Longer Operative Time Is Independently Associated With Surgical Site Infection and Wound Dehiscence Following Open Reduction and Internal Fixation of the Ankle.

Background: Little is known regarding the association of operative time with adverse events following foot and ankle surgery. This study tests whether greater operative time is associated with the occurrence of adverse events following open reduction and internal fixation (ORIF) of the ankle. Methods: The American College of Surgeons National Surgical Quality Improvement Program database was queried for cases of ankle ORIF (primary CPT codes 27766, 27769, 27792, 27814, 27822, 27823) performed during 2005-2016. Operative time was tested for association with the occurrence of adverse events with…

Extensive Mastectomy Skin Flap Necrosis Triggered by Hypertension, Psychological Stress, and Prolonged Operative Time Due to Intraoperative Needle Loss: A Case Report.

Mastectomy skin flap necrosis (MSFN) is a frequent complication influenced by multiple factors. We report a 73-year-old woman with untreated hypertension and preoperative anxiety who developed extensive MSFN after mastectomy. Intraoperative loss of a suture needle prolonged operative time by approximately one hour, contributing to progressive flap necrosis requiring repeated debridement and negative pressure wound therapy. This case was documented to highlight the importance of recognizing how overlooked comorbidities, psychological stress, and intraoperative incidents can interact to exacerba…

Characterizing pre- and post-operative cerebral blood flow and transit time in pediatric moyamoya patients using multi-delay ASL and DSC MRI.

Cerebral blood flow (CBF) and transit time are essential biomarkers for assessing brain health. While dynamic susceptibility contrast (DSC) MRI has been widely applied to measure these metrics, it is limited in the pediatric population due to the need for contrast agents. Arterial spin labeling is a non-invasive and quantitative MR modality, and multi-delay ASL can measure CBF and transit time simultaneously. Although multi-delay ASL has been used in adult neuroimaging studies, its application in children requires investigation. Moyamoya disease, a progressive steno-occlusive cerebrovascular d

Using Intra-Operative Laryngeal Ultrasonography as a Real-Time Tool in Assessing Vocal Cord Function During Radiofrequency Ablation of the Thyroid Gland.

BACKGROUND: Inadvertent injury of the recurrent laryngeal nerve can occur during radiofrequency ablation (RFA) of thyroid nodules. Methods to avoid permanent injury have not been described. Laryngeal ultrasonography (LUSG) can assess the function of vocal cords (VCs) in real time. The present study aimed to evaluate the feasibility and accuracy of LUSG in assessing real-time VC function during RFA of benign thyroid nodules. METHODS: Consecutive patients undergoing RFA for benign thyroid nodules under local anesthesia were included. Spontaneous VC movements were checked with intra-operative LU

Placement of mid-urethral mesh slings at the time of vaginal prolapse repair does not affect post-operative sexual function or orgasm.

INTRODUCTION AND HYPOTHESIS: Because of the relationship between the clitoral neurovascular supply and the urethra, the dissection for placement of mid-urethral slings (MUS) may negatively impact orgasmic function. We aimed to analyze the role of MUS in orgasmic and overall sexual function in patients undergoing prolapse surgery. METHODS: A single institution retrospective review was performed on 157 patients undergoing prolapse surgery with and without MUS from 2008 to 2014. Pelvic Organ Prolapse Incontinence Sexual Questionnaires (PISQ-12) scores at baseline, 6, and 12 months post-operative

Letter to the editor on: "Clinical and logistical determinants of operative timing, surgical approach, and admission in pediatric supracondylar humerus fractures".

This letter to the editor examines recent work by Nelson et al. regarding the clinical and logistical determinants of operative timing, surgical approach, and hospital admission in 242 pediatric supracondylar humerus fractures. While acknowledging the study's insights into resource allocation, the author raises several methodological and statistical concerns. These include potential selection bias from differential missing data on weekends, the exclusion of nonoperatively managed patients which limits triage applicability, and the recoding of surgical approaches. Furthermore, the critique high

Exposure Time Dependence of Operators' Head Entrance Air Kerma in Interventional Radiology Measured by TLD-100H Chips.

Interventional radiology offers minimally invasive procedures guided by real-time imaging, reducing surgical risks and enhancing patient recovery. While beneficial to patients, these advancements increase occupational hazards for physicians due to chronic exposure to ionizing radiation. This exposure raises health risks like radiation-induced cataracts, cardiovascular disease, and cancer. Despite regulations like the European Council Directive 2013/59/EURATOM, which sets limits on whole-body and eye lens doses, no dose limits exist for the brain and meninges, since the brain has traditionally
