P16 detection in benign, precursor epithelial lesions and carcinomas of head and neck.

BACKGROUND: Human papillomavirus (HPV) had recently been implicated in the pathogenesis of Head and Neck SCCs. The biological role of HPV in benign and pre-cancerous lesions is far less studied. p16 is a widely accepted marker to detect immonohistochemically the presence of HPV. METHODS: We evaluated, immunohistochemically, expression of p16 in 212 specimens: glottis, supraglottis, oropharynx, nasal/paranasal, with various diagnoses: hyperplasia, polyp/nodule, keratosis, papilloma, inverted papilloma, dysplasia, cancer (SCC). Analysis was completed according to location and disease. RESULTS:…

Endoscopic surgical treatment of sinonasal Schneiderian papillomas: our experience.

BACKGROUND: Sinonasal Schneiderian papillomas are uncommon epithelial tumors. Although benign, they may behave aggressively, recur after incomplete resection, and, particularly in the inverted subtype, be associated with carcinoma. Endoscopic surgery is now widely used when the site of attachment can be adequately exposed and treated. OBJECTIVE: To review recurrence, morbidity, attachment-site documentation, and metachronous carcinoma in a single-surgeon endoscopic series of sinonasal Schneiderian papillomas treated with an attachment-site-oriented technique. METHODS: We retrospectively revi

Galectin fingerprinting in naso-sinusal diseases.

Galectins, a family of endogenous lectins, are multifunctional effectors that act at various sites and can be used in immunohistochemical localization studies of diseased states. Since they form a potentially cooperative and antagonistic network, we tested the hypothesis that histopathological fingerprinting of galectins could refine the molecular understanding of naso-sinusal pathologies. Using non-cross-reactive antibodies against galectin-1, -3, -4, -7, -8 and -9, we characterized the galectin profiles in chronic rhinosinusitis, nasal polyposis, inverted papillomas and squamous cell carcino

Isolated Schneiderian papilloma of the middle ear cleft.

A 60-year-old woman presented with pulsatile tinnitus in the left ear. MRI and CT imaging suggested a soft-tissue mass in the middle ear. Exploratory tympanotomy and biopsy confirmed Schneiderian (inverted) papilloma. Endoscopic and radiological assessment showed no evidence of sinonasal lesions. The patient proceeded to mastoid surgery with removal of the ossicles for disease clearance. The small number of cases published to date of isolated middle ear inverted papilloma suggest a high recurrence and malignant transformation rate and aggressive management is therefore warranted.

Isolated oncocytic sinonasal papilloma of the sphenoidal sinus.

The ectodermally derived mucosa of the nose and paranasal sinuses may give rise to three morphologically different papillomas referred to individually as exophytic papilloma, inverted papilloma (IP) and oncocytic sinonasal papilloma (OSP). OSP is the rarest of the three different types, frequently affects the maxillary sinus, ethmoid sinus and nasal cavity while sphenoidal sinus involvement is exceedingly rare. The tumour has no gender predilection and has a wide age range with the majority above 50 years of age. Clinically, its behaviour parallels IP due to local recurrence and tendency to ma…

Life-threatening Bleed Secondary to Tumor Shrinkage Effectively Palliated with Radiotherapy.

Inverted papilloma is a typically benign, but locally aggressive tumor arising from the nasal cavity and paranasal sinuses. Malignant transformation can occur in up to 10% of cases. Although spontaneous tumor bleeding can occur with malignancies, hemorrhage secondary to tumor shrinkage has not been reported. We present a patient with metastatic squamous cell carcinoma (from inverted papilloma) who developed a life-threatening bleed shortly after chemotherapy initiation. She was managed successfully with life-saving palliative radiotherapy (RT), delivered based on clinical markup. She was subse

Cutaneous viral papilloma with local extension and subungual cyst formation in a dog.

A viral-induced digital cutaneous exophytic papilloma was diagnosed in a 2-year-old, spayed, female Siberian husky dog with lameness. Digital pain and lameness persisted after removal of the initial papilloma, and the fifth lateral digit was subsequently amputated. Upon histologic examination of the digit, a de novo digital, cutaneous, inverted, viral papilloma and subungual cyst were diagnosed. The inverted cutaneous papilloma, located at the junction of the digital paw pad and ventral nail, extended focally through the nail into the subungual space, where an expansile cyst was formed. CD163 Immunohistochemical "Circle Sign" Staining Pattern Differentiates Sinonasal Papillomas From Morphologically Similar Non-neoplastic Lesions.

