Survival following intentional succinylcholine injection for self-harm.

Succinylcholine is a short-acting depolarizing neuromuscular blocking agent. We describe a case where the above drug was employed for self-harm by a health-care worker. The patient, a 28-year-old female, was brought to the emergency department (ED) in impending respiratory arrest and altered mental status. On arrival, she had hypoxia, bradycardia, and hypotension. Although the cause for rapid deterioration in this patient was unknown, the ED physician still went ahead by resuscitating the patient's airway, breathing, and circulation. During the course of resuscitation, information was received

Crisis management during anaesthesia: recovering from a crisis.

Preventing harm to the patient is the priority during a crisis. After a major incident, and especially when a patient has been harmed, there are a number of matters to be addressed: the ongoing care of the patient; documentation of the incident; investigation of the root causes; completion of reports; interviews with the patient and/or the next of kin, together with apologies and expression of regret; updates and ongoing support for friends and relatives; a word of thanks to the staff involved for their assistance; formal debriefing of staff for quality assurance and possibly ongoing support a

Electronic prescribing and patient safety: a rigour-stratified systematic review of medication error, adverse drug events, workflow, and communication.

BACKGROUND: Electronic prescribing is promoted to improve medication safety. Whether its benefit differs between process failure and patient harm is rarely asked. OBJECTIVE: To synthesise the evidence on electronic prescribing and medication error, adverse drug events, workflow and communication against the designs able to establish each. METHODS: We searched PubMed, Scopus, Web of Science, CINAHL, Embase and ProQuest (1994 to 2024, English), reporting to PRISMA 2020 and PRISMA-S. Risk of bias was assessed at domain level with design-matched tools. Non-poolable metrics precluded meta-analysi…

Safety Signals in Telehealth Primary Care and Outpatient Settings: Incident and Reporting Analysis Using World Health Organization Classification.

OBJECTIVES: Patient safety in primary health care remains understudied compared with hospital settings, with particularly scarce comparative evidence on incident profiles across outpatient and telehealth modalities. This study addresses these gaps by characterizing safety signals and reporting dynamics in a Brazilian supplementary health program. METHODS: This observational, retrospective study analyzed incident notifications from an institutional reporting system (January 2019 to October 2024), adhering to EQUATOR and STROBE guidelines. Incidents were classified using the World Health Organi

Complications and failure of airway management.

Airway management complications causing temporary patient harm are common, but serious injury is rare. Because most airways are easy, most complications occur in easy airways: these complications can and do lead to harm and death. Because these events are rare, most of our learning comes from large litigation and critical incident databases that help identify patterns and areas where care can be improved: but both have limitations. The recent 4th National Audit Project of the Royal College of Anaesthetists and Difficult Airway Society provides important detailed information and our best estima…

Patient safety in vitreoretinal surgery: quality improvements following a patient safety reporting system.

AIMS: Vitreoretinal (VR) surgery is complex. Most clinical conditions that VR surgeons manage have a high risk for blindness or severe visual impairment. Reporting of patient safety incidents (PSI) in VR surgery was introduced at the Moorfields Eye Hospital (MEH) in the 1990s. We examine the role of PSI reporting in making VR surgery safer at our institution. METHODS: Qualitative review of PSIs from 1997 to 2009 at MEH, London, UK. RESULTS: Over the 13-year study period, 38 789 VR procedures were undertaken and 579 VR PSIs occurring in theatre or inpatient were reported. Mean rate of PSI rep

Peristaltic closure of a safety pin--an unusual fate of a safety pin seen as a foreign body in the gastrointestinal tract.

This report presents a case where an open safety pin passed through the Esophagus and where Nature herself finally solved the problem by closing the pin, which was afterwards carried in the faecal stream and discharged from the body without any harm to the patient.

Dual harm: Violent behaviour to others and self-harm behaviour in adults compulsorily admitted to a Dutch psychiatric hospital.

BACKGROUND: Verbal and physical violence in psychiatric hospitals can have harmful consequences for staff members, such as physical injury, traumatisation, and sick leave, and they often accompany involuntary admission. Harm to others may co-occur with self-harm, i.e., dual harm. However, little is known about the association between dual-harm and violent behaviour towards staff members and its clinical outcomes, such as seclusion and rapid tranquilisation after involuntary admission to a psychiatric inpatient unit. METHOD: A convenience sample of patients admitted involuntarily (N = 384; mea

A preliminary prediction model for self-harm in adolescent inpatients with mental disorders: integrating individual and family factors.

BACKGROUND: Self-harm is a major public health concern among adolescents, particularly those hospitalized with psychiatric disorders. Despite its clinical salience, risk stratification tools incorporating both individual-level and family-level factors remain limited. This study aimed to develop and preliminarily evaluate a prediction model for self-harm among adolescent inpatients with mental disorders. METHODS: This cross-sectional study included 123 adolescent inpatients (aged 12-17 years). Self-harm was defined as intentional self-injury or poisoning regardless of suicidal intent. Demograp

Types of diagnostic errors reported by paediatric emergency providers in a global paediatric emergency care research network.

BACKGROUND: Diagnostic errors, reframed as missed opportunities for improving diagnosis (MOIDs), are poorly understood in the paediatric emergency department (ED) setting. We investigated the clinical experience, harm and contributing factors related to MOIDs reported by physicians working in paediatric EDs. METHODS: We developed a web-based survey in which physicians participating in the international Paediatric Emergency Research Network representing five out of six WHO regions, described examples of MOIDs involving their own or a colleague's patients. Respondents provided case summaries an

A new clinical approach to improve the appropriate use of peripherally inserted central catheters: a prospective study.

