White sponge nevus in a patient with EEC syndrome.

A 10-year-old boy, affected by cleft lip and palate and the Ectrodactyly-Ectodermal Dysplasia-Clefting (EEC) syndrome, presented with hypodo … bundles of skeletal striated muscle fibers, and areas of hemorrhage at deeper regions. White sponge nevus was diagnosed. To our knowledge, no other case of white sponge nevus has been reported in a patient with EEC syndrome. Considering the low prevalence of this disorder and also of the EEC syndrome, future studies on the etiology of the white sponge n …

Spitz nevus: a clinicopathological study of 349 cases.

Spitz nevus is an infrequent, usually acquired melanocytic nevus composed of epithelioid and/or spindle melanocytes that can occasionally be confused with melanoma. Currently, there are no immunohistochemical markers or molecular biology techniques that can be used to make an entirely safe diagnosis of Spitz nevus or melanoma in problematic cases. A retrospective study has been carried out that included all the cases diagnosed as Spitz nevus from our files. Follow-up information of the patients was unavailable. Three hundred forty-nine cases of unequivocal Spitz nevi were included, and their c…

Giant ceribriform intradermal nevus on the back of a newborn.

A male newborn had a large cerebriform tumor covering his shoulders and almost the entire surface of his back. After exclusion of further abnormalities, the diagnosis of cerebriform intradermal nevus was made. This particular variant of giant melanocytic nevus should always be differentiated from cutis verticis gyrata, if located on the vertex. The clinical manifestation of cerebriform intradermal nevi as giant melanocytic nevi on the back is extremely rare, with only one instance reported to date. Such nevi are a therapeutic challenge, particularly if the skin lesion covers a large surface of…

Treatment of Nevus comedonicus with topical tazarotene and calcipotriene.

Nevus comedonicus is a rare developmental defect of the pilosebaceous unit. It is also thought to be a variant of epidermal nevus. Previously reported treatments include surgical excision, CO2 laser, dermabrasion, extraction, topical retinoic acid, and numerous topical keratolytics. We present a case of a 7-year-old boy with bilateral nevus comedonicus who experienced cosmetic improvement with topical tazarotene and calcipotriene cream. This combination represents a novel therapeutic approach to the treatment of this cutaneous abnormality.

Malignant melanoma arising in a sebaceous nevus of the scalp.

This is the first report of malignant melanoma arising from sebaceous nevus (SN) of the scalp, although it has been known that various benign and malignant neoplasms may develop in association with SN. After the excisional biopsy for SN with 5 mm free margin, pathological examination revealed the coexistence of nodular melanoma invading to the reticular dermis (IV level of Clark) for a maximal depth of 4 mm. After resection with another 20 mm free margin, there has been no evidence of local recurrence or distant metastasis for nine years.

MR imaging of the spine in epidermal nevus syndrome.

We report two cases of epidermal nevus syndrome (ENS) involving the spine. MR imaging of the spine demonstrated intraspinal lipomas in both cases. Abnormal, enhancing, enlarged cervical and lumbosacral nerve roots were present in one patient. Spinal imaging for patients with ENS may help in the diagnosis of subtle intracranial manifestations, as it did in both of our cases. ENS has features similar to those of other neurocutaneous syndromes, such as neurofibromatosis type 1 and encephalocraniocutaneous lipomatosis.

Eosinophilic intranuclear inclusion bodies in a melanocytic nevus.

We report a case of eosinophilic or hyaline true intranuclear inclusion bodies in a melanocytic nevus. Although intranuclear pseudoinclusions are frequently found in melanocytic nevi, true intranuclear inclusions are rare. The true intranuclear nature of the inclusions in our case was confirmed with ultrastructural examination. With reverse transcriptase in situ polymerase chain reaction (RT in situ PCR) analysis, eosinophilic bodies stained positive for molluscum-specific primers. This result suggests that such inclusions may be related to molluscum viral infection of melanocytes.

[Pathological findings in the eye of children with facial nevus].

BACKGROUND: Port-wine stains distributed to the trigeminal area may be associated with eye and/or central nervous system complications. Visual loss may be prevented with screening for eye pathology at an early age with adequate intervention. MATERIAL AND METHODS: 45 children with port-wine stains in the trigeminal area were examined by an ophthalmologist under general anaesthesia. RESULTS: Seven of 30 patients with one or both eyelids affected had a choroidal vascular anomaly. Four patients had glaucoma. These four patients had both eyelids affected. Two children with a vascular anomaly had

CNS lipoma in patients with epidermal nevus syndrome.

