Excellent results of HA coating on a grit-blasted stem: 245 patients followed for 8-12 years.

We report the outcome of a grit-blasted titanium stem designed for press-fit insertion and entirely plasma sprayed with HA. During the years 1988-1993, we performed 323 primary total hip replacements in 276 patients (189 women) with a HA-coated prosthesis. Their mean age was 48 (15-79) years. During the follow-up, 12 patients died. 19 other patients did not attend the follow-up examination, but had no major symptoms according to telephone interviews and written replies. Thus, 245 patients (291 hips) were followed for a mean of 10 (8-12) years with radiographic and clinical examinations. Only 1

Inhibiting Osteolytic Breast Cancer Bone Metastasis by Bone-Targeted Nanoagent via Remodeling the Bone Tumor Microenvironment Combined with NIR-II Photothermal Therapy.

Bone is one of the prone metastatic sites of patients with advanced breast cancer. The "vicious cycle" between osteoclasts and breast cancer cells plays an essential role in osteolytic bone metastasis from breast cancer. In order to inhibit bone metastasis from breast cancer, NIR-II photoresponsive bone-targeting nanosystems (CuP@PPy-ZOL NPs) are designed and synthesized. CuP@PPy-ZOL NPs can trigger the photothermal-enhanced Fenton response and photodynamic effect to enhance the photothermal treatment (PTT) effect and thus achieve synergistic anti-tumor effect. Meanwhile, they exhibit a photot

Peri-acetabular bone remodelling after uncemented total hip arthroplasty with monoblock press-fit cups: an observational study.

BACKGROUND: Bone stock preservation in total hip replacement is essential to allow for later revisions in an increasingly younger and fitter index patient population. While contemporary modular press-fit acetabular cups lead to rigid fixation with equatorial stress and central osteolysis, more elastic fixation may cause different peri-acetabular bone remodelling. The purpose of this study was to investigate changes in peri-acetabular bone mineral density (BMD) in uncemented elastic fixation with monoblock press-fit cups. METHODS: This prospective observational study included 45 patients with

Enhanced wear resistance of modified cross-linked polyethylene by grafting with poly(2-methacryloyloxyethyl phosphorylcholine).

We developed a cross-linked polyethylene (CLPE) modified with a phospholipid polymer in order to address the serious problem of osteolysis caused by wear particles derived from the polyethylene components of artificial hip joints. Our goal of preventing aseptic loosening could be achieved by avoiding any formation of CLPE wear particles or suppressing the activation of cell systems by the wear particles. We investigated the surface and wear resistance properties of 2-methacryloyloxyethyl phosphorylcholine (MPC) polymer grafted onto the surface of CLPE (CLPE-g-MPC). The relative density of MPC

[Surgical management of vertebral column metastatic disease].

The spine is the most frequent site of skeletal metastases. Among all spinal malignancies metastatic disease is most frequent and indicative of disseminating tumor disease. Depending on primary tumor entity, estimated survival time, general health status of the patient, presence of spinal instability and neurological deficits an oncological useful and patient-specific therapeutic intervention should be performed. New anterior approaches, resections and reconstruction techniques are making surgery a preferred method over radiation therapy. For differential indication of the multiple surgical tr

Cartilage-mimicking, high-density brush structure improves wear resistance of crosslinked polyethylene: a pilot study.

BACKGROUND: In natural synovial joints under physiologic conditions, fluid thin-film lubrication by a hydrated layer of the cartilage is essential for the smooth motion of the joints. The considerably less efficient lubrication of artificial joints of polyethylene is prone to wear, leading to osteolysis and aseptic loosening and limiting the longevity of THA. A nanometer-scale layer of poly(2-methacryloyloxyethyl phosphorylcholine) (PMPC) with cartilage-mimicking brushlike structures on a crosslinked polyethylene (CLPE) surface may provide hydrophilicity and lubricity resembling the physiologi

Mid-term results using a cementless hip prosthesis in young Chinese patients: a five- to seven-year follow-up study.

A retrospective study was undertaken to evaluate the clinical and radiographic outcomes of 74 cementless total hip arthroplasties (THA) in 69 young Chinese patients. The Asian size and MMA AML stems with smoothly tapered tip were used, and the patients were followed up for at least five years. The mean Harris hip score was 45.4 preoperatively and 95.3 at the last follow-up. The incidence of thigh pain was 5.4%, and was related to the short stature of the patient (<160 cm) (P = 0.030). Six patients (6 hips, 8.1%) had acetabular osteolysis in zone 2; reoperation was performed in one patient beca

Zn-Sr-sintered true bone ceramics enhance bone repair and regeneration.

Bone defects are one of the toughest challenges faced by orthopedic surgeons worldwide, especially at critical sizes, which are caused by severe trauma, malignancy, or congenital disease. The ideal bone tissue-engineered scaffold for bone regeneration is the one that has good osteoconductivity, osteoinductivity, pore structure, and antibacterial properties. Metal ions have been recognized in recent years to be essential regulators of bone metabolism, and they are widely used for bone tissue engineering. In particular, zinc ions are of interest because of their ideal biocompatibility, osteogene

Arrested Pneumatization of the Sphenoid Sinus.

