IMPORTANCE Retrospective studies have demonstrated disparate outcomes following acute optic neuritis in individuals of African descent compared with individuals of white race/ethnicity. However, published analyses of the prospectively collected Optic Neuritis Treatment Trial (ONTT) data identified no association between worse visual outcomes and black race/ethnicity. OBJECTIVES To investigate the associations of age, sex, and race/ethnicity with visual outcomes following acute optic neuritis through application of longitudinal data analysis techniques to the ONTT data set. DESIGN Secondary ana…

PURPOSE: There is considerable overlap in the clinical profile of patients with idiopathic optic neuritis (ON) and anterior ischemic optic neuropathy (AION). We tested the hypothesis that the retrobulbar diameter of the optic nerve may be a criterion for the differential diagnosis between ON and AION.
METHODS: The diameter of the optic nerve was measured by B-scan ultrasonography with the eye in an abducted position. Only patients with a unilateral optic neuropathy were included, 16 ON patients (mean age 24 years, 5 with and 11 without disc swelling) and 9 patients with AION (mean age 72 year…

Myelin oligodendrocyte glycoprotein (MOG) antibody-associated disease (MOGAD) is a neuro-inflammatory condition affecting adults and children. The presentations vary and include acute disseminated encephalomyelitis, optic neuritis or transverse myelitis. Optic neuritis associated with anti-MOG antibodies is typically bilateral, anterior and initially severe but usually resolves quickly and completely. Here, we describe 3 children with pseudotumoural optic neuritis associated with anti-MOG antibodies and their outcome. Recognizing these unusual presentations may reduce unnecessary work-ups and …

Optic Neuritis is rare in Lyme borreliosis. The current knowledge of optic nerve involvement in Lyme borreliosis relies solely on case reports. The aim of this systematic review was to characterize and investigate the associated factors of optic neuritis in Lyme borreliosis. We further presented a very rare case of isolated bilateral optic neuritis in a Lyme seropositive patient.

UNLABELLED: Optic neuritis (ON) is demyelinating acute inflammatory disease which affects the optic nerve. ON is classified as a typical (demyelinating) or an atypical (idiopathic). Patients often complain having a periocular pain or a visual loss. The main factor causing the optic neuritis is still unknown. It is believed that it might be a combination of genetic and environmental factors. As the optic neuritis is an inflammation disease, the RAGE gene was selected as it is a part of the inflammation process.
AIM: to determine the relation between RAGE rs1800624 and rs1800625 genotypes of pa…

BACKGROUND: Several neuroimaging studies demonstrated that optic neuritis (ON) leads to functional and anatomical architecture changes in the brain. The alterations of interhemispheric functional connectivity (IFC) in patients with AQP4-ON and myelin oligodendrocyte glycoprotein (MOG)-ON are not well understood.
PURPOSE: To investigate the differential patterns of VMHC in patients with AQP4-ON and MOG-ON.
MATERIAL AND METHODS: Twenty-one patients with AQP4-ON, 11 patients with MOG-ON, and 34 healthy controls underwent resting-state MRI scans. One-way ANOVA was used to identify regions in whi…

BACKGROUND AND PURPOSE: The aim of this study was to investigate the integrity of the optic nerve using the ratio between T1WI and T2WI (T1/T2) MRI signal intensities, and to assess whether inter-eye differences could indicate the presence of lesions in the optic nerve.
MATERIAL AND METHODS: The cohort included eighteen healthy controls and forty-nine individuals with multiple sclerosis; from those, thirty-two had a previous episode of optic neuritis. A method was trained to generate automatic segmentations of the optic nerve. Provided masks were then applied to the T1/T2 map and the mean pro…

PURPOSE: To describe a unique case of successful optic canal decompression in radiologically confirmed severe optic neuritis involving the optic canal.
OBSERVATIONS: A 24-year-old woman experienced severe visual loss in the right eye due to retrobulbar optic neuritis (ON), as an initial symptom of multiple sclerosis. High-dose corticosteroids and subsequent plasmapheresis yielded no improvement. Thus, visual prognosis was considered poor. Since MRI demonstrated ON within the optic canal with only minimal contrast enhancement, findings were compatible with a canalicular compartment syndrome. A…

Isolated ocular relapses of leukemia are rare and typically present as infiltrative lesions of the optic nerve. This report presents a case of an isolated ocular relapse of leukemia in a female patient, characterized by occult infiltrative optic neuropathy. Despite the absence of leukemic cell infiltration on MRI, a 33-year-old female patient presented with optic neuritis, which resolved after intrathecal administration of chemotherapeutic agents. However, due to her non-compliance with the treatment regimen, leukemia progressed to infiltrative optic neuropathy. She refused radiation therapy, …

BACKGROUND: Leber's hereditary optic neuropathy (LHON) is a maternally inherited mitochondrial disease, characterized by acute or subacute, painless, bilateral visual loss. LHON is often misdiagnosed as optic neuritis at an early stage because of the similarity of their clinical presentation. To date, there has been no reported case of actual optic neuritis and LHON in one patient.
CASE PRESENTATION: A 40-year-old, healthy man was referred to our clinic with acute painful visual loss in the right eye for 2 weeks. In the right eye, visual acuity decreased to 20/40, and the Ishihara colour test…

We report a case of Leber's hereditary optic neuropathy (LHON) masquerading as optic neuritis with late visual recovery. A 28-year-old man had gradual visual loss in both eyes for two weeks. Visual acuity was 0.4 in the right eye and 0.7 in the left. Fundus examination revealed hyperaemic discs in each eye. Fluorescein angiography revealed dye leakage at both optic discs in the late phase. Static perimetry (Humphrey 30-2) revealed bilateral relative central scotomata. Magnetic resonance imaging of the optic nerves was normal and his lumbar puncture showed normal opening pressure. He received s…

Optic neuritis (ON), which is an acute inflammatory autoimmune demyelinating disease of the central nervous system (CNS), often occurs in multiple sclerosis (MS). ON is an early diagnostic sign in most MS patients caused by damage to the optic nerve leading to visual dysfunction. Various features of both MS and ON can be studied following induction of experimental autoimmune encephalomyelitis (EAE), an animal model of MS, in Lewis rats. Inflammation and cell death in the optic nerve, with subsequent damage to the retinal ganglion cells in the retina, are thought to correlate with visual dysfun…

A 4-year-old healthy girl with acute visual loss in the right eye had ophthalmoscopic evidence of a swollen optic disc combined with central retinal artery and vein occlusion in the affected eye. MRI showed that the intraorbital optic nerve on the affected side was thickened and enhancing. Diffusion-weighted imaging showed restricted diffusion in the distal intraorbital segment of the optic nerve, consistent with infarction attributed to compression or inflammation of the vessels serving the optic nerve and retina. Although such clinical phenomena have been described previously, this is the fi

MRI of the optic nerves was obtained in 13 patients with acute optic neuritis and 13 with a previous optic neuritis (ON), assessed by clinical features, visual fields and visual evoked potentials. Results of the conventional short tau inversion recovery (STIR) sequence obtained with a short echo time (STE-STIR; 22 ms) were compared with those of a long echo time (LTE-STIR: 80 ms) sequence. The conventional STE-STIR sequence revealed lesions in the optic nerves in 78.5% of acute and 58.8% of previous ON. The LTE-STIR sequence showed abnormalities in 92.8% of acutely symptomatic nerves and 94.1%…
