[Treatment of injuries of the cervical spine].

Injuries of the cervical spine are re-evaluated at present. New surgical procedures and research on the stability, when these procedures are used, caused that surgical treatment is used more frequently. Attention is draw to provisions in the field as well as in hospitals, on the diagnostic approach and classification of these injuries, on the procedure of conservative surgical treatment. The domain of conservative treatment are injuries of the upper cervical spine (C1 and C2); injuries of the lower cervical spine are frequently unstable, and therefore frequently indicated for operation. Attent

Unit rod segmental spinal instrumentation for the treatment of neuromuscular scoliosis.

The Unit Rod was developed at the Hospital for Sick Children in Toronto for the surgical correction of scoliosis and pelvic obliquity associated with neuromuscular disease. Its unique contour and design allows fixation into the pelvis and decreases complications encountered with other segmental systems. Perioperative nursing care and the psychosocial impact of surgery on the family are addressed in this article. A case study is included.

Amputation stump lengthening with the Ilizarov technique. A case report.

It is often difficult to create a functional stump from a traumatic amputation, especially in a child. A case is described of a traumatic, high, below-knee amputation in a five-year-old child. The resulting stump was too short to allow a conventional below-knee prosthesis, decreasing gait efficiency. The Ilizarov technique was used to increase tibial length. Bony lengthening was very successful, but several problems were encountered with the soft-tissue reconstruction. This technique of stump reconstruction holds promise only if the soft-tissue problems are anticipated. The suggestions for fut

Biomechanical evaluation of spinal fixation devices: I. A conceptual framework.

In the field of spinal fixation devices, there is a profusion of new instrumentations. Often, the biomechanical evaluation is done in a nonstandardized manner, which makes it difficult to compare the results of one researcher with those of another, for the same device or for different devices. There is a need for a conceptual framework under which guidelines may be suggested for the evaluation of these devices in some uniform and comprehensive manner. There are three basic biomechanical tests: strength, fatigue, and stability. The strength test evaluates the failure load of the device, determi

Instrumentation of the lumbar spine. An overview.

The use of any implant system to promote arthrodesis of any segment of the spine is only as good as the technique of surgical arthrodesis. The success of the implant depends, in short, on the success of the arthrodesis. A successful arthrodesis depends on meticulous soft-tissue debridement, decortication, and copious iliac bone grafting or allograft bone grafting if autogenous graft is deficient. If the anterior approach is used, complete and thorough debridement of the intervertebral disc and cartilaginous end-plates with meticulous bone grafting of the interspace is essential for a successfu

Congenital dislocation of the hip. Use of the Pavlik harness in the child during the first six months of life.

From 1968 to 1972, twenty-three infants under six months old with twenty-seven dislocated hips were treated with a Pavlik harness. All the dislocations except three were successfully reduced. Only one child required hospitalization. All the patients were followed for more than two years. All but three of the hips were clinically and roentgenographically normal at follow-up, and none had avascular necrosis. In infants, the Pavlik harness successfully utilizes the principle of reduction in flexion, avoiding forced abduction.

Tibial lengthening using a new extension device. Report of thirty-two cases.

Using a new type of external fixation device for lengthening the tibia, lengthenings of from 3.5 to 6.5 centimeters were accomplished in thirty-two children, eight to seventeen years old, the six complications, none of which jeopardized the result. The procedure was painless and the patients were ambulatory on crutches two to three days after operation, while lengthening was continued as an out-patient procedure.

Pins and plaster treatment of comminuted fractures of the distal end of the radius.

The technique used in the treatment of seventy-five patients with severely comminuted, often intra-articular fractures near the wrist was as follows- Pins were inserted through the metacarpals and proximal part of the ulna, reduction was done, and the pins were incorporated in a cast from elbow to knuckles. This allowed movement of the fingers and the elbow. The main complications - loss of radial length and pintract infection - are avoided by attention to technical details. Of the patients with adequate follow-up, 86 per cent had good or excellent results.

A new transodontoid fixation technique for delayed type II odontoid fracture: technical note.

