The adult respiratory distress syndrome in association with diabetic keto-acidosis. A case report.

A 41-year-old man presented in stupor, with ketoacidosis and acute severe respiratory failure. He had a history of alcohol abuse and had been on insulin therapy for diabetes secondary to chronic pancreatitis for 11 years. the condition was rapidly progressive and the patient died within 5 hours of presentation of profound hypoxia and hypotension despite aggressive therapy. Autopsy confirmed the clinical diagnosis of 'shock lung'. None of the more commonly associated precipitating factors of adult respiratory distress syndrome could be detected clinically or at autopsy and the pathogenesis of t…

Burning mouth syndrome: a review of etiology, diagnosis, and management.

Burning mouth syndrome (BMS) is a chronic condition characterized by a burning sensation of the oral cavity and is often associated with taste disturbances and xerostomia. It primarily affects menopausal or postmenopausal women. Idiopathic or primary BMS can occur spontaneously and without any identifiable precipitating factors. When BMS is associated with systemic factors, it is defined as secondary BMS. While the exact etiology of BMS is still unknown, the condition appears to be multifactorial, and numerous local, systemic, and psychological factors have been associated with it. Primary BMS…

Is percutaneous glue injection one of the treatments for arteriovenous malformation pinna?

Arteriovenous malformation (AVM) of the head and neck is a rare phenomenon, more so when it is an extracranial AVM like the auricle. AVMs are caused by genetic mutations. Most are probably present in the subclinical form at birth and then evolve; some may arise postnatally or during adolescence or get aggravated by precipitating factors like trauma, infection or hormonal influence like puberty or pregnancy. Once diagnosed, the feeding vessels have to be identified using radiological investigations. They are then embolised via means of percutaneous embolisation and surgical resection.

Evaluation and Management of Gross Hematuria in Autosomal Dominant Polycystic Kidney Disease: A Point of Care Guide for Practicing Internists.

Gross hematuria is common in autosomal dominant polycystic kidney disease (ADPKD). It is an alarming symptom and may be the first manifestation of ADPKD. Cyst hemorrhage is a frequent cause of hematuria in ADPKD while other differential diagnoses include cyst infection, urinary tract infection, renal stones and an underlying malignancy. Knowledge of the precipitating factors and clinical presentation of these conditions will help practicing internists in performing an appropriate evaluation and management of these entities and their complications, as well as executing timely referrals to subsp…

Introduction to myofascial trigger points in dogs.

In dogs, muscles make up 44%-57% of total body weight and can serve as source of both pain and dysfunction when myofascial trigger points are present. However, rarely is muscle mentioned as a generator of pain in dogs, and even less mentioned is muscle dysfunction. The veterinary practitioner with interest in pain management, rehabilitation, orthopedics, and sports medicine must be familiar with the characteristics, etiology, and precipitating factors of myofascial trigger points. Additionally, the development of examination and treatment skill is needed to effectively manage myofascial trigge…

Feline thyroid storm: rapid recognition to improve patient survival.

In human medicine, thyroid storm is a well-recognized condition of acute thyrotoxicosis in which the patient's metabolic, thermoregulatory, and cardiovascular mechanisms are overwhelmed by excessive circulating levels of thyroid hormone. The etiology is unknown, but multiple precipitating factors have been proposed. Hyperthyroid cats presenting in thyrotoxic crisis have clinical signs similar to those of human thyroid storm patients; however, thyroid storm has not yet been fully characterized in veterinary medicine. Early recognition and prompt, appropriate treatment of this life-threatening c…

Masson's intravascular hemangioma masquerading as effort thrombosis.

An otherwise healthy 55-year-old white woman had acute onset of right arm swelling. No precipitating factors were identified. Venograms revealed an occluded subclavian vein, and catheter-directed thrombolysis was performed. After lysis a persistent residual intraluminal filling defect was identified, with persistent symptoms. Partial claviculectomy was performed, the mass was removed, and patch venoplasty carried out, with good outcome. Pathologic analysis demonstrated the mass to be a Masson's hemangioma, a papillary proliferation of thin-walled capillaries intimately associated with thrombus…

Leprosy with Guillain Barre Syndrome: a new neurologic manifestation?

A 19-year-old female patient of lepromatous leprosy with Type II reaction, on multidrug therapy and prednisolone, presented with acute onset flaccid quadriparesis. The cerebrospinal fluid examination revealed albumino-cytologic dissociation. Nerve biopsy showed infiltration with lepra bacilli, features of vasculitis, and demyelination. There were no other identifiable precipitating factors for Guillain Barre Syndrome in this patient. Her condition improved without any steroid therapy. This case emphasizes the hypothesis that cell injury caused by Type II reaction can expose neural antigens and…

Hallucinations in nonpsychotic children: findings from a psychiatric emergency service.

