Management of Dens Invaginatus Type II Associated with Immature Apex and Large Periradicular Lesion Using Platelet-rich Fibrin and Biodentine.

Dens invaginatus (DI) poses peculiar challenges in endodontic treatment of teeth because of distortion of pulpal space. A case of Oehlers type II DI with open apex and large periapical lesion is reported. The case was managed using cone-beam computed tomography (CBCT), operating microscope, platelet-rich fibrin (PRF), and Biodentine. A 15-year-old male patient presented with palatal swelling. Pulp sensibility testing of right maxillary lateral incisor was negative. Intraoral periapical digital radiograph revealed an Oehlers type II DI with open apex and periapical radiolucency. A CBCT scan was

Clinical efficacy of treatment procedures in endodontic infection control and one year follow-up of periapical healing.

The objective was to evaluate the clinical efficacy of chemomechanical preparation of the root canals with sodium hypochlorite and interappointment medication with calcium hydroxide in the control of root canal infection and healing of periapical lesions. Fifty teeth diagnosed with chronic apical periodontitis were randomly allocated to one of three treatments: Single visit (SV group, n = 20), calcium hydroxide for one week (CH group n = 18), or leaving the canal empty but sealed for one week (EC group, n = 12). Microbiological samples were taken to monitor the infection during treatment. Peri

[Pathogenesis of apical periodontitis and its effects on the body].

During the last 25 years there have been major advances in understanding the etiology, pathogenesis and maintenance of inflammatory processes taking place in the periapical space. Polymicrobial infection of the pulp chamber is of primary importance in initiating periapical inflammation. Egress of bacteria and their antigenes stimulate the immune system to form a granulation tissue around the apical area. Local immune response eliminates excess number of invading organisms. However, in parallel with protective reactions, local activity of immunocompetent cells and their soluble products also co

Platelet-rich Fibrin assisted Intentional Replantation in Children with Two-year Clinical Outcomes: A Case Series.

Intentional replantation (IR) is a strategic treatment modality involving the deliberate extraction and reinsertion of a tooth following extraoral endodontic management. It is particularly valuable in cases where conventional retreatment or periapical surgery is contraindicated or has previously failed. This case series (n = 3) presents three children managed with IR and contemporary biomaterials to enhance periapical healing. Regenerative adjuncts, including platelet-rich fibrin, chitosan, and xenograft bone grafts, were placed to support hard and soft tissue repair. Teeth were atraumatically

Interferon-gamma-producing cells and inducible nitric oxide synthase-producing cells in periapical granulomas.

Periapical granulomas contain a large number of T lymphocytes and monocytes/macrophages and a small number of B lymphocytes and polymorphonuclear leukocytes. Sections from eight periapical granulomas were stained by a variety of immunohistochemical methods. The vascular endothelial cells stained positively for intercellular adhesion molecule-1 and vascular cell adhesion molecule-1. Helper T cells were identified by immunostaining for CD4 and stained positively for interferon-gamma (IFN-gamma). However, CD4-positive T cells did not stain for interleukin-4 (IL-4). Monocytes/macrophages were iden

Automating Periodontal bone loss measurement via dental landmark localisation.

PURPOSE: Periodontitis is the sixth most prevalent disease worldwide and periodontal bone loss (PBL) detection is crucial for its early recognition and establishment of the correct diagnosis and prognosis. Current radiographic assessment by clinicians exhibits substantial interobserver variation. Computer-assisted radiographic assessment can calculate bone loss objectively and aid in early bone loss detection. Understanding the rate of disease progression can guide the choice of treatment and lead to early initiation of periodontal therapy. METHODOLOGY: We propose an end-to-end system that in

Delayed Apical Pathology during Trigeminal Postherpetic Neuralgia Managed with Antiviral-Antibiotic Therapy: A Case Report.

Postherpetic neuralgia (PHN) is a possible chronic sequela of trigeminal herpes zoster characterized by persistent neuropathic pain that can complicate odontogenic pain assessment. Little is known about potential associations between zoster/PHN and periapical complications years after the acute episode, or whether such pathology might respond to conservative antiviral-antibiotic therapy. This case report describes a 40-year-old man with a childhood varicella history who developed trigeminal herpes zoster in the right mandibular division-preceded by prodromal odontalgia, followed by a vesicular