Sinonasal papillomas are a diverse group of benign epithelial neoplasms of the sinonasal tract. Inverted papilloma, in particular, must be distinguished from other lesions with no malignant potential. The aim of this study was to distinguish sinonasal papillomas from morphologically similar lesions using CD163 immunostaining. Cases from a 19-year period were identified. These included 49 inverted, 10 exophytic, and 12 oncocytic papillomas, 21 chronic sinusitides with squamous metaplasia, 27 inflammatory polyps, 5 verrucae vulgares, 5 respiratory epithelial adenomatoid hamartomas, and 6 DEK::AF

Left-sided facial mass in an adult female.

A left maxillary sinus soft tissue mass was discovered on computed tomography in a 70-year-old woman who had been experiencing blood-tinged mucus for 2 years. The lesion demonstrated mild enhancement, and bony destruction. Magnetic resonance imaging displayed a cerebriform appearance of the mass, which mimicked the appearance of inverted papilloma. However, histology and staining identified the lesion as ameloblastoma. Resection of the tumor was successful with no recurrence 1 month later on follow-up computed tomography. This case represents an unusual imaging presentation of ameloblastoma, a

Deep learning model for differentiating nasal cavity masses based on nasal endoscopy images.

BACKGROUND: Nasal polyps and inverted papillomas often look similar. Clinically, it is difficult to distinguish the masses by endoscopic examination. Therefore, in this study, we aimed to develop a deep learning algorithm for computer-aided diagnosis of nasal endoscopic images, which may provide a more accurate clinical diagnosis before pathologic confirmation of the nasal masses. METHODS: By performing deep learning of nasal endoscope images, we evaluated our computer-aided diagnosis system's assessment ability for nasal polyps and inverted papilloma and the feasibility of their clinical app

Squamous papillomatosis of the bilateral nasal cavities.

The term "papillomatosis" indicates the tendency towards multicentricity and recurrence that these tumors exhibit (Snyder et al. 1972). A typical squamous papilloma arises from the nasal vestibule and is characterized by the epithelial proliferation growing an exophytic manner. We report a rare case of squamous papillomatosis of the bilateral nasal cavities. A 65-year-old man presented with a 2-year-history of bilateral nasal obstruction. Computed tomographic (CT) scans revealed a soft density mass in the bilateral nasal cavities and ethmoid sinuses. Because the tumors were limited to the nasa

Management of FDG avid Benign Sinonasal Schneiderian Papilloma: A Case Report and Review of the Literature.

BACKGROUND: The inverted and oncocytic subtypes of sinonasal Schneiderian papillomas are benign tumors with possible rare malignant transformation and are typically managed with complete surgical resection and close follow-up. While computed tomography (CT) and magnetic resonance imaging (MRI) are mainstays in preoperative evaluation of bony invasion and soft tissue extension of the lesion, their imaging characteristics by 18F-fluoro-2-deoxy-D-glucose positron emission tomography (FDG-PET) is less well characterized. OBJECTIVE: To describe the clinical presentation and management of a PET pos

Oncocytic Schneiderian papilloma occurring in a young Nigerian male: a case report.

Oncocytic Schneiderian papilloma is one of the three morphologically separate tumors that arise from the Schneiderian membrane (the others are fungi form papilloma and inverted papilloma). It is quite rare in our environment. We report a case of Oncocytic Schneiderian papilloma arising from the right maxillary sinus and extending to the ipsilateral nasal region in a 26year old Nigerian male. The growth was present for a period of 5 months before his presentation on 18-10-2005 and recurred again nine months later after excision. Both excised tissues were confirmed by histology to be Oncocytic S

[Characteristics of human papillomavirus (HPV) infection in papilloma of the head and neck--detection of HPV according to clinical features and type specificity in the head and neck].

Papilloma which develops in the head and neck is known to exhibit different clinical features depending on the region, age of onset and tumor growth. We detected human papillomavirus (HPV) DNA using molecular biological methods, and evaluated the characteristics of HPV infection. The subjects examined were 8 cases of laryngeal papilloma, 17 of nasal papilloma, 21 of oral papilloma, 1 of pharyngeal papilloma and 1 of esophageal papilloma. The detection rate of HPV was 100% in the larynx, 29% in the nasal cavity, and 5% in the oral cavity, showing a large difference in the detection rate dependi

Intraductal papilloma of the sublingual gland imitating a ranula.

Ductal papillomas are rare benign lesions of the salivary glands with two distinctive types, intraductal and inverted. The rarest anatomical location where intraductal papillomas occur is the major salivary glands. In the present study, we report an intraductal papilloma of the left sublingual gland with the relevant clinical, imaging, and histopathological findings and postoperative follow-up. To our knowledge, this is the fourth case of an intraductal papilloma of a sublingual gland to be reported in the international literature. We present this rare case because of its clinical and radiolog
