Scrub typhus eschar: Clinical and 365 nm fluorescence dermoscopic correlation and follow-up till cure.

This case report presents an instance of scrub typhus, distinguished by a solitary eschar on the thigh, and explores the application of dermoscopy with 365 nm ultraviolet illumination for real-time monitoring during treatment. Scrub typhus, caused by Orientia tsutsugamushi, typically manifests with eschars that can be easily overlooked due to their painless nature. This case highlights the utility of dermoscopy in improving eschar detection and therapeutic monitoring, offering a novel approach to managing this potentially severe disease.

Acute fulminant myocarditis in scrub typhus.

Scrub typhus is a zoonotic disease caused by Orientia tsutsugamushi in which humans are accidental hosts. It is a febrile illness which is confined to rural Asia and the Western Pacific islands. Severe complications are very rare. We report four cases of acute fulminant myocarditis in children with scrub typhus. This complication led to severe cardiogenic shock and death in two of them. We believe this is the first report of fulminant myocarditis complicating scrub typhus in children.

Mixed Infection with Severe Fever with Thrombocytopenia Syndrome Virus and Two Genotypes of Scrub Typhus in a Patient, South Korea, 2017.

Severe fever with thrombocytopenia syndrome (SFTS) is a tick-borne viral disease with a high mortality rate. Infection can also occur through close contact with an infected patient. Scrub typhus is an acute febrile illness caused by Orientia tsutsugamushi, a bacterium transmitted to humans through chigger mite bites. South Korea is an endemic region of SFTS and scrub typhus. In this study, we confirmed that a patient was coinfected with SFTS virus and two (Boryong and Taguchi) genotypes of O. tsutsugamushi.

Rickettsioses presenting as major joint arthritis and erythema nodosum: description of four patients.

Erythema nodosum and aseptic arthritis are recognized associations of rickettsial infections. However, they usually present with a febrile illness rather than with severe arthritis. We report three patients who presented with incapacitating major joint arthritis and one who presented with severe spondyloarthropathy in addition to major joint arthritis due to serologically confirmed Orientia tsutsugamushi and Rickettsia conorii infections. All of them had erythema nodosum and low-grade fever. They had rapid clinical response to doxycycline.

Scrub typhus as a possible aetiology of Guillain-Barré syndrome: two cases.

Neurological complications of scrub typhus are reported to be rare. Peripheral nervous system involvement has been reported in only one case. We present two cases of Guillan-Barré syndrome (GBS) associated with scrub typhus. In both cases, the findings of an elevated indirect immunofluorescent antibody titer for Orientia tsutsugamushi and nerve conduction study showing sensory-motor polyneuropathy, have led us to believe that scrub typhus could be one of the antecedent illnesses associated with GBS.

Rickettsial, ehrlichial and Bartonella infections of the myocardium and pericardium.

Myocarditis and pericarditis are uncommon complications of human rickettsial, ehrlichial and Bartonella infections. Myocardial inflammation usually occurs in the setting of acute disseminated infection. Organisms associated with myocarditis include: Rickettsia rickettsii, R. conorii, Orientia tsutsugamushi, Coxiella burnetii, Anaplasma phagocytophila (the causative agent of Human Granulocytic Ehrlichiosis) and Bartonella henselae. Pericarditis has been described in the setting of R. conorii and Coxiella burnetii infections. This article reviews the epidemiology, pathologic characteristics, cli…

[Tsutsugamushi Disease].

Tsutsugamushi disease is an infectious disease caused by Orientia tsutsugamushi and transmitted by chiggers. The ancient classical form (summer onset) has almost disappeared, but the new form (spring and autumn onset) occurs in 300-500 cases annually. Symptoms include fever, rash, and bites. If the disease worsens, it can lead to meningoencephalitis, disseminated intravascular coagulation, and become fatal. Diagnosis involves confirming an increase in specific serum antibodies or detecting the gene in crusts. Prompt tetracycline treatment is important.

Acute pancreatitis associated with scrub typhus.

Scrub typhus, or tsutsugamushi fever, is a zoonosis of rural Asia and the western Pacific islands. The causative organism, Orientia (formerly Rickettsia) tsutsugamushi, is transmitted to humans by the bite of a larval Leptotrombidium mite (chigger). Scrub typhus may have gastrointestinal presentations, such as acute acalculous cholecystitis, duodenal ulcer perforation, peritonitis and gastric ulceration. Acute pancreatitis with scrub typhus has been reported rarely. We report a patient of scrub typhus complicated by acute pancreatitis and acute kidney injury.

Bickerstaff encephalitis: An uncommon sequalae of a bacterial infection.

Bickerstaff brainstem encephalitis (BBE) is a rare neurological disorder characterised by the presence of the triad of ophthalmoplegia, ataxia and altered consciousness. It is thought to be associated to an autoimmune condition triggered by an antecedent infection. Scrub typhus is a zoonotic disease caused by Orientia tsutsugamushi, and it is associated with myriads of neurological complications. We hereby present an unusual case of BBE in a young lady, who was probably a sequalae of scrub typhus infection. Bickerstaff brainstem encephalitis in association with scrub typhus has not been report…

Pulmonary Nocardiosis and Scrub Typhus in an Immunocompromised Host.

