From single-race reporting to multiple-race reporting: using imputation methods to bridge the transition.

In 1997, the Office of Management and Budget issued revised standards for the collection of race information within the Federal statistical system. One revision allows individuals to choose more than one race group when responding to Federal surveys and other Federal data collections. This paper explores methods that impute single-race categories for those who have given multiple-race responses. Such imputations would be useful when it is desired to conduct analyses involving only single-race categories, such as when trends over time are being examined by race group so that data collected unde

An "acute abdomen" in Antarctica. The problems of diagnosis and management.

Current Australian Antarctic activities require a unique medical service: a sole medical officer is responsible for the total care of 20 to 30 expeditioners (and of himself) for around nine months a year. A case of "acute abdomen" at Casey, Antarctica, is described and the problems of diagnosis and management, in this isolated medical practice, are discussed.

Does preoperative urodynamics improve outcomes for women undergoing surgery for stress urinary incontinence? A systematic review and meta-analysis.

BACKGROUND: Urodynamics is widely used in the investigation of urinary incontinence. The existing evidence questions its add-on value in improving the outcome of surgical treatment for stress urinary incontinence (SUI). OBJECTIVES: To compare the surgical outcomes in women with SUI or stress-predominant mixed urinary incontinence (MUI) based on urodynamic diagnoses compared with diagnoses based on office evaluation without urodynamics. SEARCH STRATEGY: We searched Cochrane, MedLine, Embase, CINAHL, LILACS, metaRegister of Controlled Trials (mRCT) and Google Scholar databases from inception u

A Psittacosis Outbreak among English Office Workers with Little or No Contact with Birds, August 2015.

INTRODUCTION: On 14th August 2015 an office manager informed Public Health England of five employees known to have been diagnosed with pneumonia over the previous three weeks. We investigated to establish whether an outbreak occurred and to identify and control the source of infection. METHODS: We undertook case finding for self-reported pneumonia cases at local businesses (July-August 2015). Clinical samples from a hospitalised case were tested for common respiratory pathogens, but returned negative results. Further testing confirmed Chlamydia psittaci infection in this case (serology and PC

Management challenges arising from a traumatic 360 degree cyclodialysis cleft.

We describe the case of a 28-year-old naval officer who attended the ophthalmology service following blunt ocular trauma to the left eye. Clinical examination revealed a shallow anterior chamber, hypotony, and a 360° cyclodialysis cleft. We discuss the management options in this case, with an overview of the current literature.

What costs are associated with the management of third molars?

PURPOSE: The purpose of this study was to estimate the treatment costs directly related to operative and nonoperative management of asymptomatic, disease-free, third molar (M3) teeth. MATERIALS AND METHODS: The data reviewed were limited to claims submitted by oral and maxillofacial surgeons. The data collected included charges for consultations, radiographs, surgical removal of bony impacted teeth, and general anesthesia, using the 2009 data extracts from Delta Dental of Virginia. The median charges were used as a proxy for the actual costs attributable to the removal or retention of M3 teet

Operative management of intrauterine device complications: a case series report.

BACKGROUND: There is limited information about the number of intrauterine device (IUD) users requiring operative intervention for device-related complications. This is an evaluation of cases requiring in-hospital operative intervention for IUD perforations, removals and pregnancy-related complications. STUDY DESIGN: Large retrospective case series of patients who received operative management of IUD complications. RESULTS: Charts of patients from 15 hospitals in two health care systems from 2000 to 2007 were reviewed and outcomes of 276 women are reported. There were 95 operative IUD removal

Management of asymptomatic ventricular preexcitation.

An incidental discovery of Wolff Parkinson White (WPW) pattern on the electrocardiogram (ECG) is not an infrequent finding facing the physician. Most patients discovered incidentally are asymptomatic and it is hard to justify further management of such patients given the time-honored adage to "first do no harm." However, this finding does have implications. This article is an attempt to guide clinicians about this important issue that is often faced in the office.

Update on sport-related concussion.

Sport-related concussion is a common problem encountered by pediatricians and other primary care physicians in their practice. Assessment of concussion is essentially based on clinical evaluation. The Zurich consensus statement provides a basic framework to guide concussion management decisions and recommends an individualized approach to exercising clinical judgment in return-to-play decisions. This article reviews practice aspects of concussion for the adolescent athletes who present in a primary care office or clinic setting.

