[Acute peritonitis due to perforating crohn's disease].

Peritonitis is an infrequent complication of perforating Crohn's disease. Three patients, aged between 25 and 41 years, underwent surgery for peritonitis due to perforating Crohn's disease in the terminal ileum. The underlying disease differed in each patient (one patient was asymptomatic and two presented acute episodes of the disease; of these two patients, corticoids were required in one to control symptomatology). Treatment consisted of intestinal resection with primary anastomosis. In all three patients evolution was satisfactory. Taking only patients treated in our hospital into account,…

[Peritonitis following transrectal biopsy of the prostate].

OBJECTIVE: An uncommon complication of prostatic transrectal biopsy is described. METHODS/RESULTS: A case of peritonitis arising from a recto-peritoneal fistula after transrectal biopsy of the prostate is reported. The patient presented symptoms of peritonitis 15 days after undergoing ambulatory transrectal prostatic biopsy elsewhere. The imaging studies showed intraprostatic hypodense collection and peritonitis due to a recto-peritoneal fistula. The diagnosis was confirmed by laparotomy. The patient died 7 days postoperatively due to acute renal failure. CONCLUSION: Transrectal biopsy of th…

Value of diagnostic laparoscopy in tuberculous peritonitis.

OBJECTIVE: To assess the safety and efficacy of diagnostic laparoscopy in patients with tuberculous peritonitis. DESIGN: Retrospective clinical study. SETTING: University hospital, Turkey. SUBJECTS: 8 patients (2 women, 6 men; mean age 26 years) who presented with tuberculous peritonitis between January 1994 and January 1996. INTERVENTION: Laparoscopy under local anaesthesia with sedation (the 4 who presented with ascites) and laparotomy (the 4 who presented with an acute abdomen). MAIN OUTCOME MEASURES: Clinical and laboratory findings, biochemical and microbiological analysis of ascites…

[Meconium peritonitis and feto-fetal transfusion syndrome].

A case of twin-to-twin transfusion syndrome with intrauterine death of one twin and meconium peritonitis and intravascular disseminated coagulation in the other twin is reported. Meconium peritonitis follows to bowel perforation, caused by segmental severe hypoplasia of muscular layer. The Authors suggest that this structural alteration of bowel wall could be an expression of inequal distribution of some cells between the two twins, during embrional development.

[An experimental model of acute purulent peritonitis].

There are important shortcomings in all the existing models of acute peritonitis. The original model, most close to clinical conditions, has been proposed. The grey rabbits of 1.5-2.0 kg were used in experimental trials. The 5 cm laparotomy has been performed in all the rabbits under thiopental anesthesia. In rabbits of the control group the wound was closed without any manipulations in abdominal cavity. In the other animals the terminal part of the small intestine (15-18 cm) was isolated with two intestinal forceps and saline solution was injected inside. The intestine content was aspirated a…

Macrophages and mesothelial cells in bacterial peritonitis.

Research in recent years has examined the mechanisms underlying cellular host defence in the peritoneal cavity. These studies have established that the resident cells of the peritoneal cavity, the peritoneal macrophages (PM phi) and the mesothelial cells (HPMC) contribute to the initiation, amplification and resolution of peritoneal inflammation. Ex vivo measurements of intra-peritoneal inflammatory mediators during peritonitis has elucidated the time courses for the generation of proinflammatory, chemotactic and anti-inflammatory cytokines and have identified that their secretion occurs large…

[Spontaneous bacterial peritonitis: diagnostic and prognostic aspects].

Spontaneous bacterial peritonitis (SBP) is a frequent complication of cirrhosis with ascites. As clinical symptoms are often mild or lacking, the condition may not be perceived in otherwise severely ill patients. This study focuses on diagnostic and prognostic aspects in 25 patients with 26 episodes of SBP. A microbiological diagnosis was established in 18 patients by positive culture of ascitic fluid or positive gram stain. In 8 episodes the diagnosis was presumed on the basis of an elevated polymorphonuclear leukocyte (PMN) count in the ascitic fluid (> 250 PMN/microliters). The mean (+/- SD…

Generalized peritonitis in India--the tropical spectrum.

Generalized peritonitis is a common surgical emergency in India, the 'Tropical Spectrum' of generalized peritonitis being different from the western spectrum. A total 155 cases of generalized peritonitis were surgically treated at the All India Institute of Medical Sciences between 1981 and 1987, all patients undergoing peritoneal toilet with drainage after the cause of their peritonitis had been treated. The most common cause of peritonitis was peptic ulcer perforation, with simple closure being associated with a 2 per cent mortality, while typhoid perforation was the second most common cause…

[Sclerosing peritonitis following continuous ambulatory peritoneal dialysis].

Sclerosing peritonitis is a severe complication after CAPD treatment. The visceral peritoneum is thickened and interenteric adhesive parts are found. Myofibroblasts are proliferated and the collageneous tissue is hyperplastic. The mean clinical symptom is the mechanical obstruction of the small bowel. We observed this illness in three out of sixty patients under CAPD. These patients had higher incidence of bacterial peritonitis. In the ascites high concentrations of PG E2 and Thromboxan B2 were observed. After treatment of the infection the concentrations fell down to normal values. Electronop…
