Myositis ossificans of the medial pterygoid muscle.

Myositis ossificans (MO) is a disease whose main feature is the formation of heterotopic bone involving muscle or any other soft tissue. Pathogenesis is not all clear, but trauma is universally recognized as a trigger event. Myositis ossificans can be divided into 2 classifications: MO progressiva and MO traumatica. The authors present a case of a 64-year-old man who was diagnosed with MOT of the left medial pterygoid muscle. The patient had a long clinical history of signs and symptoms of temporomandibular articulation related to previous conditions, and no major traumatic events were reporte…

[Myositis as differential diagnosis in polymyalgia rheumatica].

An 82-year-old man was diagnosed with polymyalgia rheumatica. Rapid improvement was achieved after peroral prednisolone administration. Two years later while being treated with low-dose prednisolone, the acute phase reactants increased, now also with proximal muscle weakness, weight loss and elevated muscle enzymes. Needle biopsy confirmed dermatomyositis. Polymyalgia rheumatica is a common disease among the elderly. It has several differential diagnoses including dermatomyositis and polymyositis.

Myositis ossificans traumatica of the medial pterygoid.

Myositis ossificans traumatica (MOT) is a non-neoplastic, heterotopic bone formation within muscle or fascia, presumably due to acute trauma, a burn, surgical manipulation, or repeated injury. A 36-year-old woman presented with progressive limitation of mouth opening over the past 5 months. Computed tomography revealed an irregular ossified mass attached to the medial aspect of the left ramus, extending to the medial and lateral pterygoid plates. MOT of the medial pterygoid is rare, with only four previously reported cases in the English language literature. Surgical excision of the ossificati…

Review article: the gastrointestinal complications of myositis.

BACKGROUND: The inflammatory myopathies are a group of acquired diseases characterized by a proximal myopathy caused by an inflammatory infiltrate of the skeletal muscle. The three major diseases are dermatomyositis, polymyositis and inclusion body myositis. AIMS: To review the gastrointestinal manifestations of myositis. METHODS: Over 110 articles in the English literature were reviewed. RESULTS: Dysphagia to solids and liquids occurs in patients with myositis. The pharyngo-oesophageal muscle tone is lost and therefore patients develop nasal speech, hoarseness, nasal regurgitation and aspi…

[Diagnosis and differential diagnosis of granulomatous myositis].

Granulomatous myositis is a rare neuromuscular disorder histologically characterized by the development of endomyseal and/or perimyseal granulomas. Clinical hallmarks are generalized muscle weakness, myalgias, and bulbar symptoms. The association of granulomatous myositis with sarcoidosis is well known; less recognized is the association with several infectious diseases, inflammatory bowel diseases, malignancy, thymoma, graft-vs-host disease, and myasthenia gravis. In absence of sarcoidosis or other underlying disorders, the diagnosis of isolated or primary granulomatous myositis must be consi…

Is it really myositis? A consideration of the differential diagnosis.

PURPOSE OF REVIEW: The idiopathic inflammatory myopathies are an important and treatable group of disorders. However, the potential toxicity … has been debate over the requirements for an accurate diagnosis of inflammatory myopathy (i.e., polymyositis and dermatomyositis). It is increasingly recognized that there can be clinical and muscle histopathologic overlap between the features of inflammatory myopathies and those of other muscle disorders, in particular, the genetic muscular dystrophies. P …

[A perspective on the control of myositis ossificans progressiva].

OBJECTIVE: To report four cases of myositis ossificans progressiva, its form of presentation, in addition to new control and treatment options. DESCRIPTION: The authors describe four cases of myositis ossificans progressiva, presenting its clinical manifestations, evolution, and available forms of control and treatment. The main congenital anomalies associated with myosistis ossificans progressiva are described, namely: hallux valgus, clinodactyly, short thumbs, exostosis, and syndactyly. The evolution and therapeutic responses of clinical improvement to oral ascorbic acid and biphosphonate in…

Traumatic myositis ossificans of the quadriceps in infants.

Acute traumatic myositis ossifications is uncommon and usually occurs in adolescents and young adults after a significant direct blow to the affected muscle. It is extremely rare in infants, and we have been able to find only two other cases in the English literature. We present two cases of traumatic myositis ossificans in infants who, except for the use of diagnostic ultrasonography, would have been misdiagnosed as having osteomyelitis (Patient 1) and a possible malignancy (Patient 2).

Tongue involvement in a patient with granulomatous myositis.

A 64-year-old woman had difficulty swallowing and talking, weakness of the tongue, and progressive muscle weakness that was proven to be caused by a granulomatous myopathy. This case resembled a motor neuron disease, indicating that granulomatous myopathy should be considered in patients with similar presentations, since it is a treatable condition with a more benign prognosis.

[Myositis--a rare complication of Crohn disease].

Crohn's disease is a chronic relapsing inflammatory disorder primarily affecting the gastrointestinal system. Extraintestinal manifestations such as erythema nodosum, iridocyclitis, arthritis and sclerosing cholangitis are frequent. We report on a 57-year old male patient with the symptoms of Crohn's disease. Clinically he showed signs of a muscle disorder and muscle biopsy demonstrated a perimysial inflammation with lymphocytes and histiocytes. Complete remission was achieved by immunosuppressive treatment. Six cases presented in the literature are reviewed and the autoimmunological aspects o

Structures targeted by the immune system in myositis.

Polymyositis and dermatomyositis are characterized by the production of a series of autoantibodies to various cellular constituents. Some of these autoantibodies are found specifically in patients with polymyositis or dermatomyositis or myositis overlap syndrome and predict clinical subgroups and prognosis. If we are to understand the etiology and pathogenic mechanisms of polymyositis and dermatomyositis, it will be particularly important to elucidate the structure and function of target autoantigens recognized by these myositis-specific autoantibodies. In recent years, many autoantigens and s…

Necrotizing fasciitis and myositis: a case report.

Necrotizing fasciitis is a relatively rare, potentially life-threatening infection involving the subcutaneous tissues. We report a case of group A streptococcal necrotizing fasciitis/myositis in which CT played an important role in differential diagnosis.

Useful investigations in a refractory case of 'myositis'.

The diagnosis of polymyositis is made on the basis of clinical features, muscle enzymes, EMG studies and muscle biopsy. Treatment requires corticosteroids often in combination with immunosuppressive agents, and relapse in spite of treatment may render management difficult. The case described below appeared to demonstrate features from which the diagnosis could be made, but in which the almost complete resistance to conventional therapy led to a correct reappraisal of the diagnosis. This illustrates an unusual cause for diagnostic confusion and the value of diagnosis reassessment under such cir…

[Localized nodular myositis--a variation of polymyositis].

A case of focal nodular myositis is reported. The patient was a 37-year old male with no prior medical history. He had three episodes of focal nodular myositis over a three month period, recovering completely after each attack. During the last attack there were three lesions in the left quadriceps and one in the biceps muscle of the left arm. Histologically there was severe myositis. After the last attack he has been well for 12 months without sequelae.
