Pectus carinatum.

In the mirror of the pertinent literature, we present our experiences gained in 161 operations for pectus carinatum. The Type I (keel chest) deformity is corrected by bilateral resection of the costal cartilages, transverse osteotomy of the sternum, detachment of the xiphoid process, and resection of the lower end of the body of the sternum. The sternum is maintained in its corrected position by utilizing the pulling force of the rectus muscles through the reattached xiphoid and by tacking the pectoralis muscles together in front of the breast bone. Type II (pouter pigeon breast) is handled by…

Silicone Pectoral Implant to Solve Aesthetic Chest Deformity After Pectoralis Flap Harvesting for Laryngotracheal Reconstruction.

BACKGROUND: The pectoralis major musculocutaneous flap has been considered for decades the workhorse in head and neck reconstruction. However, the disadvantages of the pectoralis flap include the morbidity of the donor site in terms of cosmetic and functional results. A silicone pectoral implant  can be used to solve such aesthetic chest deformity in male patients. METHODS: A 33-years-old man with a history of cervical tracheoesophageal fistula after a blunt trauma due to a motorcycle accident, previously reconstructed with a pectoralis major flap, consulted our Plastic Surgery Department for

[A man with an asymmetrical thoracic wall].

A 57- year-old policeman fell during a violent arrest. He presented with pain and a hematoma in the right axillary region. Physical examination showed an asymmetrical contour of the anterior axillary fold and loss of strength during internal rotation and flexion of the right shoulder. These symptoms are specific for a pectoralis major muscle rupture.

Muscle flap reconstruction of shoulder defects.

Defects of the shoulder area pose difficult problems for the hand surgeon, especially when associated with irradiation or exposed, infected bone. The development of muscle and myocutaneous flaps has greatly simplified the reconstruction of such defects. This report discusses the use of the pectoralis and the latissimus flaps for this purpose, with case examples and emphasis upon technique and applications.

RNA-Seq data provide new insights into the molecular regulation of breast muscle glycogen reserves, a key factor in muscle function and meat quality in chickens.

Research is needed to better understand the molecular mechanisms that influence muscle glycogen reserves in chickens due to their critical influence on muscle function and meat quality. In this study, breast muscle RNA sequencing data (RNA-Seq) were used to compare the transcriptomic profile of two original chicken lines divergently selected for breast muscle ultimate pH, which is a proxy for glycogen reserves. Weighted gene co-expression network analysis (WGCNA) of muscle and jejunum RNA-Seq data was also performed to highlight biological processes specifically involved in the gut-muscle dial

Modified radical mastectomy sparing one or both pectoral muscles in the treatment of breast cancer: intra and postoperative complications.

CONTEXT AND OBJECTIVE: Modified radical mastectomy is widely utilized in breast cancer treatment. However, no prospective comparison has yet been made between the Madden technique (preservation of the pectoralis minor muscle) and the Patey technique (resection of this muscle). The aim of this work was to compare these two modified radical mastectomy techniques, by analyzing their degrees of difficulty and complications. DESIGN AND SETTING: Randomized trial at the Breast Unit of Hospital Araújo Jorge, Goiás; and Faculdade de Medicina da Universidade Federal de Goiás. METHODS: 430 patients wit

Musculotopic organization of the motor neurons supplying forelimb and shoulder girdle muscles in the mouse.

We identified the motor neurons (MNs) supplying the shoulder girdle and forelimb muscles in the C57BL/6J mouse spinal cord using Fluoro-Gold retrograde tracer injections. In spinal cord transverse sections from C2 to T2, we observed two MN columns (medial and lateral) both with ventral and dorsal subdivisions. The dorsolateral column consisted of the biceps brachii, forearm extensors, forearm flexors, and hand MNs, and the ventrolateral column consisted of the latissimus dorsi, trapezius, teres major, deltoid, and triceps MNs. The supraspinatus muscle MNs were located in the dorsomedial column

Capillary configuration in contracted muscles: comparative aspects.

We compared the degree of orientation (anisotropy) of capillaries in skeletal muscles of animals with large differences in oxygen needs and/or tolerance to hypoxia (mammals of different size; reptiles; birds; mammals native to high altitude; diving mammals). In terrestrial mammals, we found a substantial increase in capillary tortuosity with fiber shortening, in muscles with large differences in capillary density (capillary counts/fiber mm2 in transverse sections ranging 450-4350). There was no systematic difference in muscle capillary tortuosity with body size (mouse to pony), or with adaptat

Successful use of adipose-derived mesenchymal stem cells to correct a male breast affected by Poland Syndrome: a case report.

Poland syndrome is a rare congenital anomaly in which affected persons are born with missing or underdeveloped muscles on one side of the body. In this case study we present the case of a 28-year-old male with absence of all middle phalanges of the right hand and other rare anomalies, who underwent reconstruction with a new method that combines a mixture of adipose-derived mesenchymal stem cells and fat transfer. The patient's restoration of the pectoralis area was aesthetically successful with no complications and remained unchanged even after 3.5 years. The proposed method represents an inte

CT of interpectoral lymph nodes.

The presence of lymph nodes between the pectoralis major and minor muscles (Rotter's nodes) has been noted in the anatomic and surgical literature. We analyzed the appearance of the interpectoral space and nodes on chest CT scans of 25 patients without known chest wall abnormalities or causes for lymphadenopathy. In some of these cases small structures were detected in the interpectoral fat that could be either vessels or nodes. In addition we studied chest CT scans in six patients with interpectoral adenopathy due to metastases from breast cancer. The nodes were oval soft-tissue densities ran

Exploring the Design of Upper Limb Strength Training Through High-Intensity Interval Training Combined With Exergaming: Usability Study.

BACKGROUND: High-intensity interval training (HIIT) has become a popular exercise strategy in modern society, with the Tabata training method being the most popular. In the past, these training methods were mostly done without equipment, but incorporating exergaming into the training may provide a new option for muscle training. OBJECTIVES: The aim of this study was to explore the differences in upper limb muscle activation using an HIIT program combined with exergaming. METHODS: A total of 15 healthy male participants were recruited for the study, and the differences in muscle activation we

The in vivo passive stretch response of the pectoralis major is region-specific.

The pectoralis major (PM) is a broad muscle commonly divided into three regions, which contribute uniquely to shoulder stability and movement. The PM muscle regions likely respond differently to stretch, but this has never been shown in vivo. We used shear wave elastography to assess the stretch response of different PM muscle regions during shoulder abduction and external rotation in 20 healthy male participants. Participants' shoulder was passively rotated through their range of motion in 5.7° increments and shear wave velocities (SWV) were obtained for each muscle region. A piece-wise model

Correlative anatomy for the sternum and ribs, costovertebral angle, chest wall muscles and intercostal spaces, thoracic outlet.

The structures of the chest wall and thoracic outlet are complex. A working knowledge of their anatomy and of its variations is essential to any thoracic surgeon working in the area. Correlating imaging with anatomy is just as important if one wants to recognize surgical indications, and potential operating difficulties. In the past, conventional radiographic examination was the norm but interpretation was often difficult and incomplete. Currently, CT and MRI are the best available imaging tools, and most times they have complementary roles in the evaluation of chest wall anatomy.

Subclavian and axillary vein compression of musculoskeletal origin.

Twenty instances of subclavian-axillary vein compression of musculoskeletal origin in 14 patients are reported. In 9 thrombosis had occurred and in the remainder the compression was intermittent. The cause appeared to be extraluminar factors such as the subclavius muscle, the scalenus anterior, the pectoralis minor and a malunited fracture of the clavicle. Ten cases were treated surgically with satisfactory results.
