OBJECTIVE: To study characteristics of peripheral neuropathy in a group of Thai HIV-infected patients by clinical and electrophysiologic eva … x (MM) for 17.6%, acute inflammatory demyelinating polyneuropathy (AIDP) for 11.8%, and progressive polyradiculopathy (PP) for 5.9% of cases with HIV-related neuropathy. The presence of neuropathy was not correlated with age, sex, body mass index, and duration of HIV infection. However, patients with HIV-related neuropathy had significantly lower nadir CD4 cell cou …

BACKGROUND: Occlusive abdominal aortic thrombus is a rare but critical clinical emergency with life-threatening consequences. Clinical presentation may mimic other diagnoses, resulting in a delay in the appropriate investigations for this condition. Spinal arterial involvement is a recognized complication of aortic thrombus and can result in pain, lower limb weakness, and loss of continence. These symptoms are usually associated with local spinal compression or stenosis manifesting as cauda equina syndrome (CES): a clinical finding of disrupted motor and sensory function to the lower extremiti

A 30-year old man was admitted with right hip pain and gait disturbances. Neurological findings revealed muscular weakness in the lower limbs, hyporeflexia, dysesthesia in the sacral region, and bowel and bladder disturbances. Cerebrospinal fluid (CSF) examination indicated a white blood cell count of 371/μl (lymphocyte:polymorphonuclear leukocyte = 97:3), protein levels of 463 mg/dl and sugar of 20 mg/dl. Although CSF culture was negative, tuberculous infection was presumed. Magnetic resonance imaging revealed areas of enhancement in the intramedullary region surrounding the spinal cord and c

Arteriovenous malformations (AVMs) of the spine display a variety of different locations, angioarchitectures, and clinical presentations. The authors describe an exceptional case of a filum terminale AVM that is not described in any classification and discuss the origin and management of this malformation. A 59-year-old woman was admitted in June 2012 for cauda equina syndrome. Magnetic resonance imaging and spinal angiography revealed an AVM of the filum terminale, located below the conus medullaris, fed by the anterior spinal artery. After an unsuccessful attempt to reach the nidus with a mi

Cauda equina syndrome (CES) is a serious neurological condition and the most common cause is a central disc herniation. Migration of a bullet down the spinal canal is uncommon. In this report, the authors present an unusual case of cauda equina syndrome caused by a migrated bullet in dural sac. The patient's cauda equina syndrome was related to possible compression of cauda equina resulting from both bone fragments in L1 level and bullet itself in S2 level. The patient's symptoms and neurological deficits resolved considerably after surgery. In our opinion, it is important to pay attention to

In this case report, a case of cauda equina syndrome arising from lumbosacral disease in an eight-year-old Burmese cat is described. The cat had a history of chronic right pelvic limb lameness. Neurological evaluation was consistent with a lesion involving the cauda equina. Radiographic findings were consistent with a diagnosis of lumbosacral disease. Due to perceived dorsoventral lumbosacral instability, dorsal decompression and stabilisation of the lumbosacral junction was performed. An original method of spinal stabilisation for this indication is described. The cat had a successful long-te

The use of fibrin glue injection to treat symptomatic dural ectasia is controversial. A case of cauda equina syndrome following percutaneous fibrin glue injection is presented, followed by a review of dural ectasia and its possible treatments.

To report a case of Cauda Equina syndrome with the completion of the paralysis after the reduction of a L4L5 dislocation due to a herniated disc. Although several articles have described a post-traumatic disc herniation in the cervical spinal canal, this is not well known in the lumbar region. A 30-year-old man was admitted to the emergency room with blunt trauma to the chest and abdomen with multiple contusions plus a dislocation of L4-L5 with an incomplete neurological injury. After an emergency open reduction and instrumentation of the dislocation, the patient developed a complete cauda equ

The role of urgent surgery in improving the outcome of cauda equina compression following lumbar central disc prolapse remains controversial. Some series claim improved outcome from emergency decompression whilst others have found no benefit. Resolution of this issue is important because the opportunity to reverse neurological impairment may already have been lost by the time of hospital admission. Removal of a large central disc prolapse can be considerably more difficult than routine discectomy, and may require an extensive exposure. When performed under less than optimal conditions, as ofte

The high incidence of non-specific, benign back trouble often delays the diagnosis of specific causes of back pain and thus result in neurological deficits. Degenerative abnormalities in the cervical spine may narrow the spinal canal and result in acute myelopathy subsequent to minor injuries. Large cervical or lumbar disc herniations causing myelon, conus or cauda equina compression are rare but must not be overlooked. Spinal metastases may remain unrecognised for a long period and lead to the correct diagnosis only after presentation with neurological deficits. Acute myelopathy or cauda equi

Cryptococcosis is an important cause of lymphocytic meningitis, especially but not necessarily in immunocompromised patients. We present the case of a 23-year-old man with a severe and rapid course of a cryptococcal meningoencephalitis, which led to visual and hearing loss, psychotic illness and radiculopathy. There was no evidence of immunodeficiency. Treatment with amphotericin B and flucytosine led to improvement of the symptoms but did not eradicate the micro-organisms from the cerebrospinal fluid (CSF). Maintenance therapy with fluconazole was necessary and led to improvement of the CSF p

The twenty-five patients with the intermittent claudication syndrome of th cauda equina included 19 men and 6 women aged from 23 to 70 years. The results of operation were very good in 16 cases, with improvement in 6 others and failure in 3 cases. There are various mechanisms which can cause the stenosis. The authors stress the importance of congenital vertebral abnormalities and the presence of apophyseal appendices which, at the present time, have not been considered responsible.
