controversy is revisited regarding the laryngeal and phonatory signs that should be considered indicative of unilateral ESLN paralysis/paresis. RECENT FINDINGS: Diagnosis of unilateral ESLN paralysis remains challenging. Despite advances in laryngeal electromyography and improved techniques to visualize the larynx, there is no consensus regarding whether any laryngoscopic or phonatory featu

Congenital contractural arachnodactyly with neurogenic muscular atrophy: case report.

We report the case of a 3-(1/2)-year-old girl with hypotonia, multiple joint contractures, hip luxation, arachnodactyly, adducted thumbs, dolichostenomelia, and abnormal external ears suggesting the diagnosis of congenital contractural arachnodactyly (CCA). The serum muscle enzymes were normal and the needle electromyography showed active and chronic denervation. The muscle biopsy demonstrated active and chronic denervation compatible with spinal muscular atrophy. Analysis of exons 7 and 8 of survival motor neuron gene through polymerase chain reaction did not show deletions. Neurogenic muscul

Dysregulation of Tweak and Fn14 in skeletal muscle of spinal muscular atrophy mice.

BACKGROUND: Spinal muscular atrophy (SMA) is a childhood neuromuscular disorder caused by depletion of the survival motor neuron (SMN) protein. SMA is characterized by the selective death of spinal cord motor neurons, leading to progressive muscle wasting. Loss of skeletal muscle in SMA is a combination of denervation-induced muscle atrophy and intrinsic muscle pathologies. Elucidation of the pathways involved is essential to identify the key molecules that contribute to and sustain muscle pathology. The tumor necrosis factor-like weak inducer of apoptosis (TWEAK)/TNF receptor superfamily memb

The muscular pain-fasciculation syndrome.

Five cases of a chronic neuromuscular syndrome consisted of muscular aching and sometimes burning pain, fasciculations, cramps, fatigue, and occasional paresthesia. The disorder affected the legs and, less commonly, the girdle, trunk, and arm muscles. The symptoms were enhanced by physical activity and were usually improved by rest. Neither muscular wasting nor weakness was found, although the condition was present for an average of 4.7 years and, in one patient, as long as 10 years. Electrophysiologic studies showed motor abnormalities indicative of axonal degeneration and muscle fiber denerv

X-linked recessive bulbospinal neuronopathy (Kennedy's syndrome): a neurophysiological study.

We examined 8 men with X-linked recessive bulbospinal neuronopathy. Quantitative electromyography showed large amplitude motor unit action potentials of prolonged duration and increased polyphasia. There was a pronounced loss of motor units in all but one muscle at maximal volition even in muscles with normal or only mildly decreased force. Denervation activity was present in 53% sampled limb muscles, and fasciculation was recorded in 33% of limb muscles. Nerve conduction studies showed small amplitude sensory action potentials in 6 out of 8 patients. The motor and sensory conduction velocity

Concentric needle jitter analysis of the genioglossus muscle in patients with motor neuron disease.

OBJECTIVES: Difficulty relaxing the genioglossus muscle makes the evaluation of spontaneous activity problematic in patients with motor neuron disease (MND). We performed jitter analysis using conventional disposable concentric needle electrodes (CNEs) of the voluntarily activated genioglossus muscle in patients with and without MND to detect the denervation-reinnervation process. METHODS: CNE jitter analysis was performed at the genioglossus muscle in 21 MND(+) patients and 22 MND(-) subjects. The jitter analysis was considered abnormal if the jitter values exceeded these limits for the mean

Titin-based mechanosensing modulates muscle hypertrophy.

BACKGROUND: Titin is an elastic sarcomeric filament that has been proposed to play a key role in mechanosensing and trophicity of muscle. However, evidence for this proposal is scarce due to the lack of appropriate experimental models to directly test the role of titin in mechanosensing. METHODS: We used unilateral diaphragm denervation (UDD) in mice, an in vivo model in which the denervated hemidiaphragm is passively stretched by the contralateral, innervated hemidiaphragm and hypertrophy rapidly occurs. RESULTS: In wildtype mice, the denervated hemidiaphragm mass increased 48 ± 3% after 6

Use of dexamethasone with TTX block of nerve conduction shows that muscle membrane properties are fully controlled by evoked activity.

This paper provides further evidence that motorneurons control extrajunctional properties of skeletal muscles through the activity evoked in the muscle fibres. The experiments compare the amount of action potential resistance to tetrodotoxin (TTX resistance) in denervated soleus muscle with that in soleus whose nerve was crushed and then allowed to regenerate in the presence of a block of the sciatic impulse conduction. Measurements were taken after about 2-3 weeks to allow full reinnervation and recovery of trophic regulation by the nerve. Blocking sciatic impulse conduction with TTX solution

The use of local anaesthetics to produce prolonged motor nerve block in the study of denervation hypersensitivity.

