Effect of knot location on the biomechanical strength and gapping characteristics of ex vivo canine gastrocnemius tenorrhaphy constructs.

OBJECTIVE: To evaluate the effect of knot location on the biomechanical strength and gapping characteristics of ex vivo canine gastrocnemius tenorrhaphy constructs. SAMPLE: 36 cadaveric gastrocnemius tendons from 18 adult dogs. PROCEDURES: Tendons were randomly assigned to 3 groups (12 tendons/group) and sharply transected and repaired by means of a core locking-loop suture with the knot at 1 of 3 locations (exposed on the external surface of the tendon, buried just underneath the external surface of the tendon, or buried internally between the apposed tendon ends). All repairs were performe

Lymphatic Injury After Vertebral Column Resection from a Posterior Approach for Spinal Deformity Correction: A Case Report.

CASE: We describe a case of lymphatic injury that occurred during a 2-level vertebral column resection for spinal deformity correction from a posterior-only approach. Large surgical drain volume output with laboratory findings of high levels of triglycerides and lymphocytes prompted conservative treatment with an elastic wrap bandage and a "no fat" diet. The patient responded to the treatment with no sequelae noted. CONCLUSION: Lymphatic complications are rare in spine surgery. All reported cases are associated with anterior approaches. The lymphatic injury presented here demonstrates the pot

Coronal realignment and reduction techniques and complication avoidance.

Scoliosis is a broad term encompassing multiple pathologies with different etiologies. Patients may range from the infant with congenital deformity, to the adolescent with idiopathic scoliosis, to the elderly patient with severe degenerative scoliosis. Treatment must be tailored to individual circumstances and the pathoanatomy of each deformity. Various coronal reduction techniques have been described and will be discussed within this article. While scoliosis is generally considered a deformity in the coronal plane, often deformity is present in the sagittal and axial planes also. Treatment of

An approach to the supraclavicular and infraclavicular aspects of the brachial plexus.

The surgical approach to the brachial plexus is of great importance during surgery for brachial plexus injuries--obstetric or traumatic. A number of different approaches have been described over the years for these injuries, with each 1 having its own advantages and disadvantages. We describe a novel approach to the supra-clavicular and infra-clavicular aspects of the brachial plexus, which has been used by senior author successfully for the past 15 years. It involves creation of 2 windows, 1 above and 1 below the clavicle in case of adult traumatic plexus injuries. A slight variation of joini

[Surgical treatment of radicular pain using minimally invasive techniques].

Radicular pain can be caused by disc herniation, lateral stenosis, isthmic spondylolisthesis with foraminal stenosis, or foraminal encroachment due to asymmetrical disc degeneration or scoliosis. Surgery is indicated following failure of conservative treatment. Minimally invasive discectomy is indicated for subjects presenting with radicular pain with or without neurological deficit and appropriate sized herniation in MRI. It offers equivalent efficacy but quicker recovery than microdiscectomy. Minimally invasive fusion is indicated for radicular pain due to foraminal compression in isthmic sp

Smoking-related complications in foot and ankle surgery: a systematic review.

OBJECTIVE: The detrimental effects of cigarette smoking on overall health are well-documented, with nicotine and carbon monoxide contributing to peripheral vasoconstriction and impaired oxygen delivery to tissues. This study reviews the impact of smoking on wound and bone healing, specifically in foot and ankle surgery, given its significant role as a modifiable risk factor for complications in orthopedic procedures. MATERIALS AND METHODS: A systematic literature review was conducted in May 2024 following PRISMA guidelines. An electronic search of PubMed was performed using keywords related t…

Orthopaedic management of the Ehlers-Danlos syndromes.

The role of orthopedic surgery in Ehlers-Danlos syndrome is inherently controversial, opaque to most patients and many medical providers, and difficult to discern from available medical literature. Non-operative treatment is preferable, but for carefully selected patients, specific joint stabilization and nerve decompression procedures can provide symptomatic relief when conservative measures fail. © 2017 Wiley Periodicals, Inc.

Systemic nickel allergy after internal fixation of a bunionectomy.

