Tropical spastic paraparesis (TSP) is a chronic and slowly progressive endemic myelopathy occurring in geographical isolates in the Caribbean, South India, South Africa, the Seychelles, and Colombia. Extramedullary haemopoiesis (EMH) is a complication commonly associated with beta-thalassaemia intermedia; it is frequently asymptomatic but can sometimes lead to symptomatic tumour-like masses. No guidelines or common consensus are available in literature regarding the different treatment strategies and only single cases have been reported. We describe a case of spinal cord compression due to intrathoracic EMH masses treated with combined radiotherapy and hydroxyurea.

The clinical spectrum of retroviruses is expanding rapidly. Human T-cell lymphotropic virus type I (HTLV-I) was the first retrovirus to be described, and its role had been established in adult T-cell leukemia/lymphoma and tropical spastic paraparesis. We report the case of a 35-year-old woman with HTLV-I and the unusual combination of cutaneous T-cell lymphoma, tropical spastic paraparesis, cerebral vasculitis, and protein S deficiency. CASE: An 8-year-old girl with congenital thoracolumbar kyphosis underwent posterior instrumented correction with a closing-opening wedge osteotomy. Seven years later, minor trauma caused acute paraparesis. Imaging showed intact implants and solid fusion, but muscle herniation into the laminectomy defect had impinged the spinal cord.
