An unusual position of retromandibular vein: a surgical challenge in parotid surgeries.

A 44-year-old man presented with swelling in the left parotid region. The swelling was firm and Fine Needle Aspiration Cytology report proved pleomorphic adenoma. In the CT scan, the tumour was confined to the superficial lobe of parotid. So, left superficial parotidectomy was planned. Modified Wilson Blair's incision was used. On course of identifying the facial nerve, a large calibre vein was identified running vertically through the parotid substance. Assuming it as retromandibular vein, further dissection was carried out more meticulously. Marginal mandibularbranch of facial nerve was iden…

Nodular fasciitis in the parotid gland: a case report and review of the literature.

Nodular fasciitis (NF) is a benign, reactive lesion with a self-limiting process. Because NF is rare in the parotid gland and has many cytological similarities to other benign or malignant tumors, cytological misinterpretation is common. The patient, a 30-year-old woman, had a painless mass in her right parotid gland. Fine needle aspiration cytology (FNAC) was performed. Spindle cells with basophilic and well-demarcated cytoplasm were observed in a mucoid-like background. The mucoid-like substance was metachromatic and appeared to be the matrix of PA. Histopathologically, spindle-shaped cells

Presentation of first branchial cleft anomalies: the Sheffield experience.

Abnormalities of the first branchial cleft are rare. They may present with a cutaneous defect in the neck, parotid region, external auditory meatus or peri-auricular area, or with inflammatory or infective lesions at these sites. A retrospective case note review of the patients treated by the senior author is presented. This group consisted of 18 patients and represents the largest published UK series to date. Eleven patients (65 per cent) had undergone incomplete surgery prior to referral. Over half the patients had a clinically apparent lesion in relation to the external auditory meatus. The…

Parotid metastasis--an independent prognostic factor for head and neck cutaneous squamous cell carcinoma.

BACKGROUND: Metastatic parotid cutaneous squamous cell carcinoma (SCC) is the most common parotid gland malignancy in New Zealand and Australia. The current AJCC TNM staging system does not account for the extent of nodal metastasis. A staging system that separates parotid (P stage) from neck disease (N stage) has been proposed recently. AIM: To review the outcome of patients with metastatic head and neck cutaneous SCC treated at our multidisciplinary Head and Neck Service using the proposed staging system. METHOD: Consecutive patients were culled from our Head and Neck/Skull Base Database,

A case report of primary myxoid liposarcoma mimicking as a parotid cystic lesion.

RATIONALE: Myxoid liposarcoma (MLS) is an extremely rare tumor of the salivary gland and it arises from undifferentiated pluripotent mesenchymal cells. We report a rare case of a primary MLS in the parotid gland. PATIENT CONCERNS: The patient was a 49-year-old female who presented with a hard fixed mass in the left parotid region. DIAGNOSIS: On computed tomography and MR images, this tumor has a low attenuation center with a thick enhancing wall and ill-defined margins. The absence of high-signal-intensity foci on T1-weighted images makes a MLS indistinguishable from most other soft-tissue m…

[Tumor-like amyloidosis of the parotid : A case report].

INTRODUCTION: Tumor-like amyloidosis or amyloïdoma is a nodular lesion related to abundant amyloid deposits that can clinically mimic a malignant tumor. Its etiologic diagnosis requires searching an underlying infectious disease, a connective tissue disorder or a lymphoma. Parotid amyloïdoma is exceptional, and only four cases have been reported in the literature from 1988 to 2021 (PubMed research). CASE REPORT: We reported the case of a 60-year-old, diabetic and hypertensive woman, presenting an isolated swelling of the right parotid region without facial paralysis or cervical lymphadenopath…

Cytological Features of Carcinosarcoma ex Pleomorphic Adenoma of the Parotid Gland: A Case Report.

BACKGROUND: Carcinosarcoma of the salivary gland is an extremely rare tumor composed of carcinomatous and sarcomatoid components. This report describes the cytological and pathological findings of a case of carcinosarcoma ex pleomorphic adenoma arising in the right parotid gland. CASE: A 47-year-old female visited a hospital with swelling of the right parotid region, slight pain and facial palsy. Fine-needle aspiration smears showed both clustered epithelium-like cells and singly scattered cells in a necrotic background. The cells, especially the latter, exhibited significant cellular pleomor…

Basal cell carcinoma metastatic to parotid gland.

Metastasis from basal cell carcinoma of the skin is very rare with cases being documented in the lymph nodes, lung, bone and parotid gland. The main histopathological differential diagnosis is the locally arising basal cell adenocarcinoma from which it is difficult to distinguish by morphology and routine immunohistochemistry. Approximately 85 % of all reported metastatic basal cell carcinomas arise in the head and neck region. Here we present a case of basal cell carcinoma of the skin of the left lateral canthus of the eye which metastasized to the intraparotid lymph nodes with infiltration o

[Primary hydatid cyst of the parotid gland].

