Low weight/length ratio to assess risk of cerebral palsy and perinatal mortality in twins.

The etiology of increased rates of cerebral palsy (CP) in twins is unclear, but likely is associated with growth retardation, which occurs more often in twins. Asymmetric growth restriction, a form of growth retardation, has been found associated with increased rates of perinatal morbidity in infants with normal centile birthweights, and occurs more often in twins. Data from 55,457 infants were evaluated. Associations between twinning, CP, and neonatal mortality were evaluated. Influences of confounding factors, such as prematurity, perinatal depression, and asymmetric growth were assessed. Al

[Influence of prenatal control on maternal and perinatal morbidity/mortality in a 2nd-level hospital center].

The objective of this study was to determine the effectiveness of prenatal care on maternal and perinatal morbi-mortality. The obstetrics hospitalizations from "Hospital General Regional, Orizaba, Veracruz" were reviewed from 1991 to 1992. 2595 patients were studied and classified according to the attendance or not to the prenatal care program. We analyzed data related to mother, child and obstetric care. 73% of women had prenatal care with 9.4% of complications. From the group without prenatal care 8.9% had complications. When we compared the demographic characteristics of groups, we found th

The relationship between absent end-diastolic velocities of the umbilical artery and perinatal mortality and morbidity.

To determine the perinatal mortality and neonatal morbidity of fetuses with absent end-diastolic velocities (AEDV) of the umbilical artery, the outcome of 120 fetuses, with a gestational age of 24 weeks or more and a birth weight of 500 g or more, with AEDV at the last Doppler examination, were analyzed. The study population came from 348 women who had pregnancies at high risk of placental insufficiency and had had Doppler velocimetry examinations. In all the women, the Doppler velocimetry result was withheld from the clinician managing the woman. Of the fetuses with AEDV, 57 (52%) died and on…

Government statistics of certified causes of perinatal mortality in Moscow and certain problems with its improvement.

While preparing the Ninth Revision of the International Classification of Diseases, Injuries and Causes of Death (1970-1975), the World Health Organisation sought ways of improving the accuracy of statistics of perinatal mortality. A new Certificate of Cause of Perinatal Death, developed for use in all regions of the USSR and introduced there in 1974, was recommended for introduction in other countries. This was based on the reorganised and more clear-cut coding outlined in Chapter XV of ICD-9. Comparison of the perinatal death rates in 1975 with those in 1986 showed a fall from 25 to 20 per 1…

Perinatal morbidity and mortality in pregnancy hypertensive disorders: prognostic value of the clinical and laboratory findings.

In 521 pregnancies complicated by hypertensive disorders (PHD) and in 200 control cases, we studied the incidence of intrauterine growth retardation (IUGR), depression in the newborns, general morbidity of live newborns requiring admission and perinatal mortality. We also analyzed the relationship between these conditions and the type and severity of hypertension, gestational age, presence of symptoms of the classic EPH triad and of abnormal uric acid values, hemoconcentration, and low urinary estriol values. Perinatal mortality (especially antepartum) was significantly increased in severe pre…

Predictors of perinatal mortality in the seven major hospitals of Lusaka Zambia: A case control study.

BACKGROUND: In 2023, approximately 2.3 million babies globally were lost before birth or within the first week of life, primarily due to preventable causes. Global perinatal mortality declined from 5.7 million in 2000 to 4.1 million by the end of 2015. However, despite this progress, for example 45% of all stillbirths were reported from high-income countries, which contribute less than 2% of the global burden of stillbirths. Perinatal mortality rates for sub-Saharan Africa and Zambia are at 37.4 and 33/1000 live births, respectively. The aim of this study was to determine the predictors of per…

Perinatal Maternal Mortality in Sickle Cell Anemia: Two Case Reports and Review of the Literature.

As outcomes of patients with sickle cell anemia improve and survival into adulthood with good quality of life and expectation of long-term survival becomes more common, challenges have developed, including issues related to reproduction. Pregnancy is frequently complicated in patients with sickle cell anemia with mortality up to 4.0%. Here we report maternal perinatal mortality in two women with sickle cell anemia who died post-partum due to acute chest syndrome (ACS), caused by bone marrow fat embolism and review the literature pertinent to this subject. Patient A was a 28-year-old woman with…

The role of microbial communities in parturition: is there evidence of association with preterm birth and perinatal morbidity and mortality?

In 2005, the World Health Organization estimated that 9.6% or 12.9 million births worldwide were born preterm at <37 weeks of gestation and were accompanied by a mortality rate as high as 42% (http://www.who.int/bulletin/volumes/88/1/08-062554). Significant data suggesting that intrauterine infection is an important modifier for the risk of preterm birth have emerged over the past four decades. However, causative microbial culprits have yet to be identified, and interventional trials with antimicrobials have uniformly failed to demonstrate a significant benefit. To the contrary, treatment for

Utility of local health registers in measuring perinatal mortality: a case study in rural Indonesia.

BACKGROUND: Perinatal mortality is an important indicator of obstetric and newborn care services. Although the vast majority of global perinatal mortality is estimated to occur in developing countries, there is a critical paucity of reliable data at the local level to inform health policy, plan health care services, and monitor their impact. This paper explores the utility of information from village health registers to measure perinatal mortality at the sub district level in a rural area of Indonesia. METHODS: A retrospective pregnancy cohort for 2007 was constructed by triangulating data fr…

Lesser than diabetes hyperglycemia in pregnancy is related to perinatal mortality: a cohort study in Brazil.

BACKGROUND: Gestational diabetes related morbidity increases along the continuum of the glycemic spectrum. Perinatal mortality, as a complication of gestational diabetes, has been little investigated. In early studies, an association was found, but in more recent ones it has not been confirmed. The Brazilian Study of Gestational Diabetes, a cohort of untreated pregnant women enrolled in the early 1990's, offers a unique opportunity to investigate this question. Thus, our objective is to evaluate whether perinatal mortality increases in a continuum across the maternal glycemic spectrum. METHOD…

Placental abruption and perinatal mortality with preterm delivery as a mediator: disentangling direct and indirect effects.

The authors use recent methodology in causal inference to disentangle the direct and indirect effects that operate through a mediator in an exposure-response association paradigm. They demonstrate how total effects can be partitioned into direct and indirect effects even when the exposure and mediator interact. The impact of bias due to unmeasured confounding on the exposure-response association is assessed through a series of sensitivity analyses. These methods are applied to a problem in perinatal epidemiology to examine the extent to which the effect of abruption on perinatal mortality is m…
