Implantation mycoses: A nationwide survey on diagnostic and treatment modalities in Nepal.

Implantation mycoses (IM) are a group of fungal diseases which occur after a transcutaneous trauma. The World Health Organization has listed some of the IM, namely sporotrichosis, chromoblastomycosis and eumycetoma as one of the Skin Neglected Tropical Diseases targeted for control by 2030. There are no robust data on IM from Nepal since these diseases are not a part of routine disease surveillance. In this study, an online and in-person survey was conducted using a standard set of questionnaires among the registered dermatologists of Nepal. For this survey sporotrichosis, chromoblastomycosis …

Systemic endemic mycoses: From a geographical risk to a concern expansion.

Blastomycosis, coccidioidomycosis, histoplasmosis, paracoccidioidomycosis, emergomycosis, and talaromycosis are sapronoses restricted to specific geographical areas associated with particular environmental niches that can lead to high morbidity and mortality if diagnosed or treated late. Challenges in clinical awareness arise from their nonspecific clinical presentations and the limited availability of sensitive diagnostic tools. Although advances in molecular diagnosis and antigen detection-based tests are promising, they are not available for all systemic endemic mycoses (SEM). Treatment gui…

The Geographic Distribution of Dimorphic Mycoses in the United States for the Modern Era.

BACKGROUND: The dimorphic mycoses (DMs) of the United States-Histoplasma, Coccidioides, and Blastomyces-commonly known as endemic mycoses of North America (in addition to Paracoccidioides) are increasingly being diagnosed outside their historical areas of endemicity. Despite this trend, the maps outlining their geographic distributions have not been updated in more than half a century using a large, nationwide database containing individual-patient-level data. METHODS: This was a retrospective analysis of >45 million Medicare fee-for-service beneficiaries from 1 January 2007 through 31 Decemb…

[Mycoses due to Rare Moulds in Our Country: A Systematic Review].

An increase is observed in the frequency and diversity of fungal infections in the world and in our country. Improving the quality of patient care in infections due to rare moulds depends on early diagnosis and appropriate treatment. Raising awareness about these infections will facilitate taking the necessary steps for diagnosis and treatment in similar cases. In addition to 165 cases out of 96 studies included in this review article, 28 studies reporting rare mould isolation with limited case information were examined. The number of studies reporting cases that meet the criteria has increase

The Growing Problem of Antifungal Resistance in Onychomycosis and Other Superficial Mycoses.

Superficial mycoses are becoming increasingly resistant to current antifungal medications. As alternative therapeutic options are limited, the increasing frequency of reports of antifungal resistance is alarming. This epidemic parallels the rise of antibiotic resistance; however, the significance of this problem has yet to gain global attention. Here, we discuss the reports of antifungal resistance from around the world, present our own experience with treatment-resistant infections, and examine alternative treatment strategies. The majority of reports of recalcitrant infections indicate terbi…

Systemic mycoses: a potential alert for complications in COVID-19 patients.

As the global COVID-19 pandemic spreads worldwide, new challenges arise in the clinical landscape. The need for reliable diagnostic methods, treatments and vaccines for COVID-19 is the major worldwide urgency. While these goals are especially important, the growing risk of co-infections is a major threat not only to the health systems but also to patients' lives. Although there is still not enough published statistical data, co-infections in COVID-19 patients found that a significant number of patients hospitalized with COVID-19 developed secondary systemic mycoses that led to serious complica…

[Therapy of invasive organ mycoses in patients with systemic hematologic diseases].

Fungal infections have increased substantially in patients with acute leukemia as well as in patients receiving allogeneic stem cell transplantations. Most frequently Aspergillus ssp. and Candida ssp. are observed. Despite the recent introduction of new azoles and lipid-based formulations of amphotericin B, there are few randomized, controlled studies on the use of antifungal drugs in patients with proven invasive fungal infections. Conventional Amphotericin B is considered gold standard for the treatment of invasive fungal infections, however is limited by nephrotoxicity and infusion related

The role of murine models in the development of antifungal therapy for systemic mycoses.

Animal testing is crucial to the development of new antifungal compounds. This review describes the role that murine and other animal models have played in the development of three classes of antifungal agents: the polyenes, the triazoles and the echinocandins and the ways in which these models have been either the positive link in the path from in vitro studies to the patient, or have foreclosed later clinical evaluation. Efficacy studies in particular mycoses are discussed, as well as studies designed to determine whether combinations of antifungal drugs may have value over single agents. Co…

[Epidemiology of invasive mycoses. Experience of a university hospital center in Paris].

The incidence of opportunistic fungal infections have dramatically increased during the past decades. Data from a retrospective study conducted between January 1992 and December 1994 in Necker-Enfants Malades Hospital and a review of the literature were analyzed to assess the main epidemiological features of these infections. Candidiasis remained largely the most frequent: candidemia and invasive candidiasis were diagnosed in respectively 71 and 11 patients. Candida albicans was the agent the most frequently isolated from these infections. However, its percentage was not over 49.35% in our exp

Successful treatment of subcutaneous mycoses with fluconazole: a report of two cases.

Lymphocutaneous sporotrichosis and chromoblastomycosis are subcutaneous mycoses caused by traumatic implantation of the fungus into the skin. Medical treatments for chromoblastomycosis has been disappointing, while lymphocutaneous sporotrichosis usually responds well to iodides. Here we present a case of chromomycosis and a case of lymphocutaneous sporotrichosis. Both patients were treated successfully with fluconazole.

The growing problem of mycoses in patients infected with the human immunodeficiency virus.

With the rapid increase in cases of AIDS over the past 10 years, certain mycoses have dramatically risen in frequency, particularly those contained by T cell-mediated mechanisms of host defense. In this clinical setting mucocutaneous candidiasis as well as systemic cryptococcosis, histoplasmosis, and coccidioidomycosis pose special diagnostic and/or therapeutic challenges. Compared with fungal infections in general, AIDS-associated mycoses are more likely to have nonspecific clinical manifestations; moreover, treatments effective in other settings seldom are curative. These problems have led t…
