A systematic review examining nutrition support interventions in patients with incurable cancer.

PURPOSE: Recent guidelines by the European Society for Clinical Nutrition and Metabolism (ESPEN) have advocated increased attention to nutritional support in all patients with cancer; however, little is known about the optimal type of nutritional intervention. The aim of this review was to assess the current evidence for nutrition support in patients with incurable cancer. METHODS: This review was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guidelines. Embase, MEDLINE and CINAHL were searched from 1990 to 2018. Evidence was appr…

Home parenteral nutrition support in adults: experience of a medical center in Asia.

BACKGROUND: Parenteral nutrition (PN) support is mandatory in patients with gut failure. Short bowel syndrome is a term used for symptoms and pathophysiological disorders brought about by the removal or malfunction of a large portion of the small bowel. Inflammatory bowel disease, vascular disease, and malignancy are the most common causes of short bowel syndrome in adults. There are many complications associated with chronic use of PN. Cholestasis, nutrient deficiency, small bowel bacterial overgrowth, and catheter-related infections were noted in these patients. Due to the various etiologies

Immune modulating nutrition support for a patient with severe toxic epidermal necrolysis.

Toxic epidermal necrolysis (TEN) is a rare but serious condition with a high mortality rate, whose clinical features resemble major thermal injury. Nutritional substrates which can affect the immune response have been shown to benefit burned and critically ill patients but there is a lack of data in the TEN literature. A case study is presented of a 63-year-old female admitted to our burn unit with extensive skin and mucosal involvement. Immune modulating artificial nutrition support was given initially via the parenteral route and then the nasogastric route. Maintenance of the patient's nutri

Nutrition support for the patient with an open abdomen after major abdominal trauma.

OBJECTIVE: Nutrition support in the severely injured trauma patient is crucial to minimize the hypermetabolic stress response. Even though enteral nutrition is the preferred method of feeding, it is not always feasible after multiple trauma. We present a complex nutritional case in a patient who sustained severe abdominal trauma with a severe liver injury, rib fractures, and pulmonary contusion. METHODS: The patient required several repeat laparotomies, abdominal packing, and temporary abdominal closure. The clinical course was complicated by hypotension requiring multiple vasopressors; coagu

Effect of nutrition support on immunity in paediatric patients with beta-thalassaemia major.

Nutritional deficiencies have been variably observed in thalassaemia and the aetiology of many of the immune abnormalities in thalassaemic children are poorly defined. Therefore, we tested the hypothesis that certain immune abnormalities have a nutritional basis. Nutritional status, selective quantitative and functional indices of immunity were studied in twelve children (7 females, 5 males; mean age 28 months, SD 5 and range 19.8-35.5), with thalassaemia major before and after a one month period of intensive nutrition support (the study diet consisted of 'Enfapro' liquid formula (Mead Johnson

Perioperative stabilization of glucose levels in patients supported with total parenteral nutrition.

Fourteen nondiabetic and 6 diabetic patients were maintained on total parenteral nutrition during major surgery. Glucose levels when compared to preoperative levels increased during and after surgery although there were no significant differences between the diabetic and nondiabetic groups. Serum sodium demonstrated a slight decrease and serum potassium a slight increase after surgery in both groups. Total parenteral nutrition can be maintained during surgery provided glucose and electrolyte levels are observed and appropriate insulin doses are administered to diabetic patients.

Five-year patient-reported outcomes of individualized nutrition therapy supporting nutritional ketosis in adults with type 2 diabetes.

INTRODUCTION: Sleep disturbance, depressive symptoms, and musculoskeletal pain are common in type 2 diabetes (T2D). The long-term patient-reported effects of nutrition-based interventions targeting metabolic dysfunction remain unclear. We examined 5-year changes in sleep, depressive symptoms, and knee pain in adults with T2D participating in a digitally delivered continuous care intervention providing individualized nutrition therapy supporting nutritional ketosis. METHODS: Adults with T2D enrolled in the intervention arm of a non-randomized clinical trial were followed for 5 years (N = 194).

Digital Nutritional Care in Oncology: Opportunities to Enhance Quality of Life and Support Patient-Centered Care.

Background/Objectives: Nutritional impairment is common across the cancer care continuum and may negatively affect treatment tolerance, functional status, quality of life, and caregiver burden. Digital tools may offer new opportunities to extend nutritional care beyond conventional clinical encounters, but their application in oncology remains heterogeneous and poorly defined. This narrative review aims to describe the current landscape of digital nutritional care in oncology, focusing on quality of life and patient perspectives while also considering the emerging role of caregivers. Methods:

Preoperative nutritional support in children with Hirschsprung disease: a prospective multicenter open-label randomized controlled trial.

Preoperative undernutrition is prevalent among children with Hirschsprung disease (HSCR). To evaluate whether preoperative nutritional support reduces the incidence of postoperative HSCR-associated enterocolitis (HAEC), we conducted a prospective, multicenter, open-label randomized controlled trial. The trial was initiated on January 1, 2021 and completed on October 31, 2022. It was carried out across seven tertiary hospitals in China. A total of 110 patients admitted for pull-through surgery for HSCR were enrolled. Eligible participants were randomly assigned 1:1 via a centralized web-based r…