BACKGROUND: A different transodontoid screw fixation technique was studied in delayed type II odontoid fractures. This study presents observations on a different transodontoid fixation technique to remove and decrease the amount of sclerotic layers to accelerate the fusion process after the operation. METHODS: Ten cases of chronic type II odontoid fractures were operated on via transodontoid screw fixation between 2000 and 2007 which were admitted 6 weeks or later after the trauma. Four of these 10 delayed patients were operated on using a new anterior transodontoid screw fixation technique,

Migrated Inferior Vena Cava (IVC) Filter Presenting as Tricuspid Valve Mass, Right-Sided Heart Failure, and Parodoxical Emboli.

A 58-year-old male with an unknown medical history presented with acute encephalopathy, receptive aphasia, and hypertensive emergency. The patient did not have any family members from whom a collateral history could be obtained. He underwent X-rays of the abdomen and bilateral humeri/femurs to check for foreign bodies. He was found to have right femoral open reduction and internal fixation with retained screw fragments. He was diagnosed with ischemic stroke on MRI. Transthoracic echocardiogram (TTE) revealed right-sided heart failure and a tricuspid valve mass as well as right to left shunting

The use of circular external fixators in the management of lower limb trauma in Dublin: a single surgeon's 20-year experience.

INTRODUCTION: It has been estimated that approximately 520,000 injury presentations are made to Irish accident and emergency departments each year. Fractures account for 20 % of these injuries. Circular external fixators (frames) have been shown to be a safe and effective method of treatment for long bone fractures where internal fixation is impossible or in-advisable. We present the outcomes of all frames applied at our institution for stabilisation of acute fractures over a 20-year period. METHODS AND METHODS: We retrospectively reviewed a prospectively compiled database of all frames appli

Designation and Validation of a Posterior Anatomical Plate for the Anterior Column of the Acetabulum.

BACKGROUND Surgical treatment of acetabular fractures is one of the greatest challenges for orthopedic surgeons. Fixation of most displaced fractures requires extensive exposure, which may lead to complications, including blood loss, neural or vascular injury, postoperative infection, wound healing problems, and heterotopic bone formation. MATERIAL AND METHODS This study was conducted to certify an anatomic plate with an anterior column lag screw guiding device to repair the posterior acetabulum. Complete pelvic spiral computed tomography (CT) scan data were collected from 56 patients. The pos

Fixation of osteochondral fragments in the human knee using Meniscus Arrows.

The aim of this study is to compare the hold in bone of Meniscus Arrows and Smart Nails, followed by the report of the results of the clinical application of Meniscus Arrows as fixation devices. First, pull-out tests were performed to analyse the holdfast of both nails in bone. Statistical analysis showed no significant difference; therefore, the thinner Meniscus Arrow was chosen as fixation device in the patient series of two patients with a symptomatic Osteochondritis dissecans fragment and three patients with an osteochondral fracture of a femur condyle. The cartilage margins were glued wit

[Dynamic stabilisation and compression without fusion using Dynesys for the treatment of degenerative lumbar spondylolisthesis: a prospective series of 25 cases].

PURPOSE OF THE STUDY: We used the Dynesys stabilization to treat degenerative lumber spondylolysis by decompression without fusion with the objective of decreasing the morbidity related to instrumented arthrodesis in older patients yet preventing progression of the displacement. MATERIAL AND METHODS: This was a prospective study of 25 patients with symptomatic degenerative lumber spondylolysis associated with degenerative spinal canal stenosis documented by saccoradiculography. For inclusion, static anteroposterior intervertebral displacement had to be at least 3mm in the upright position, ir

ACL reconstruction using the Rigidfix femoral fixation device via the anteromedial portal: a cadaver study to evaluate chondral injuries.

The aim of this anatomic descriptive cadaver study is to evaluate the entrance points of cross-pins and the possible chondral damages, using a two cross-pin femoral fixation device via anteromedial portal, during anterior cruciate ligament (ACL) reconstruction. Twenty fresh-frozen cadaver knees (12 cadavers) were tested. We employed the Rigidfix Cross Pin device (Mitek, Norwood, MA), designed to use two biodegradable pins (PLA, length 42 mm, diameter 2.7 mm). Instead of PLA pins, we used color coded metallic pins. Femoral tunnel drilling and cross-pin guide insertions were performed through th