Sixty-two cases of children with hallucinations but without psychosis were identified in a psychiatric emergency service. Auditory hallucinations were more frequent than visual ones. There were positive trends between the content of auditory hallucinations and diagnosis. Recognition of this clinical phenomenon of hallucinations in children in the absence of psychosis and awareness of underlying psychopathology and precipitating factors is necessary in evaluating hallucinations in nonpsychotic children. Children with such presentations run the risk of being misdiagnosed as having psychosis or s…

Violence in the accident and emergency department.

A survey of Accident and Emergency nurses was undertaken to study the prevalence, types and possible precipitating factors of violence in the workplace. Violence was sub-divided into physical and verbal. Verbal violence was more common than physical assault but often not reported. Often physical violence is under-reported but in fact had affected one third of respondents. Associations with different types of violence are discussed. The need for further training in defusing violent situations and analysis of violent incidents is identified. Some suggestions for inclusion in a training programme…

[The reflex sympathetic dystrophy syndrome associated with breast cancer].

Reflex sympathetic dystrophy syndrome (RSDS) is a rare entity of unknown etiopathogenesis, associated to different precipitating factors such as malignant tumors of several localizations. A new clinical variety has been recently described which has been denominated palmar fasciitis and polyarthritis syndrome. We present here two patients with RSDS associated to breast cancer: one case presenting fasciitis and polyarthritis and another case also associated to polymyalgia rheumatica. We emphasize the importance of reducing the tumor mass in the treatment of this syndrome, as well as including it…

Hepatobiliary abnormalities associated with total parenteral nutrition.

Hepatobiliary abnormalities occur often in patients receiving TPN. Although TPN use may be important in their pathogenesis, many other factors could also be involved. In any individual patient it may be difficult to separate the contribution of TPN itself from that of the other precipitating causes. The most worrisome observation to date is the development of severe progressive liver disease in a small number of patients receiving long-term TPN. Further work is needed to determine whether the use of TPN actually contributes to this condition or is simply an associated finding.

[The concept of postconcussional syndrome (author's transl)].

The author shows that postconcussional syndromes develop along three lines: somatic functional troubles, character and affective troubles, efficiency troubles. Most patients present such symptoms within six months after the trauma. In some patients, the syndrome remains for several reasons. The presence of objective sensory troubles, the weakness of socio-economic status and the lack of professional qualification, a psychiatric history and a history of emotional shock constitute precipitating factors. The necessity to determine, along these criteria, a high risk population to be followed psych…

The induction of retinal detachment.

The induction of retinal detachment is dependent upon several factors whose relative importance remains controversial. These factors are retinal breaks, often associated with vitreous traction, inertial forces and changes in chorioretinal adhesion. Retinal breaks may be classified as holes, tears and dialyses. The relation to the vitreous base is an important prognostic indicator. Once a retinal break has occurred, inertial forces disturb the normal vitreo-retinal association. At that point, the development of a clinical detachment also depends upon loosening of the normal chorioretinal adhesi

Primary metabolic alkalosis.

Primary metabolic alkalosis is a disorder caused by both precipitating and perpetuating mechanisms. Correction of the abnormality requires intervention at both levels. Diuretic therapy and prolonged nasogastric suctioning are the major precipitating events, while volume depletion is the most common aggravating factor. Treatment is directed toward correction of dehydration and removal of the precipitating cause when possible. If this does not suffice, hemodialysis with a high chloride dialysate, with or without hydrochloric acid infusion, is the treatment of choice.

Brief report: ultraviolet radiation exposure is associated with clinical and autoantibody phenotypes in juvenile myositis.

OBJECTIVE: Genetic and environmental factors may contribute to the etiology of the juvenile idiopathic inflammatory myopathies (IIMs), which are systemic autoimmune diseases that are characterized by muscle and skin inflammation. We undertook this study to investigate the association between ultraviolet radiation (UVR) exposure and the clinical and autoantibody expression of juvenile IIM. METHODS: The relationship between UVR exposure in the month before symptom onset and the prevalence of juvenile dermatomyositis (DM), compared to juvenile polymyositis (PM), was assessed in 298 juvenile IIM

Study of the polyribosyl-ribitol-phosphate precipitation mechanism by salts and organic solvents.

Precipitation has been widely applied to purification and fractionation of biological macromolecules. Several physical-chemical factors contribute to the destabilization of those solutions, such as the nature of solvent employed, presence of salts, temperature, and concentration of the macromolecule. In the case of charged biopolymers, electrostatic forces are the major contributors to their stability in solution. However, the role of each variable and the exact mechanism of precipitation are not completely understood yet. The aim of this work was to study the precipitation of polyribosyl-ribi

[Characteristics of chemical compositions of precipitation in Beijing].

Characteristics of chemical compositions of precipitation in Beijing were analyzed. The average value of pH was 5.19 from 2005 to 2009, showing stable characteristics of acidification with precipitation. The lowest annual average pH was 4. 87 in 2008 with the highest acidification frequency of 42% and 23% in Chegongzhuang and Daxing districts respectively. The inorganic ion concentrations declined in 5a, indicating an increasing improvement of air quality in Beijing. The concentrations of NH4+ and NO3- were found to increase and contributed to the high nitrogen amount in precipitation. Differe