[Management of hypertension in pregnancy and lactation].

Hypertensive disorders in pregnancy are common and carry significant maternal and fetal risks. Robust evidence from the CHAP- and CHIP-studies support clear management strategies including validated home or office blood pressure measurements, organ function assessment and sFlt-1/PIGF quotient for preeclampsia risk stratification. Treatment thresholds balance maternal protection and fetal growth, with methyldopa, nifedipine and metoprolol as primary options. Postpartum follow-up is essential, as affected women have lifelong elevated cardiovascular risk.

An office-based orthotic system in treatment of the arthritic foot.

Patients with inflammatory arthritic disease of the feet invariably require conservative office management. The simplest method of obtaining relief has been to use commercially available extra-depth shoes combined with custommade, removable, closed-celled polyethylene foam arch supports. These supports are soft, light, and can be accurately adjusted to each patient's pathology. The technique and modifications used for relieving the commonest problems, such as anterior metatarsalgia and plantar heel pain, are discussed.

Primary Care Removal of Fishhooks, Rings, and Foreign Bodies from the Ear, Nose, and Superficial Eye and Conjunctiva.

Embedded fishhooks, digital ring entrapment, and foreign bodies of the ear, nose, and superficial eye and conjunctiva may present to primary care clinics and can often be managed there. This review is a guide for primary care clinicians for effective, pragmatic, and safe techniques to address these scenarios in the office and when to refer them to a surgeon.

Blunt laryngotracheal trauma.

Management of blunt trauma to the larynx and trachea is based upon accurate assessment of the anatomic site of the injury, the tissue involved, the time since the injury, and the general status of the patient. Early and orderly diagnosis and classification result in early management, which is essential to maximize function and minimize morbidity. To evaluate the management and treatment results of patients with blunt trauma to the larynx and trachea, a retrospective study was performed. The office charts and hospital records of all patients with blunt trauma to the larynx and trachea from 1966

Paroxysmal supraventricular tachycardia: improving diagnosis and management within the accident and emergency department.

Accident and emergency (A&E) senior house offices (SHOs) have difficulties in interpreting the electrocardiogram (ECG) in narrow complex tachycardia. Correct ECG interpretation is fundamental to patient management in paroxysmal supraventricular tachycardia. This study aimed to assess the ability of A&E SHOs to interpret the ECG in this setting. Correct diagnosis in 63% of cases improved to 86% with the introduction of a standard protocol sheet. Ensuring that SHOs follow agreed guidelines in rhythm recognition and management of PSVT improves patient care.

Risk factors for improper vaccine storage and handling in private provider offices.

CONTEXT: Preventing loss of vaccine potency during storage and handling is increasingly important as new, more expensive vaccines are introduced, in at least 1 case requiring a different approach to storage. Little information is available about the extent to which staff in private physicians' offices meet quality assurance needs for vaccines or have the necessary equipment. Although the National Immunization Program at the Centers for Disease Control and Prevention (CDC) in 1997 developed a draft manual to promote reliable vaccine storage and to supplement published information already availa

Adverse drug reactions in dentistry.

An adverse drug reaction (ADR) is an undesirable effect of a drug. ADRs are possible with any medication that is prescribed or administered in the dental office. While most pharmacological agents in use today have favourable drug profiles and are relatively safe, the prudent clinician must be aware of the potential ADRs that can occur and be prepared to manage any complications. Here we review the most commonly used agents in dentistry, namely local anaesthetics, sedatives, analgesics and antibiotics, and their ADRs and management.

Floor of mouth haemorrhage and life-threatening airway obstruction during immediate implant placement in the anterior mandible.

A majority of the procedures performed in the dental office setting are considered safe and minimally invasive. Despite this fact, as healthcare providers it is our responsibility to be able to anticipate, recognize and manage life-threatening emergencies that may occur. In the following report, the authors will describe a life-threatening complication that resulted from the placement of mandibular implants.

Vaginitis: finding the cause prevents treatment failure.

Even though vaginitis has a limited number of causes, many physicians find it difficult to diagnose accurately and manage effectively. Before vaginitis is treated, the cause must be ascertained, but this is often not done, and treatment failure results. The physical evaluation and a simple office microscopic analysis are key to pinpointing the cause and tailoring treatment.
