Intracranial epidermoid cysts are developmental central nervous system abnormalities that arise from erroneous ectodermal inclusion during neural tube closure. Epidermoids are relatively common, representing 1% of all intracranial masses, and are particularly predominant in the cerebellopontine angle (CPA), where they are the third most prevalent diagnosis after schwannomas and meningiomas. Although small, asymptomatic, or incidental lesions may be safely observed, in the presence of symptomatic mass effect, epidermoid cysts typically require surgical resection. As shown in Video 1, we demonst

Spina bifida is a static congenital neural tube disorder resulting in continuing evolving disease involving multiple organ systems. Those affected require lifelong surveillance with coordinated management involving the patient, parents, general practitioners, neurologists, physiotherapists, occupational therapists, social workers, nurses, neurosurgeons, urologists, orthopaedic surgeons, physicians and other health professionals.

The subspecialty of craniofacial surgery emphasizes skeletal exposure, preservation of critical structures, and provision of a superior cosmetic result. In recent decades, an emphasis on minimally invasive neurosurgical exposure has paved the way for increased collaboration between neurosurgeons and craniofacial surgeons. The 1990s saw the growing popularity of an eyebrow incision for orbital roof craniotomies in neurosurgery to address lesions in the anterior skull base. Disadvantages of this approach included conspicuous scarring above the brow skin, risk of injury to the frontal branch of th…

His seminal research continues to impact neurologists, endocrinologists, and neurosurgeons, and enables them to treat patients who had no alternatives prior to du Vigneaud's breakthroughs in peptide structure and synthesis. OBJECTIVE: To survey the management of head-injured patients in 1997 and to identify differences compared with a survey conducted in 1991.
DESIGN: A two-page questionnaire was mailed to all neurosurgeons in North America certified by the American Board of Neurologic Surgeons, asking their views regarding the most appropriate acute care of patients with severe traumatic brain injury (TBI).
SETTING: North American neurosurgical practices.
PATIENTS: Not applicable.
INTERVENTIONS: None.
MEASUREMENTS AND MAIN RESULTS: Compared with a 1991 survey, there was a significant increase in the proport…

TARGET POPULATION: These recommendations apply to adult patients diagnosed with progressive glioblastoma (pGBM). QUESTION (Q1): In adult patients with pGBM does the use of temozolomide (TMZ) with alternative dosing or the use of TMZ in combination with other cytotoxic treatments result in increased overall survival compared to other chemotherapy?
RECOMMENDATION: Level III: Adult patients with pGBM might derive benefit in treatment with TMZ, especially those who progress after more than 5 months of TMZ-treatment free interval.
LEVEL III: Combination of TMZ with other cytotoxic agents such as

We report a case of a hydatid cyst of the eyelid in a 12-year-old boy associated with cerebral involvement. The patient was initially treated by neurosurgeons for brain cysts. The course after an interval of two months was marked by regression of the palpebral cyst on albendazole.

Hemorrhages of brainstem cavernomas may cause severe neurological deficits. Surgical strategies are frequently described, and advanced neuromonitoring with intraoperative imaging can help neurosurgeons to achieve good results. However, patients are often confronted with significant therapeutic risks by the primary doctor before talking to an experienced brainstem neurosurgeon. On the other hand, lethal progression with repeated hemorrhages is rarely described, although many would agree on this possibility by experience or assumption. Our reported case represents the natural development of a pa…

Schilder's myelinoclastic diffuse sclerosis is a rare demyelinating disease, which often mimics intracranial neoplasm or abscess. This disease is of importance to the neurosurgeon in that the clinical manifestations and course may closely simulate brain tumour, including the development of papilloedema and other signs of increased intracranial pressure. We have treated a 29-year-old right-handed young man presenting with left hemiplegia, aphasia and behavioural changes. Magnetic resonance imaging showed two large lesions, irregular contrast-enhancing, in the subcortical white matter of the rig

STUDY DESIGN: A systematic literature review and consensus using Delphi method.
OBJECTIVE: This review aims to provide an overview on Chiari malformation in pediatric patients, highlighting the specific clinical manifestations and surgical treatment options.
SUMMARY OF BACKGROUND: Chiari malformation in children presents a real difficulty to the general neurosurgeon because children are not smaller adults. In the absence of pediatric neurosurgeons, as in many countries of the world, a need for education of general neurosurgeons on the management of Chiari malformation in children was identif…

BACKGROUND: Papua New Guinea (PNG) is a developing Pacific Nation of 7.3 million people. Although neurosurgery training was introduced to PNG in the year 2000, it was in 2003 that a neurosurgery service was established. Prior to this time, neurosurgery in PNG was performed by general surgeons, with some assistance from visiting Australian neurosurgeons. Neurosurgical training was introduced to PNG in 2000. The model involved a further 3 years of training for a surgeon who had already completed 4 years of general surgical training. We aim to review the output, outcomes and impact achieved by tr…

OBJECTIVE: A survey-based study was designed to assess opinions related to transsphenoidal endoscopy, to evaluate the current global attitudes of neurosurgeons regarding the use of this procedure, and to solicit basic data from key institutions worldwide, with the advantage that large series of cases become available for analysis.
METHODS: A web-based multi-item questionnaire was distributed to the surgical departments of 393 neurosurgical centers by means of an invitation e-mail. The questionnaire was composed of 2 sections: section 1 focused on the surgical technique for pituitary adenomas …

During the last decade there has been a remarkable increase in the interest of neurosurgeons in surgical approaches along the base of the skull. A plethora of different approaches, most of which involve a slight variation of a previously described approach, have been reported in the recent literature, frequently with proprietary claims of originality. This increased confidence of the neurosurgeon in approaching lesions along the base is due to our renewed interest in the anatomy of the skull base and our willingness to collaborate with some of our colleagues in otorhinolaryngology who have con…

Ventral brainstem compression is an uncommon clinical diagnosis seen by pediatric neurosurgeons and associated with Chiari malformation, type I. Presenting clinical symptoms often include headaches, lower cranial neuropathies, myelopathy, central sleep apnea, ataxia, and nystagmus. When ventral decompression is required, both open and endoscopic transoral/transnasal approaches are highly effective.

BACKGROUND: An intrathoracic meningocele is a relatively rare disease, and it commonly accompanies neurofibromatosis type 1. Patients tend to have no symptom but if its size is too large and compresses a lung and neighboring organs, it needs shunt drainage or surgical resection.
CASE PRESENTATION: Herein, we present the case of a 52 year-old female patient with huge intrathoracic meningoceles associated with neurofibromatosis type 1, who has complained about chest discomfort and dyspnea at rest. As for a preliminary treatment, a neurosurgeon had performed a cystoperitoneal shunt, but the symp

Seventy-two lesions were demonstrated in 70 patients who were examined using intraoperative real-time ultrasonography. Fifty-five lesions were supratentorial and 17 were infratentorial. The neurosurgeon used ultrasonography to evaluate his operative field before, during, and after resection. Nineteen lesions contained cystic components. In eight cases, ultrasonography added information not provided by CT including definitive characterization of cystic components and loculations or localization of solid masses within the cysts. Biopsy of 13 lesions was performed under ultrasonic guidance. In al