INTRODUCTION: The hydatid cyst is an anthropozoonosis due to the development of the Echinococcus granulosus tapeworm larva in humans. Its intraparotid localization is extremely rare. CLINICAL CASE: A 10-year-old-male patient was admitted for an isolated renitent swelling of the left parotid region having grown for several months. Cervical ultrasonography revealed a left parotid cystic mass, characterized by a double wall, and a small daughter cyst. The CT-scan typically showed the absence of enhancement after iodine contrast medium injection. The imaging-based diagnosis of a hydatid cyst was …

Stress response to surgical procedures in the submandibular region and its influence on salivary secretion in mice.

Saliva stimulated by pilocarpine was collected from the mouths of adult ICR male mice after either submandibular-sublingual gland (SM-SL) exposure and SM-SL removal under pentobarbital anaesthesia. Salivary flow rates decreased and salivary protein concentrations increased after both surgical procedures. Serum cortisol was elevated after the operations. Salivary protein separation patterns on sodium dodecylsulphate-polyacrylamide gel electrophoresis (SDS-PAGE) from surgically treated animals were very similar to those from mice given an alpha 1-adrenergic agonist (phenylephrine), but differed

A case of initially metastasizing pleomorphic adenoma of parotid gland.

Metastasis of pleomorphic adenoma (PA) is rare and usually presented as a locoregional recurrence developed many years after excision of the primary tumor although the PA is the most common neoplasm in the parotid gland. We described a case of a 48-year-old male with a parotid tumor with multiple enlarged ipsilateral lymph nodes which suggested a malignancy. The tumors had been neither evaluated nor excised and preoperative evaluation revealed benign PA in both lesions. After the complete surgical excision, the final pathology was notable for benign PA with metastasis to regional lymph nodes.

An anatomy-aware framework for automatic segmentation of parotid tumor from multimodal MRI.

Magnetic Resonance Imaging (MRI) plays an important role in diagnosing the parotid tumor, where accurate segmentation of tumors is highly desired for determining appropriate treatment plans and avoiding unnecessary surgery. However, the task remains nontrivial and challenging due to ambiguous boundaries and various sizes of the tumor, as well as the presence of a large number of anatomical structures around the parotid gland that are similar to the tumor. To overcome these problems, we propose a novel anatomy-aware framework for automatic segmentation of parotid tumors from multimodal MRI. Fir

Parotid duct repair with suturing and anastomosis using tissue adhesive, evaluated by sialography: an experimental study in the dog.

PURPOSE: The most common method of parotid duct anastomosis is suturing. In addition to the postoperative complications in suturing (ie, anastomosis leakage, fistula formation, and infection of maxillofacial spaces), the surgical duration and suturing difficulties arise as major problems. The efficacy of fibrin glue in parotid duct anastomosis was evaluated and compared with suturing in 15 dogs. MATERIALS AND METHODS: Fifteen dogs (of the same breed and gender) were studied in this experimental trial. Intravenous cephalothin (1 g) and intramuscular dexamethasone (8 mg) were administered preop

Longitudinal assessment of parotid function in patients receiving tomotherapy for head-and-neck cancer.

BACKGROUND AND PURPOSE: Conventional radiotherapy is associated with high doses to the salivary glands which causes xerostomia and adverse effects on quality of life. The study aims to investigate the potential of helical tomotherapy (Hi-Art Tomotherapy) to preserve parotid function in head-and-neck cancer patients. PATIENTS AND METHODS: Seven consecutive patients treated with helical tomotherapy at the UZ Brussel, Belgium, were included. During planning, priority was attributed to planning target volume (PTV) coverage: > or =95% of the dose must be delivered to > or =95% of the PTV. Elective

Elective parotidectomy in the management of advanced auricular malignancies.

OBJECTIVE: The purpose of this study is to determine whether elective parotidectomy is necessary in patients with clinically and radiographically N0 parotid disease with isolated primary auricular nonmelanoma cutaneous malignancies. Auricular malignancies are typically managed by wedge resection or wide local excision. Although small lesions are easily managed by conservative excision, large bulky tumors, those with multifocal involvement, and recalcitrant lesions, often require more extensive resection including total auriculectomy, parotidectomy, and/or neck dissection. STUDY DESIGN: A 10-y

The Accessory Parotid Gland and its Clinical Significance.

PURPOSE: The accessory parotid gland is a collection of salivary tissue separate from the main parotid gland. When present, it may complicate parotidectomies, promote parotitis, and serve as a potential site for benign and malignant lesions to arise. The aim of this study was to provide a comprehensive and current overview of the anatomy of the accessory parotid gland, as there is a wide discrepancy in the literature regarding its prevalence. MATERIALS AND METHODS: The authors conducted a search in PubMed, Embase, ScienceDirect, Web of Science, SciELO, BIOSIS, Current Content Connect, Korean
