Emerging evidence of occipital nerve compression in unremitting head and neck pain.

Unremitting head and neck pain (UHNP) is a commonly encountered phenomenon in Headache Medicine and may be seen in the setting of many well-defined headache types. The prevalence of UHNP is not clear, and establishing the presence of UHNP may require careful questioning at repeated patient visits. The cause of UHNP in some patients may be compression of the lesser and greater occipital nerves by the posterior cervical muscles and their fascial attachments at the occipital ridge with subsequent local perineural inflammation. The resulting pain is typically in the sub-occipital and occipital loc…

The burden of chronic pain after major head and neck tumor therapy.

INTRODUCTION: Little is known about the burden of chronic pain after major head and neck tumors' therapy. In this study, we aimed to estimate the prevalence of chronic pain, explore the factors associated with the presence of chronic pain, and assess the consequences of chronic pain on the patients' quality of life. METHODOLOGY: This was a cross-sectional survey among patients who had completed their therapy (e.g., surgery, radiotherapy, and chemotherapy) for major head and neck (larynx, nasopharynx, oropharynx, hypopharynx, oral cavity, tongue, and sinuses) tumors after at least 3 months. We

Physical characteristics associated with neck pain and injury in rugby union players.

BACKGROUND: Neck pain and injury are common in rugby union. Physical characteristics predisposing players to neck injury are largely unknown. This study aimed to determine physical characteristics associated with neck pain and injury in rugby union players. METHODS: Semi-professional rugby union players (N.=142) underwent pre-season measurements including cervical active range of motion (AROM), strength, sensorimotor proprioception (joint position error), and anthropometry. A structured interview established previous neck injury history, current symptoms, playing position, competition level, …

Measurement of outcomes for patients with centralising versus non-centralising neck pain.

OBJECTIVE: The purpose of this study is to determine whether individuals with neck pain who demonstrate centralisation of symptoms have more favourable outcome than individuals who do not demonstrate centralisation. METHODS: Eleven subjects with neck pain were evaluated and treated by two physical therapists certified in Mechanical Diagnosis and Therapy (MDT). Eleven physical therapy patients underwent a routine initial evaluation and were treated 2-3 times per week using MDT principles and other physical therapy interventions. The Neck Disability Index (NDI) tool was administered at the init…

Migraine co-existing tension-type headache and neck pain: Validation of questionnaires.

Aabstract Background and aim Migraine often includes co-existing tension-type headache (TTH) and neck pain (NP). Multiple headache questionnaires assessing headache impact have beendescribed previously; however, none of the existing questionnaires have been designed to cover migraine with co-existing TTH and NP. Therefore a new questionnaire was developed to measure these co-morbidities. The aim was to determine face and content validity of the newly developed questionnaire, "Impact of Migraine, Tension-Type Headache and Neck Pain" (impact M-TTH-NP) and to determine face and content validity o…

Does muscle morphology change in chronic neck pain patients? - A systematic review.

BACKGROUND: Neck pain is a common disabling worldwide health problem with a high socio-economic burden. Changes underlying the transition to, or the maintenance of a chronic state are still barely understood. Increasing evidence suggests that morphological muscle changes, including changes in cross-sectional area (CSA) or fatty infiltration, play a role in chronic neck pain. However, a structured overview of the current evidence of morphological changes is lacking. OBJECTIVE: To systematically review the morphological muscle changes in patients with chronic neck pain, including those with whi…

Neck and back pain and intervertebral disc degeneration: role of occupational factors.

Back pain is a near-universal human experience at some time during life, and neck pain is also common. The overwhelming majority of low back and cervical pain is considered to be due to unspecified mechanical factors or disc degeneration, which is a common with ageing and, hence, in people of working age. Back pain and disc disease appear to have significant heritability, based upon twin studies, but environmental factors also contribute - including physical occupational activities in some studies - although the strength of this association remains uncertain. This article examines the contribu…

Cerebrospinal fluid from a 6-year-old dog with severe neck pain.

A 6-year-old, intact female, Labrador Retriever/Terrier cross was presented to the University Veterinary Hospital, University College Dublin with a 3-week history of therapy-resistant cervical pain and intermittent fever. Physical examination findings included marked cervical pain resulting in neck extension and vocalization. Examination of the CSF revealed mild pleocytosis (total nucleated cells = 0.009 x 10(9)/L, reference interval <0.005 x 10(9)/L). Cytocentrifuged preparations of the CSF were of low cellularity, containing predominantly macrophages and occasional small lymphocytes. Several

Multidisciplinary biopsychosocial rehabilitation for neck and shoulder pain among working age adults.

BACKGROUND: Multidisciplinary biopsychosocial rehabilitation programs for neck and shoulder pain require substantial staff and financial resources. Despite questionable scientific evidence of their effectiveness, they are widely used. Neck and shoulder complaints are common among working age adults and they are often associated with physical work load and stress. Pain in the neck and shoulder area cause biopsychosocial difficulties for the patient, especially if disability due to pain is prolonged. To help patients with biopsychosocial problems, or to prevent their development, multidisciplina

Headache in patients with neck-shoulder-arm pain of cervical radicular origin.

In a series of 81 patients with chronic cervicobrachialgia, 54 (67%) reported that they also suffered from recurrent headache. Forty-four (81%) of these patients were classified as having cervical headache, 5 as having migraine, 2 with tension-type headache, and 3 patients were not classifiable according to the diagnostic system of the International Headache Society (IHS). Patients with headache presented significantly higher tenderness scores and pain intensity in the neck-shoulder-arm region than patients without headaches. Twenty-three (52%) of the 44 patients with cervical headache reporte

[Pain syndromes of the head, neck and locomotor system--determining current status].

A small series of pain syndrome patients shows that disturbances of the head and neck motor system can lead to various pain syndromes as the vicious circle between pain and muscle tension is initiated by a triggering factor. These pain syndromes include varying combinations of the following symptoms: headache, referred otalgia, arthralgia of the temporomandibular joint, styloid syndrome, tendopathia of the hyoid bone, carotidynia, cervical dysphagia and probably most patients with superior laryngeal nerve neuralgia or glossopharyngeal neuralgia. A detailed differentiation of those syndromes is

Association between cervical sagittal alignment, neck awareness, and disability in chronic neck pain: The possible role of probable sleep bruxism.

BackgroundChronic neck pain (CNP) is a major cause of disability. Cervical sagittal alignment, neck awareness, and probable sleep bruxism may influence symptom burden.MethodsThis cross-sectional study included 76 patients (18-60 years) with CNP. Probable sleep bruxism was classified using self/partner report and clinical findings. Pain was rated on a visual analog scale (VAS), neck awareness by the Fremantle Neck Awareness Questionnaire-Turkish (FreNAQ-T), and disability by the Neck Disability Index (NDI). Lateral radiographs provided C0-2 and C2-7 Cobb angles, cervical sagittal vertical axis …

Differences in Cervical Muscle Activity of Pilots and Nonpilots With and Without Neck Pain.

INTRODUCTION: Neck pain is common and often persistent, especially among military pilots exposed to high physical demands during flight. These conditions may alter neuromuscular control, contributing to chronic symptoms. While surface electromyography can detect pain-related changes in muscle activity, little is known about these patterns during low-load exercises in pilots. This study identifies differences in cervical muscle activity between pilots and nonpilots with and without neck pain. METHODS: A total of 18 military pilots and 23 control subjects were categorized into symptomatic and a…
