In order to evaluate the usefulness of thallium-201 (201TI) myocardial scintigraphy in delineating the location and size of prior myocardial infarction, 32 patients were evaluated at a mean of 7 +/- 2 months after infarction with a 12-lead ECG, resting 201TI myocardial scintigram, biplane left ventriculogram and coronary angiograms. BACKGROUND: As an alternative to cardioplegic arrest, cardiac surgical conditions have been produced using beta-blocker-induced minimal myocardial contraction (MMC) during cardiopulmonary bypass. The degree of inward motion at mid-ejection was examined for seven segments on the silhouettes of left ventriculograms taken in the 30 degrees RAO projection in patients with normal coronary arteries. The pattern of wall motion described in these patients was used to distinguish abnormalities in mid-systolic wall motion. BACKGROUND: Postoperative ejection phase parameters and patient survival rates for mitral valve replacement surgery are considerably lower than for similar aortic valve surgery. While chordal transection probably is the major contributor to the lowered values, there is also evidence for decreased preoperative myocardial contractile reserve in mitral regurgitation patients. This study characterizes abnormalities in the force-frequency relation that may underlie impaired function of myocardium isolated from mitral regurgitation patients with New York Heart Association class II-III heart failure.

A 64-year-old man presented with several weeks of intermittent irregular palpitations. He had no prior history of cardiac disease, hypertension or syncope. A 12-lead ECG revealed sinus rhythm with premature atrial and ventricular contractions and high QRS voltages consistent with LV-hypertrophy. Cardiac MR revealed asymmetrical septal hypertrophy and marked mid-myocardial hyperenhancement of the interventricular septum.

Left ventricular contractility, central hemodynamics and a clinical course of acute myocardial infarction were investigated in inpatients on systemic thrombolytic therapy and controls. This treatment is shown to diminish the risk of acute left ventricular failure, ventricular fibrillation and atrioventricular block. Furthermore, it prevents inhibition of left ventricular contractility in acute period of the infarction. Subsequently, noticeable differences between the test group and controls disappeared.