1. Silastoseal cuffs containing 25% and 45% lignocain, 25% marcain and 25% NaCl were placed onto the sciatic nerve. The effects of this on the sciatic nerve as well as the soleus and extensor digitorum longus (EDL) muscles was studied. 2. Cuffs containing local anaesthetics caused both early and late phases of paralysis, cuffs containing NaCl caused only the late phase of paralysis, showing that the late phase of paralysis was not due to the anaesthetic properties of the drugs. The paralysis produced by these procedures was irreversible. 3. Responses to indirect and direct stimulation of soleu

Supersensitivity of the isolated guinea-pig vas deferens induced by a toxic fraction of scorpion venom (Tityus serrulatus).

Tityustoxin (TsTx), a toxic fraction of Tityus serrulatus venom, was studied on the isolated guinea-pig vas deferens. It increased significantly the maximal response of the preparation to both norepinephrine and acetylcholine and decreased the effective median dose of norepinephrine. The effect of TsTx on norepinephrine median dose was unchanged when atropinized or pharmacologically "denervated" preparations were used but was abolished when both procedures were associated. Atropinization of pharmacologically denervated muscles almost never modify the TsTx-induced increase in the maximal respon

Motor deficit in a tauopathy model is induced by disturbances of axonal transport leading to dying-back degeneration and denervation of neuromuscular junctions.

Several neurodegenerative diseases are characterized by both cognitive and motor deficits associated with accumulation of tau aggregates in … mice, but their maintenance was disturbed in adult Tg30 mice, resulting in a progressive and severe muscle denervation. This muscle denervation was associated with early electrophysiological signs of muscle spontaneous activities and histological signs of muscle degeneration. Early loss of synaptic vesicles in axon terminals preceding motor deficits, accumulation of …

Muscle breakdown and lysosomal activation (biochemistry).

Muscle tissue levels of lysosomal catheptic enzymes, such as cathepsins D, A, B1, C, and dipeptidyl peptidase II, were measured in control subjects and patients with muscular dystrophies, polymyositis, and certain denervating diseases. The results show that, in general, the activities of these enzymes are increased in muscles of patients with muscular dystrophies and other diseases. The increases in cathepsin D and autolytic activities are not significant until the late stage of the disease process. Cathepsins A, B1, and C are, however, significantly elevated in mildly affected dystrophic and

S-allyl cysteine: A potential compound against skeletal muscle atrophy.

BACKGROUND: Oxidative stress is crucial player in skeletal muscle atrophy pathogenesis. S-allyl cysteine (SAC), an organosulfur compound of Allium sativum, possesses broad-spectrum properties including immuno- and redox-modulatory impact. Considering the role of SAC in regulating redox balance, we hypothesize that SAC may have a protective role in oxidative-stress induced atrophy. METHODS: C2C12 myotubes were treated with H2O2 (100 μM) in the presence or absence of SAC (200 μM) to study morphology, redox status, inflammatory cytokines and proteolytic systems using fluorescence microscopy, bio

The effect of reinnervation on force production and power output in skeletal muscle.

Failure to fully restore contractile function after denervation and reinnervation of skeletal muscle engenders significant disability in patients suffering peripheral nerve injuries. This work tested the hypothesis that skeletal muscle denervation and reinnervation result in a deficit in normalized power (W/kg), which exceeds the deficit in specific force (N/cm2), and that the mechanisms responsible for these deficits are independent. Adult Lewis rats underwent either transection and epineurial repair of the left peroneal nerve (denervation-reinnervation, n = 13) or SHAM exposure of the perone…

Dietary nitrate supplementation mitigates age-related changes at the neuromuscular junction in mice.

In ageing, denervation and neuromuscular junction (NMJ) instability occur alongside mitochondrial alterations and redox unbalance, potentially playing a significant role in the process. Moreover, the synthetic pathway was shown to be critical for proper innervation and NMJ stability. Nitric oxide (NO) modulates redox status, mitochondrial function and the synthetic pathway. Its bioavailability declines with age. We hypothesize that nitrate supplementation could counteract age-related neuromuscular alterations. We compared young (Y) (7 months old), old (O) (24 months old) and old mice supplemen

Magnetic resonance imaging of lower limb musculature in acute motor axonal neuropathy.

Magnetic resonance imaging (MRI) is an extremely useful technique to diagnose muscle denervation. This report presents an acute motor axonal neuropathy (AMAN) patient in whom, over 2 years, serial clinical-electrophysiological evaluation and MRI examination of lower limb musculature were performed. A 74-year-old patient was admitted with a 24-h history of ascending weakness culminating a few days later in flaccid and areflexic tetraplegia, which was followed by progressive improvement except for severe residual paresis of foot flexors/extensors. Electrophysiological studies showed motor nerve …

Electrical stimulation of transplanted motoneurons improves motor unit formation.

Motoneurons die following spinal cord trauma and with neurological disease. Intact axons reinnervate nearby muscle fibers to compensate for the death of motoneurons, but when an entire motoneuron pool dies, there is complete denervation. To reduce denervation atrophy, we have reinnervated muscles in Fisher rats from local transplants of embryonic motoneurons in peripheral nerve. Since growth of axons from embryonic neurons is activity dependent, our aim was to test whether brief electrical stimulation of the neurons immediately after transplantation altered motor unit numbers and muscle proper
