Wearable Technologies for Gait Instability Rehabilitation: Mechanisms, Clinical Evidence, and Future Directions.

Wearable technologies are reshaping gait rehabilitation by shifting assessment and therapy from intermittent, clinic-based observation to continuous, data-driven, and adaptive care. This narrative review synthesizes the biomechanical basis of gait instability, the architecture and classification of wearable rehabilitation systems, and the clinical evidence for motion sensors, smart insoles, biofeedback devices, robotic and orthotic wearables, neuromodulatory systems, immersive platforms, and artificial intelligence (AI)-enabled closed-loop interventions. Its main contribution is an integrated

Diagnosis and management of posterior plagiocephaly.

OBJECTIVE: The management of infants with posterior plagiocephaly has been controversial both because of widely differing estimates in the literature of the relative frequencies of true lambdoidal synostosis vs positional molding and because of divergent approaches to treating this problem in different institutions. Based on our experience, we hypothesized that the vast majority of children with posterior plagiocephaly did not have true synostosis and that the cosmetic impairment in such patients could be effectively treated with nonsurgical modalities. METHODS: Between 1992 and 1995, we pros

The Application of Integrated Force and Temperature Sensors to Enhance Orthotic Treatment Monitoring in Adolescent Idiopathic Scoliosis: A Pilot Study.

Orthosis-wearing compliance is crucial for achieving positive treatment outcomes in patients with adolescent idiopathic scoliosis (AIS), for whom 23 h of daily wear is typically prescribed. However, self-reported compliance is subjective and often based on patients' memory, leading to inaccuracies. While portable electronic devices have been developed to objectively monitor compliance, relying solely on temperature or force data can be insufficient. This study introduced a novel method that integrated both force and temperature data to estimate orthosis-wearing compliance. Twelve patients (eig

Design and evaluation of Mina: a robotic orthosis for paraplegics.

Mobility options for persons suffering from paraplegia or paraparesis are limited to mainly wheeled devices. There are significant health, psychological, and social consequences related to being confined to a wheelchair. We present the Mina, a robotic orthosis for assisting mobility, which offers a legged mobility option for these persons. Mina is an overground robotic device that is worn on the back and around the legs to provide mobility assistance for people suffering from paraplegia or paraparesis. Mina uses compliant actuation to power the hip and knee joints. For paralyzed users, balance

Predicting Forefoot-Orthosis Interactions in Rheumatoid Arthritis Using Computational Modelling.

Foot orthoses are prescribed to reduce forefoot plantar pressures and pain in people with rheumatoid arthritis. Computational modelling can assess how the orthoses affect internal tissue stresses, but previous studies have focused on a single healthy individual. This study aimed to ascertain whether simplified forefoot models would produce differing biomechanical predictions at the orthotic interface between people with rheumatoid arthritis of varying severity, and in comparison to a healthy control. The forefoot models were developed from magnetic resonance data of 13 participants with rheuma

Treatment of temporomandibular joint syndrome for relief of cervical spine pain: case report.

An adult female presenting with clinical impressions of cervical myositis with radiculopathy which began three days earlier following a motor vehicle accident, was referred for dental consultation because of tenderness over the left temporomandibular joint; response of the cervical spine pain to manipulation and physical therapy, including transcutaneous electrical nerve stimulation (TENS), was minimal. Dental examination revealed a malocclusion and following treatment with an intraoral orthotic device, along with chiropractic care, the patient's cervical spine pain was completely relieved. It

Osteochondral Allograft Transplant for Degenerative Joint Disease of the First Metatarsophalangeal Joint: A Case Report.

The first metatarsophalangeal joint is the most common location for arthritis in the foot. Pain and limited mobility associated with arthritis of the first metatarsophalangeal joint are the hallmarks of this disease. Treatments include shoe modification, orthotic devices, nonsteroidal anti-inflammatory drugs, injections, physical therapy, and surgery. Surgery has been the most perplexing, with surgical treatments ranging from simple ostectomies to fusion of the first metatarsophalangeal joint. Implant arthroplasty, with its various designs and techniques, has yet to be proven as the definitive…

[Foot and ankle surgery].

Rheumatoid arthritis is a systemic disease that often affects the foot and ankle (85%-100% of patients). There are characteristic deformities in relation to the stage of disease. Clinical assessment of both lower limbs is important, since factors such as valgus deformity of the knee can cause malposition of the foot and ankle. In the early stages, patients present with joint effusion which causes destruction of the cartilage ("stiff type") and distends ligaments and capsules ("loose type"). Medial and lateral ankle tendons are destroyed and become insufficient. A reduction in walking distance,

The relationship of the angle of immobilisation of the knee to the force applied to the extensor mechanism when partially weight-bearing. A gait-analysis study in normal volunteers.

We describe the influence of the angle of immobilisation during partial weight-bearing on the forces across the extensor mechanism of the knee. Gait analysis was performed on eight healthy male subjects with the right knee in an orthotic brace locked at 0 degree, 10 degrees, 20 degrees and 30 degrees, with the brace unlocked and also without a brace. The ground reaction force, the angle of the knee and the net external flexion movement about the knee were measured and the extensor mechanism force was calculated. The results showed a direct non-linear relationship between the angle of knee flex

The effects of total contact insole with forefoot medial posting on rearfoot movement and foot pressure distributions in patients with flexible flatfoot.

The purpose of this study is to evaluate the therapeutic effect of total contact insole with forefoot medial posting (TCIFMP) orthosis in patients with flexible flatfoot. The TCIFMP insole was custom- mode, made from semi-rigid plastazote and PPT. Using the gait analysis and the plantar-pressure measure systems, we investigate rearfoot motion and plantar pressure redistribution in these patients. The results of this study showed that the excessive valgus movement of the rearfoot can be reduced significantly by the TCIFMP insole in these patients. Besides, there were significant decreases in th

A novel synergistic device for joint inflammation - efficacy on ankle sprain cases.

Joint inflammation results from soft-tissue injuries and cartilage damage. PRICE is the standard treatment approach for acute soft tissue injuries like ankle sprain. Electrical stimulation, application of orthotic braces, etc. is also effective for this. In a synergistic device all these components are combined and applied simultaneously. This device was developed for joint inflammation and tested for grade I & II acute ankle joint sprain. To test a synergistic - semirigid device, combining PRICE & electrical stimulation for acute ankle sprains of grades I & II for pain, range of motion and sw

Girolamo Fabrici d'Acquapendente and the Oplomochlion: The Several Applications of an Effective Rehabilitation Tool.

Girolamo Fabrici d'Acquapendente (1533-1619) was an Italian anatomist, surgeon and physiologist and a protagonist of the scientific revolution of the Renaissance. He made anatomy a scientific discipline and is justly considered a precursor of modern orthopaedics. He invented and used several external corrective devices for the treatment of congenital and acquired deformities of the limbs and spinal column, especially those following tubercular infection and rickets, torticollis, vertebral caries kyphosis, scoliosis, and rachitic deformities of the leg, but also congenital dislocation of the hi

The use of magnetic resonance imaging to determine splint position in the management of internal derangements of the temporomandibular joint.

Internal derangements of the temporomandibular joint (TMJ) can elicit symptoms of pain, clicking and noises described subjectively as popping, grinding or grating. These symptoms may be the precursor of a more severe disease process such as degenerative joint disease (DJD) or osteoarthritis. The most common factor predisposing to degenerative changes within the TMJ is a displaced meniscus. Maintenance of a proper anatomic meniscal-condylar relationship is essential in preventing DJD. In the past, non-surgical treatment utilizing intraoral orthotic devices has been attempted to "recapture" the …
