BACKGROUND: Mucormycosis is a rare infection caused by ubiquitous fungi of the Mucorales order that mainly affects immunocompromised patients. These fungi have an important tropism for blood vessels that allows them to spread rapidly and cause thromboembolic events.

The association between mature B-cell phenotype and KMT2A rearrangements in acute lymphoblastic leukemia is a very rare finding. Among fungal infection, mucormycosis is a rare but severe etiology in immunocompromised patients. Lung and sinus are the usual sites; the involvement of blood vessels is also described. A 7-y-old boy with relapsed acute lymphatic leukaemia developed fungaemia due to Acremonium strictum, a fungus belonging to the group of the hyaline hyphomycetes. A 22-year-old man presented with multiple raised erythematous skin lesions, pyrexia and epistaxis. A diagnosis of common acute lymphoblastic leukaemia (C-ALL) was made by morphological, cytochemical, immunological and cytogenetic examination of peripheral blood and bone marrow.
