Is there a role for optokinetic nystagmus testing in contemporary orthoptic practice? Old tricks and new perspectives.

Optokinetic nystagmus is a physiological oscillation of the eyes that requires intact development of motion perception, pursuit function, and saccadic function to be smoothly executed. Neuro-ophthalmological disorders often produce distinct perturbations in optokinetic responses that can be used to unmask the clinical diagnosis. With the advent of modern neuroimaging, optokinetic testing has slowly become a lost art in clinical examination. The purpose of this paper is to review the conditions wherein optokinetic testing provides a critical neurodiagnostic role, and to revitalize interest in t

[Vertigo and dizziness: the neurologist's perspective].

The spectrum of diagnoses of patients with dizziness as the leading symptom who consult a neurologist does not differ greatly from the spectrum of those who consult ear nose and throat (ENT) specialists or general practitioners (GP). The most frequent forms are benign paroxysmal positioning vertigo (BPPV), phobic postural vertigo, central vertigo disorders, Menière's disease, vestibular neuritis and bilateral vestibulopathy. However, the first and most important question that is posed to neurologists is whether it is a central or peripheral syndrome. In more than 90 % of cases this differentia

Eye movement control and its disorders.

We review the supranuclear control centers and pathways leading to individual cranial nerve nuclei in the brainstem. We discuss horizontal and vertical gaze and their abnormalities, and review the cranial nerves which subserve eye movements, III, IV, VI, including their clinical testing. We highlight the sites at which these nerves are clinically affected, which often result in characteristic associated features with neurological localizing value. Differential diagnoses of cranial nerve palsies including Duane and Möbius syndromes are also described. Lastly, we discuss the nature of childhood

Transposition surgery for internuclear ophthalmoplegia.

PURPOSE: To report outcomes of an extraocular muscle transposition procedure for surgical correction of internuclear ophthalmoplegia. METHODS: Records of patients operated on between January 1999 and May 2011 were reviewed to identify patients treated with an extraocular muscle transposition procedure for internuclear ophthalmoplegia. Indications for this procedure included a known unilateral or bilateral internuclear ophthalmoplegia with a large-angle exotropia, a moderate adduction limitation, and a large-angle exodeviation in primary position. All patients underwent orthoptic evaluation pr

Occurrence of eye movement disorders in motor neuron disease.

The diagnosis of amyotrophic lateral sclerosis (ALS) relies on symptoms and signs related to upper and lower motor neuron injury. Preservation of normal ocular motor movements is an important criterion for making this diagnosis as oculomotility pathways are classically spared in ALS. However, some authors report eye disturbances resulting from nuclear and supranuclear ophthalmoplegia in autopsy-proven ALS. Here, we report a case in which eye movement disorders were an early sign associated with a bulbar-onset ALS. The association of progressive ophthalmoplegia, dysexecutive syndrome and automa

[Inflammatory Brown syndrome: case report].

Brown syndrome fits the group of restrictive strabismus and is caused by a movement limitation of the superior oblique tendon through the trochlea. It is characterized by parallelism in the primary gaze position, limitation or absence of elevation in adduction, frequent depression of the eye in adduction with anisotropy in V-pattern and positive passive duction in the elevation in adduction. It is called inflammatory Brown syndrome, a secondary disorder, which main causes are local inflammation in the orbit and inflammatory diseases such as rheumatoid arthritis and tenosynovitis. A case of a 4

An unusual cause of head drops.

The presence of vertical ocular motor apraxia should prompt a careful assessment to detect an underlying neurometabolic condition. But congenital vertical ocular motor apraxia is a rare entity that may be attributed to perinatal stroke, hypoxia or kernicterus. We report a case of a 5-year old girl with congenital vertical ocular motor apraxia. Her magnetic resonance imaging (MRI) brain showed symmetrical high signal in the inferomedial thalamic nuclei in the distribution of the vessel of Percheron, a rare posterior cerebral artery variant. Awareness of this rare cause of childhood stroke may o

Oculomotility disorders arising from disruptions in brainstem motor neuron development.

The identification and analysis of pedigrees with rare congenital oculomotility syndromes has led to the definition of the congenital cranial dysinnervation disorders. These disorders appear to result from mutations in genes that are essential to the normal development and/or connectivity of cranial motoneurons. This review highlights the clinical features and genetic etiology of 3 congenital cranial dysinnervation disorders: the human homeobox A1 (HOXA1) syndromes, in which early motoneuron development is disrupted; horizontal gaze palsy with progressive scoliosis, in which there is aberrant

Deformational plagiocephaly associated with ocular torticollis: a clinical study and literature review.

The etiology of craniofacial asymmetry secondary to positional plagiocephaly with or without concomitant congenital muscular torticollis has been well established. It has been proposed that the craniofacial asymmetry secondary to congenital superior oblique palsy involves a similar etiology. The causal relationship has been thought to be a result of the adoption of certain head and neck positioning, which predisposes the infant to develop preferential resting positions during supine sleep. We present a single subject with ocular torticollis and resulting plagiocephaly, and we distinguish the r

Opsoclonus persisting during sleep in West Nile encephalitis.

BACKGROUND: Recent outbreaks of West Nile virus infection have alerted the public to disabling paralysis as an outcome. Ocular motor involvement with West Nile virus is rare. OBJECTIVE: To describe a patient with West Nile virus encephalitis that resulted in opsoclonus-myoclonus syndrome with persistent ocular oscillation on electroencephalography during stage 2 sleep. PATIENT: A 53-year-old man who presented with viral prodrome followed by intense vertigo and encephalopathy. In addition to multifocal myoclonic jerks in the extremities, his eye movements were disrupted by bursts of high-freq

New autosomal recessive cerebellar ataxias with oculomotor apraxia.

Autosomal recessive cerebellar ataxias (ARCAs) are a phenotypically and genetically heterogeneous group of diseases. Recently, a subgroup of ARCA associated with oculomotor apraxia (AOA) has been delineated. It includes at least four distinct genetic entities: ataxia-telangiectasia, ataxia-telangiectasia-like disorder, and ataxia with oculomotor apraxia type 1 (AOA1) and type 2 (AOA2). The phenotypes share several similarities, and the responsible genes, ATM, MRE11, APTX, and SETX, respectively, are all implicated in DNA break repair. As in many other DNA repair deficiencies, neurodegeneration

Ocular plagiocephaly: ocular torticollis with skull and facial asymmetry.

PURPOSE: To observe facial asymmetry in patients with ocular torticollis to better understand its cause. DESIGN: Observational case series. PARTICIPANTS: Forty-four consecutive patients with ocular torticollis in one author's (MFG) private practice were examined for facial asymmetry from January 1998 to August 1998. Some of these, as well as selective others before January 1998 were photographed for a total of 53 photos. METHODS: One author (MFG) examined the frontal, maxillary, and mandibular facial areas of 44 consecutive torticollis patients for appearance of unilateral compression or re

Eye muscle surgery.

In the past year experimental studies have analyzed the ease with which specific sutures can be adjusted, have investigated ways to prevent adhesions in animal models, and have studied variations of the inferior oblique at the surgical capture site. Our knowledge of rectus extraocular muscle pulleys continues to improve, and the value of botulinum toxin type A in the treatment of several conditions has been further documented. New surgical techniques include slanted recessions of the lateral rectus muscles, combined monocular resection and bilateral anterior transposition of the inferior obliq
