Chronic dyspnea unexplained by history, physical examination, chest roentgenogram, and spirometry. Analysis of a seven-year experience.

The purpose of this article is to describe the spectrum and frequency of diseases presenting as unexplained dyspnea and to develop a logical diagnostic approach to such patients. Seventy-two consecutive physician-referred patients had dyspnea greater than one-month duration unexplained by the initial history, physical examination, chest roentgenogram, and spirometry. Patients underwent a standard diagnostic evaluation. A definite cause for dyspnea was recognized in 58 patients, and no answer was found in 14. Twenty-two diseases were recognized in the patient group. Dyspnea was due to pulmonary…

Effectiveness of video learning materials for musculoskeletal physical examinations in medical skills: a randomized controlled trial.

BACKGROUND: Medical students frequently report low confidence in performing musculoskeletal (MSK) physical examinations despite the high prevalence of MSK complaints in clinical practice. Traditional instructional resources have often emphasized procedural demonstrations with limited integration of anatomy and pathology. Video-based learning has emerged as a promising addition, however the optimal design for MSK education remains unclear. This study aims to evaluate the effectiveness of integrated instructional videos on medical student confidence and perceived understanding of MSK physical ex…

Examining the relationship between D3Cr muscle mass, physical performance, and functional limitation in postmenopausal women.

BACKGROUND: The D3-creatine (D3Cr) dilution method provides a non-invasive and accurate assessment of muscle mass. Our objective was to examine the relationship between D3Cr muscle mass, objective measures of physical performance, and self-report physical function postmenopausal women. METHODS: We enrolled 2382 women from the Women's Health Initiative cohort to complete the D3Cr dilution protocol. The primary exposure variable is D3Cr muscle mass, examined as an absolute value in kilograms (kg) and relative to total body weight (D3Cr muscle mass/body mass in kg). Tertiles of absolute D3Cr mus

Examining the Relationship Between Urinary Incontinence and Women's Physical Activity Engagement: Barriers and Disclosure Patterns.

BACKGROUND: This study investigates the relationship between urinary incontinence (UI) and women's participation in physical activity (PA). Women are less active than men across their lifespan, and while interventions aim to bridge this gap, the unique challenge posed by UI remains underexplored. UI disproportionately affects women and often results in reduced self-confidence and avoidance of PA. METHODS: Employing a mixed-methods design, the study utilised an online survey (n = 345) and semi-structured interviews (n = 14) to explore women's experiences of UI during PA and its perceived impac

Examining physical activity participation barriers among adults 50 years and older: a scoping review.

BackgroundAddressing physical activity (PA) barriers is essential for increasing PA levels in middle-aged and older adults. However, there are no recommendations on selecting PA barrier assessment tools.ObjectivesThus, we aimed to identify and provide clinimetric properties on PA barrier assessment tools that healthcare providers, exercise experts, and public health officials can use to examine potential barriers faced by community-dwelling adults 50 years and older.MethodsWe performed a systematic search of the following databases: PubMed, PsycINFO, CINAHL, and Web of Science. Articles were i

Physical and relational aggression as predictors of children's friendship experiences: Examining the moderating role of preference norms.

Aggressive behavior is generally detrimental to children's friendships, both in terms of having friends and in terms of keeping friends. Despite this general tendency, many aggressive children have friends and some of these friendships are stable. We examined the moderating role of preference norms in the classroom and child's sex in the association between children's physical and relational aggression and their friendship experiences. A total of 1135 children (M = 10.24 years, SD = 1.01) in Grades 4 to 6 completed a peer nomination inventory in the Fall (T1) and Spring (T2) of the same school

Evaluation of the Diagnostic Accuracy of the Interview and Physical Examination in the Diagnosis of Endometriosis as the Cause of Chronic Pelvic Pain.