Pulmonary infections are not uncommon in patients with an underlying immunocompromised condition. Unusual combination of microorganisms causing concomitant infections among these patients has also been reported. However, certain rare dual occurrences are usually unanticipated as in the case we present here. This case highlights the importance of being aware of the possible coexistence of infections in immunocompromised patients. To the best of our knowledge, this is the first report of coinfection with Nocardia otitidiscaviarum and Orientia tsutsugamushi in a critically ill immunocompromised p…

Scrub typhus with unusual presentation.

Scrub typhus is an acute, febrile zoonosis caused by an obligate intracellular bacterium Orientia tsutsugamushi. The clinical manifestations of the disease range from subclinical to fatal organ failure. The common symptoms are fever, chills, headache, myalgia, dry cough, lymphadenopathy, and gastrointestinal disturbances. The presentation with complications is usually due to delay in diagnosis and treatment. We report three cases of scrub typhus that presented with features of meningo-encephalitis, which is very unusual, and all patients recovered with empirical doxycycline treatment.

Scrub typhus associated with transient parkinsonism and myoclonus.

Parkinsonism with myoclonus is rarely associated with infectious disease in adults. We present a 55-year-old man experiencing acute onset bilateral limb tremor, rigidity, and myoclonus with small-stepped gait, and skin rash involving the trunk and limbs, after a fever. Serum was positive for anti-Orientia tsutsugamushi immunoglobulin M antibody. Brain MRI revealed no abnormalities. The fever improved with oral doxycycline, and the parkinsonism and myoclonus improved with amantadine and clonazepam. This is a rare case of parkinsonism with myolonus associated with scrub typhus infection.

Contribution of rickettsioses in Sri Lankan patients with fever who responded to empirical doxycycline treatment.

Twenty-eight febrile Sri Lankan patients with undiagnosed fever for 7 days after hospital admission, who responded to empirical treatment with doxycycline, were retrospectively investigated using microimmunofluorescence assay to verify whether they had rickettsial infection. Eleven (39%) patients were confirmed as having spotted fever group rickettsioses and 10 (36%) as having Orientia tsutsugamushi. Seven were negative for all tests. This suggests that greater use of doxycycline appears justified for patients with undiagnosed fever in settings where rickettsial diseases are endemic or re-emer…

Acute scrub typhus in Northern Thailand: EKG changes.

The electrocardiographic (EKG) manifestations of scrub typhus were prospectively evaluated in 29 adult patients who acquired Orientia tsutsugamushi infection in Chiang Rai, Northern Thailand. EKGs were normal in 22 of the 29 patients (76%); minor non-specific changes were found in the other 7 patients; ie ST segment/T wave changes (10%), U waves (7%), and premature ventricular contractions (4%). These results suggest that EKG changes in scrub typhus acquired in areas of diminished antibiotic susceptibility are similar to those observed in O. tsutsugamushi infection acquired elsewhere.

Extracorporeal membrane oxygenation for extremely complicated scrub typhus.

Scrub typhus is a mite-borne disease caused by Orientia tsutsugamushi. Although early diagnosis and appropriate antibiotic therapy improve the prognosis for the majority of patients, life-threatening complications are not uncommon. Here, we present a case of successfully performed veno-veno type extracorporeal membrane oxygenation for scrub typhus-induced complications, including acute respiratory distress syndrome, sudden cardiac arrest, and multiorgan dysfunction. To our knowledge, this is the first case report of successful extracorporeal membrane oxygenation in complicated scrub typhus.

An unusual presentation of scrub typhus as tetany: A case report.

Scrub typhus is a mite-borne rickettsial infection caused by Orientia tsutsugamushi and is transmitted through the bite of larval trombiculid mites. It is an important cause of acute febrile illness in endemic regions. Although the disease typically presents with fever, rash, lymphadenopathy, and eschar, its clinical manifestations are highly variable and may include severe systemic complications. We report a case of scrub typhus presenting with symptomatic hypocalcaemia and hypophosphataemia probably the result of renal tubular electrolyte loss, highlighting a potentially under-recognized met…

First record of chigger mites (Acari: Trombiculidae) parasitizing birds in the Ogasawara Islands.

Larvae of trombiculid mites (Acari: Trombiculidae), known as chiggers, are ectoparasites that parasitize various animals and transmit pathogens such as Orientia tsutsugamushi. We report the first record of chigger mites in the Ogasawara Islands, collected from Bonin Greenfinch Chloris kittlitzi and Warbling White-eyes Zosterops japonicus. By morphological and molecular analysis, the chiggers were identified to the genus Toritrombicula which predominantly parasitize avian hosts. Although the mites could not be identified to the species level, this finding provides new insight into the distribut…

Scrub typhus due to vertical transmission in a neonate: rare presentation of a common tropical infection.

Vertical transmission as a route of infection has been well reported in many viral infections. Scrub typhus is a zoonotic disease transmitted by ticks which has had a resurgence in recent times in several tropical countries. It affects all age groups including neonates. Reports of neonates affected with scrub typhus are few, and vertical transmission is rare. We report a case, where a newborn was symptomatic with signs of infection within the first 72 hours of life and Orientia tsutsugamushi, the causative organism was confirmed by PCR in both mother and baby.