OBJECTIVES: Peripherally inserted central catheters (PICCs) are central venous catheters commonly used for administration of chemotherapy, prolonged antibiotic treatment, or parenteral nutrition. It is advisable to use the PICC with the fewest lumens and the smallest possible diameter to reduce major complications. A pharmaceutical analysis and validation of PICC requests was designed to improve efficiency and patient safety. The aim of this study was to evaluate the impact of pharmaceutical interventions (PIs) by the clinical pharmacist in the PICC process. METHODS: A prospective pilot study

Establishing the protocols for the South Australian Emergency Department Admission Blood Psychoactive Testing (EDABPT) programme for drug surveillance.

OBJECTIVE: ED presentations because of illicit use of psychotropic drugs and pharmaceuticals result in significant medical harm and resource consumption. Patient assessment is complicated by the regular emergence of new psychoactive substances, difficulties associated with their identification and a lack of information about their effects. Here we report the protocol for the Emergency Department Admission Blood Psychoactive Testing (EDABPT) programme, an observational study utilising clinical data capture and definitive drug identification to assess the medical impact and patterns of illicit d

Harm reduction techniques among cisgender gay, bisexual, and queer men using anabolic androgenic steroids: a qualitative study.

BACKGROUND: Anabolic androgenic steroids (AAS) are synthetic forms of testosterone frequently used as performance enhancing drugs among gay, bisexual, and queer (GBQ) men. Despite widespread use, associated harms, and the likely existence of an AAS use disorder, there is no medical consensus on standards of care for people who use AAS, with most medical providers focusing exclusively on abstinence. Individuals using AAS have developed community-based harm reduction strategies to mitigate these harms. METHODS: This paper is a sub-analysis of qualitative data obtained through semi-structured in

The Role of Verbal Auditory Hallucinations in Influencing and Retrospectively Predicting Physical Harm Prevalence in Early Psychosis.

BACKGROUND: Research has established a relationship between psychosis and physical harm in the early course of psychosis. However, little is known about the relationship between specific psychosis symptoms, such as hearing voices, and physical harm. OBJECTIVE: This study aimed to determine the prevalence and typology of physical harm related to hearing voices, as well as what aspects of the voice-hearing experience retrospectively predicted incidents of harm within an Early Intervention in Psychosis Service (EIPS). METHODS: We conducted a quality improvement project in a single EIPS. We revi

Observation burden: why surveillance in ophthalmology deserves the same scrutiny as treatment.

Ophthalmology manages a substantial share of its patients through prolonged observation, spanning nearly every subspecialty, from choroidal nevi and glaucoma suspects to optic disc drusen and diabetic retinopathy. Yet surveillance schedules rarely face the scrutiny of cost, benefit and harm demanded of treatment. This article reframes observation as an intervention in its own right, with direct costs, patient burden, psychological harm and downstream testing cascades that are seldom quantified when monitoring protocols are created. It introduces the concept of surveillance inertia, whereby pro

Resilience as a Mediator Between Childhood Trauma and Adult Psychopathology: The Moderating Role of Harm Avoidance in Korean Adults.

Background/Objectives: Childhood trauma is a well-established risk factor for adult psychopathology, yet the underlying neuropsychiatric mechanisms remain unclear. Therefore, this study examined whether resilience mediates the relationship between childhood trauma and depressive and anxiety symptoms, and whether this pathway is moderated by harm avoidance (HA). Methods: A total of 218 Korean adults (aged 19-50 years; 79 men and 139 women) completed validated measures of childhood trauma (Childhood Trauma Questionnaire (CTQ)-Short Form), harm avoidance (Temperament and Character Inventory-Harm

Buprenorphine provider preferences to expanding harm reduction services in rural and suburban primary care settings: a short report.

INTRODUCTION: The opioid overdose crisis continues to worsen in suburban and rural regions. Expanding harm reduction approaches in primary care offers a patient-centered approach to reduce opioid overdoses. METHODS: We conducted a semi-structured survey among 26 buprenorphine prescribers to assess acceptability of office-based harm reduction strategies for people who use opioids (PWUOs) and expanding partnerships with community stakeholders to ensure low-threshold access to buprenorphine. RESULTS: Respondents were mostly very or somewhat supportive of providing HIV testing (96.2%), informati

Medication incidents and medication errors in Danish healthcare: A descriptive study based on medication incident reports from the Danish Patient Safety Database, 2014-2018.

In Denmark, reporting of safety incidents to the nationwide Danish Patient Safety Database (DPSD) is mandatory. Medication incident reports constitute the largest category of safety reports. We aimed to provide numbers and characteristics of medication incidents and MEs reported to DPSD focusing on medication, their severity and the trends therein. This is a cross-sectional study of medication incident reports for individuals ≥18, submitted to DPSD in 2014-2018. We performed analyses on the (1) medication incident and (2) ME levels. Out of 479 814 incident reports, 61.18% (n = 293 536) were re

The Nature and Severity of Adverse Events in Select Outpatient Surgical Procedures in the Veterans Health Administration.

BACKGROUND: Research on adverse events (AEs) in outpatient surgery has been limited. As part of a Veterans Health Administration (VA) projec … es selected based on likelihood of an AE, nearly 40% of identified events carried more than minimal patient harm, undermining the claim that outpatient surgery is relatively safe. Prevalent and preventable AEs such as wound dehiscence and urinary retention may be useful targets for quality improvement.