The sphenoid bone is a complex structure in terms of its embryological origin. At birth, the sphenoid sinus is nonpneumatized. Arrested pneumatization of the sphenoid sinus is considered a normal anatomic variant but may be mistaken for disease in imaging studies. We report 2 cases of arrested pneumatization of the sphenoid sinus, a normal variant commonly misdiagnosed as a serious disease of the skull base. A 29-year-old man with a complaint of dizziness visited a local clinic for assessment. Computed tomography (CT) of the paranasal sinuses (PNS) showed a noneroding, nonexpansile, and nonhom

Crosstalk between bone niche and immune system: osteoimmunology signaling as a potential target for cancer treatment.

There is a well recognized link between the bone and the immune system and in recent years there has been a major effort to elucidate the multiple functions of the molecules expressed in both bone and immune cells. Several molecules that were initially identified and studied in the immune system have been shown to have essential functions also in the bone. An interdisciplinary field embracing immune and bone biology has been brought together and called "osteoimmunology". The co-regulation of the skeletal and immune systems strikingly exemplifies the extreme complexity of such an interaction. T

The enigmas of bone without osteocytes.

One of the hallmarks of tetrapod bone is the presence of numerous cells (osteocytes) within the matrix. Osteocytes are vital components of tetrapod bone, orchestrating the processes of bone building, reshaping and repairing (modeling and remodeling), and probably also participating in calcium-phosphorus homeostasis via both the local process of osteocytic osteolysis, and systemic effect on the kidneys. Given these critical roles of osteocytes, it is thought-provoking that the entire skeleton of many fishes consists of bone material that does not contain osteocytes. This raises the intriguing q

[Seven cases of inverted nasosinusal papilloma treated at Cotonou].

OBJECTIVE: Sinonasal inverted papilloma is a rare benign tumor with a high recurrence rate and potential malignant transformation. The purpose of this study was to analyze the clinical and radiological aspects and to identify the suitable surgical approaches to be used in developing countries. METHODOLOGY: In 3 years, 7 cases have been analyzed in a retrospective study. Patients presented with unilateral nasal obstruction and epitasis, a whitish unilateral polypoid mass, sinonasal opacity with osteolysis on CT scan or on sinus X-ray. Using these data we adopted the Krouse staging which classi

The relative timing of exposure to phagocytosable particulates and to osteoclastogenic cytokines is critically important in the determination of myeloid cell fate.

During granulomatous inflammatory reactions, myeloid cells can differentiate into activated phagocytic macrophages, wound-healing macrophages, foreign body giant cells, and bone-resorbing osteoclasts. Although it is appreciated that a variety of stimuli, including cytokines, cell-matrix interactions, and challenge with foreign materials can influence myeloid cell fate, little is known of how these signals integrate during this process. In this study, we have investigated the cross talk between receptor activator of NF-kappaB ligand (RANKL)-induced osteoclastogenesis and particle phagocytosis-i

Rat models of bone metastases.

Bone metastases occur frequently in patients with advanced breast or prostate cancer. Bone metastases can be predominantly osteolytic, osteoblastic or mixed. Studies with animal models allow advances in understanding the molecular basis for bone metastases and provide new targets for therapy. Several animal models have been developed in rat with different pathophysiologies; they required injection or implantation of neoplastic cells into orthotopic locations, bones or the left ventricle of the heart. Several specific strains of rat have an increased incidence of spontaneous tumors. Carcinomas

Early scanning electron microscopic studies of hard tissue resorption: their relation to current concepts reviewed.

This paper highlights some observations made by the authors in SEM studies of hard tissue resorption and considers their significance in relation to current concepts. All mammalian mineralised tissues may undergo physiological resorption, the resulting surface reflecting the density of mineralisation and the organic matrix chemistry, organisation and orientation. Resorption-repair coupling may follow the resorption of any tissue, but SEM studies first noted this process in the case of the dental tissues. The difference between fetal and adult bone formation and resorption provided evidence aga

SENP6-mediated desumoylation of SMAD5 regulates osteogenic fate in periodontal ligament stem cells.

BACKGROUND: Periodontal ligament stem cells (PDLSCs) are essential for periodontal tissue regeneration, but their osteogenic differentiation is significantly impaired in the inflammatory microenvironment of periodontitis. This study aimed to investigate the regulatory role of the SENP6-SMAD5-SOX2 axis in PDLSC osteogenesis. METHODS: Transcriptomic analysis of public datasets was used to identify key regulators. In vitro functional experiments, including siRNA-mediated knockdown, co-immunoprecipitation, and luciferase assays, were performed on PDLSCs isolated from healthy donors. The expressio

Targeting RANKL in metastasis.

Acting through its cognate receptor, receptor activator of nuclear factor-κB (RANK), RANK ligand (RANKL) is an essential mediator of osteoclast function and survival. Preclinical data have now firmly established that blockade of tumor-induced osteoclastogenesis by RANKL inhibition will not only protect against bone destruction but will also inhibit the progression of established bone metastases and delay the formation of de novo bone metastases in cancer models. In patients with bone metastases, skeletal complications are driven by increased osteoclastic activity and may result in pathological