Allergic reactions to implanted metals have been estimated to occur in 1% to 5% of orthopedic cases. Stainless steel screws, which contain 14% nickel, are commonly used for internal fixation in an array of podiatric procedures. We present a rare case of a systemic allergic reaction to nickel secondary to stainless steel screw fixation in a bunionectomy procedure.

A Quantitative and Qualitative Literature Analysis of the Orthopedic Surgeons' Experience: Reflecting on 20 Years in the Global War on Terror.

INTRODUCTION: After over 20 years of war in the Middle East, orthopedic injuries have been among the most prevalent combat-related injuries, accounting for 14% of all surgical procedures at Role 2/3 (R2/R3) facilities according to the DoD Trauma Registry. To further delineate the role of the deployed orthopedic surgeon on the modern battlefield, a retrospective review was performed highlighting both quantitative and qualitative analysis factors associated with orthopedic surgical care during the war in the Middle East. METHODS: A retrospective review was conducted of orthopedic surgeons in th

Institutional disparities in the treatment of bone metastases by orthopaedic surgeons at training facilities designated by the Japanese orthopaedic association-A nationwide survey.

BACKGROUND: Bone metastases are the main cause of locomotive syndrome in cancer patients (Cancer Locomo). They should be appropriately managed to improve patients' activities of daily living and quality of life, especially in designated cancer care hospitals (DCCHs). Still, the actual involvement of orthopedic surgeons in managing bone metastases was uncertain. MATERIALS AND METHODS: In 2018, an online questionnaire of 38 questions was used to conduct a nationwide survey among orthopaedic training facilities. Eleven questions were used to clarify whether orthopaedic surgeons were actively inv

Acquired thoracic scoliosis following minimally invasive repair of pectus excavatum.

The minimally invasive pectus excavatum repair as described by Nuss et al. is rapidly gaining acceptance as an effective method of repair of severe pectus excavatum deformities in the pediatric population. It potentially offers several advantages over previous techniques. The incidence of major complications of the procedure has been reduced by recent modifications including utilization of video-assisted thoracoscopy during placement of the Lorenz pectus bar as well as utilizing the pectus bar stabilizer that provides more rigid fixation of the strut. We report two cases of acquired thoracic s

Selective dorsal rhizotomy and rates of orthopedic surgery in children with spastic cerebral palsy.

If the spasticity of cerebral palsy (CP) is reduced in children at a young age by selective dorsal rhizotomy, the incidence of lower-extremity deformities requiring orthopedic surgery may be reduced; however, this has never been investigated in detail. The authors examined the effects of selective dorsal rhizotomy on rates of lower-extremity orthopedic surgery in 178 children with CP. Age at selective dorsal rhizotomy ranged from 2 to 19.3 years (mean 5.5 years) with follow-up intervals ranging from 24 to 70 months (mean 44 months). Spastic CP was classified as quadriplegia (33%), diplegia (65

Time to closure of orthopaedic surgical incisions: a novel skin closure device versus conventional sutures.

OBJECTIVE: New technologies are being developed to optimise healing of surgical incisions. BandGrip (US) is a micro-anchor skin closure device that replaces the need for subcuticular suturing and further dressing. The purpose of this study is to perform a matched cohort analysis comparing time to closure of surgical incisions between sutures and the novel skin closure device. METHOD: Patients undergoing orthopaedic surgery in 2019 underwent skin closure with either conventional sutures or the novel skin closure device. Patients were divided into three groups according to their procedural inci

Guillain-Barré syndrome after pelvic fracture fixation: a rare cause of postoperative paralysis.

STUDY DESIGN: Case report. OBJECTIVE: To describe the important epidemiology, clinical presentation, and pathophysiology of Guillain-Barré syndrome (GBS) after orthopedic surgery in a trauma patient. SUMMARY OF BACKGROUND DATA: Little in the orthopedic literature describes trauma as an etiology of GBS. We report a case of post-traumatic GBS in a 52-year-old male who developed ascending weakness after experiencing pelvic trauma that required 2 separate orthopedic procedures for pelvic stabilization after a fall from a height of 12 ft. After the index operative procedure, the patient complaine…