BACKGROUND: Chronic pelvic pain affects approximately 15% of reproductive age women. It is mainly caused by adhesions (20-40%). Despite CPP being the main symptom of endometriosis, the disease is confirmed by laparoscopy only in 12-18% of cases. The aim of this study was to evaluate the results of laparoscopy in women with CCP and to assess the sensitivity and specificity of elements of an interview and clinical examination. MATERIALS AND METHODS: The study included 148 women with CPP. Each patient underwent laparoscopy. In laparoscopy, the presence of endometriosis and/or peritoneal adhesion

Reduced Antibiotic Exposure by Serial Physical Examinations in Term Neonates at Risk of Early-onset Sepsis.

BACKGROUND: Suspected early-onset sepsis (EOS) results in antibiotic treatment of a substantial number of neonates who are uninfected. We evaluated if an approach using serial physical examinations (SPEs) can reduce antibiotic exposure for suspected EOS in term neonates during the first 3 days of life, without affecting safety. METHODS: Within a quality-improvement framework, SPEs for 24-48 hours for neonates with suspected EOS was implemented in the neonatal intensive care unit, Stavanger, Norway. The proportion of neonates ≥37 weeks gestation exposed to antibiotics, antibiotic therapy-days …

Accuracy of Physical Examination to Detect Synovial and Extra-Synovial Pathologies in Psoriatic Arthritis in Comparison to Ultrasonography.

We aimed to explore the accuracy of physical examination (PE) to detect the synovial and extra-synovial pathologies in psoriatic arthritis (PsA) in comparison to ultrasonography (US). Twenty-nine PsA patients with hand pain were included in the study. A detailed PE of the hands was performed and US scans were performed for the joints, extensor and flexor tendons, and entheses of the second to fifth fingers of both hands. The agreement between PE and US findings was calculated. The strongest agreement for the joints was between "swollen joints" and power Doppler (PD) signals in the metacarpopha…

Ultrasound as a replacement for physical examination in clinical staging of axillary lymph nodes in breast cancer patients.

BACKGROUND: The status of axillary lymph nodes (ALNs) is one of the important factors in decision-making for breast cancer treatment. Physical examination (PE) has long been the main, or even the only, means of clinical staging for ALNs in breast cancer. However, the sensitivity and accuracy of PE remains unsatisfactory. The results from this study suggest that axillary ultrasonography (US) should replace PE as a standard method for the clinical staging of ALNs in breast cancer. METHODS: Consecutive and nonselective breast cancer patients treated between September 2018 and November 2018 in ou…

The Relationship Between Physical Examination and Ultrasonography of Large Entheses of the Achilles Tendon and Patellar Tendon Origin.

OBJECTIVE: To investigate the relationship between physical examination (PE) and sonographic features of enthesitis, based on anatomical sites. METHODS: The analysis was done using merged raw data of 3 studies on 2298 entheses. RESULTS: Patients with clinical Achilles enthesitis had more abnormalities on ultrasound (US): hypoechogenicity, p < 0.001; thickening, p = 0.001; Doppler signals, p = 0.002; and erosions, p = 0.02. The patellar tendon origin also correlated with PE but distal patellar tendon insertion and plantar aponeurosis were uncoupled from the US. CONCLUSION: The relationship b…

Intergenerational associations in physical maltreatment: Examination of mediation by delinquency and substance use, and moderated mediation by anger.

Findings as to whether individuals' experiences of physical maltreatment from their parents in childhood predict their own perpetration of physical maltreatment toward their children in adulthood are mixed. Whether the maltreatment experienced is severe versus moderate or mild may relate to the strength of intergenerational associations. Furthermore, understanding of the roles of possible mediators (intervening mechanisms linking these behaviors) and moderators of the intervening mechanisms (factors associated with stronger or weaker mediated associations) is still relatively limited. These is

The reliability of the physical examination to guide fluid therapy in adults with severe falciparum malaria: an observational study.

BACKGROUND: Adults with severe malaria frequently require intravenous fluid therapy to restore their circulating volume. However, fluid must be delivered judiciously as both under- and over-hydration increase the risk of complications and, potentially, death. As most patients will be cared for in a resource-poor setting, management guidelines necessarily recommend that physical examination should guide fluid resuscitation. However, the reliability of this strategy is uncertain. METHODS: To determine the ability of physical examination to identify hypovolaemia, volume responsiveness, and pulmo…
