Three patients with intracranial atypical teratomas presented with symptoms of endocrine dysfunction (diabetes insipidus, hyperprolactinemia, and anterior hypopituitarism) suggestive of a hypothalamic disorder. Computer-assisted tomography done in two of the three cases enabled us to document the presence of a mass and institute radiotherapy, without prior neurosurgical exploration. Computer-assisted tomography also provided a safe and effective method of assessing the effects of radiotherapy.

A retrospective survey of 41 patients with unbiopsied thalamic, pontine, and medullary tumors treated with radiation therapy demonstrated 30% to 40% 3-year survival, with seven long-term survivors. Eleven of the 12 patients surviving over 3 years have essentially normal neurological function. The clinical features of three children in whom a slow tremor involving the head, trunk, or limbs was associated with macrocephaly are presented. The authors retrospectively studied 32 neonates with macrocrania associated with a pericerebral collection. Differential diagnosis was made between external communicating hydrocephalus, atypical communicating hydrocephalus, subdural hygroma and local cerebral atrophy by means of CT scan, isotope cisternography, and isotope brain scintigraphy. Four patients are described with a colloid cyst in the anterior part of the third ventricle, causing obstruction of the foramina of Monro. Stereotactic aspiration of the cyst was performed after exact neuroradiological localization, including CT scan. The great advantages of the technique presented are discussed.

Twelve patients between the years of 1964 and 1971 underwent surgical treatment for the so-called Dandy-Walker syndrome at the Johns Hopkins Hospital. Five patients belonged to an early group (1-8 weeks); four patients to an intermediary group (3-20 months); and a late group consisted of three patients (3 years, 14 years and 34 years). Three patients with nasal visual field defects are described. In each case it is believed that compression of the lateral fibres of the optic nerve by the anterior cerebral or internal carotid artery was the cause. Binasal hemianopia can thus be produced by a single lesion and is as much a true hemianopia as the common bitemporal one. The value of careful neuroradiological investigation to display the relationships of a tumour to the chiasma, optic nerves, and related vessels and thus explain the field defects is demonstrated.

As part of an epidemiological study of congenital hydrocephalus in Utah, we focused on the effect of ascertainment sources and temporal variability to further delineate the causes of this relatively common, handicapping birth defect. The incidence and distribution of 934 reported cases diagnosed prior to age 6 months, and born to Utah residents from 1940 to 1979, were analyzed. Data were ascertained by examination of multiple sources, e.g., 982,066 birth, 11,161 fetal death, and 248,208 death certificates, and selected hospital and clinic records. Twenty-six patients under 20 years of age having cerebrovascular disease were studied from 1968 to 1972. Common risk factors such as hypertension, diabetes mellitus, hyperlipidemia and heart disease were not present. Angiographical study showed a variety of abnormalities. No consistent defect was present. There was a high incidence of pyrexia and convulsions in the early stages of stroke and it appears possible that some form of arteritis might have been important in the production of the cerebral infarction.

Meningoencephaloceles of the temporal bone are rare. Although most often seen following otologic surgery or trauma, congenital meningoencephaloceles can exist.