Epidermal nevus syndrome (ENS) is a congenital neurocutaneous disorder characterized by linear nevus with a significant involvement of the nervous, ophthalmological and skeletal systems. Clinical manifestations of ENS include neurological features such as mental retardation, seizures, and movement disorders which are caused by a wide range of neuropathological lesions. We describe three patients with ENS, all of whom had in addition to the characteristic features of ENS intracranial and/or intraspinal lipomas. In one patient the lipoma extended from the thoracal vertebra 8 to the 4th ventricle…

Sacral nevus flammeus simplex: the role of imaging.

Neonates with midline lumbar, thoracic, or occipital cutaneous lesions should be suspected of having spinal dysraphism and should undergo an imaging study. The aim of the present study was to evaluate whether sacral nevus flammeus simplex (SNFS) in neonates is associated with spinal dysraphism. During a period of 6 months, we prospectively examined all neonates in our center for the presence of SNFS. Those affected were evaluated for associated anomalies by physical and neurologic examination and ultrasound imaging of the lumbosacral area. Twenty-eight of 3623 neonates (0.77%) were diagnosed a…

[Proliferative characteristics of nevus in children with organ transplants].

INTRODUCTION: Organ-graft recipients are at increased risk for developing cutaneous tumors, mostly carcinomas; however, melanocytic nevi (MN) also develop in excess numbers in these patients. The aim of this study was to assess whether MN developing in pediatric organ graft recipients (OGR-MN) have a higher proliferative profile than similar lesions developing in non-immunosuppressed subjects (C-MN), a fact that could confer to them potential for malignant transformation into melanoma. MATERIAL AND METHODS: The immunohistochemical expression of two proliferation-associated markers (MIB1/Ki67

[Pigmented nevus in children as a diagnostic and treatment problem].

Congenital and acquired nevi are common cutaneous malformation occurring in childhood. 315 children were cured in 1990-1995. 62% of nevi did not show any abnormalities of growth pattern and typical appearance. In 119 nevi there were danger signals present, that suggested unusual activity or possible malignant transformation. Changes of pigmentation were present in 36%, enlargement and abnormal texture in 21%, satellite nevi in 11%. Histologically, they were dermal in 28% and epidermodermal in 24%. Only in 3 children melanoma malignum was found, in 2 melanoma juvenile, in 4 dysplastic nevi. All

Oculocerebral dysgenesis in the linear nevus sebaceous syndrome.

PURPOSE: The authors document the association of peripapillary staphyloma and an atypical variant of hemimegalencephaly with the linear nevus sebaceous syndrome. BACKGROUND: Linear nevus sebaceous syndrome is an uncommon neurocutaneous disorder that has a propensity to involve the eyes. METHODS: Clinical, histopathologic, and neuroimaging findings are examined in a child with linear nevus sebaceous syndrome. RESULTS: In addition to bilateral peripapillary staphylomas, ophthalmologic abnormalities included a corneal dermoid, a complex conjunctival choristoma, macular hypoplasia, and optic ne…

Ocular findings in Japanese women with nevus of Ota.

BACKGROUND: Nevus of Ota is common in Japanese women, but most patients are not examined ophthalmologically. METHODS: We performed ophthalmologic examinations on 16 Japanese women who had had bluish pigmentation in the periorbital region, sclera, and conjunctiva since birth. RESULTS: Fifteen patients had unilateral involvement, and one had bilateral lesions. The visual acuities were good, and the intraocular pressures were within normal range. All patients had a negative family history. Three patients had light pigmentation in the optic disc in the affected eye. CONCLUSION: We believe that …

The oral blue nevus in children: a case report.

The intraoral blue nevus is a benign, relatively rare lesion that usually occurs in adults and most often in women. The reported case is interesting because the patient was only 11 years old. The lesion, located in the hard palate, was surgically removed. The specimen was sent for histologic examination, resulting in the final diagnosis of common blue nevus.

Histologic correlations with the clinical diagnosis of dysplastic nevus.

In melanoma kindreds the presence of dysplastic nevi correlates with greatly increased melanoma risk. The relative importance of sporadic, nonfamilial dysplastic nevi as a risk factor for melanoma is less certain. Although the clinical features of dysplastic nevi have been well described, the histologic basis for the diagnosis is not as firmly established. This study examines the degree of correspondence between the clinical and histopathologic diagnosis of dysplastic nevus. Histologic review of nevi with clinical features of dysplasia from 1,000 individuals demonstrated classic histologic fea…
