Childhood stressful events, HPA axis and anxiety disorders.

Anxiety disorders are among the most common of all mental disorders and their pathogenesis is a major topic in psychiatry, both for prevention and treatment. Early stressful life events and alterations of hypothalamic pituitary adrenal (HPA) axis function seem to have a significant role in the onset of anxiety. Existing data appear to support the mediating effect of the HPA axis between childhood traumata and posttraumatic stress disorder. Findings on the HPA axis activity at baseline and after stimuli in panic disordered patients are inconclusive, even if stressful life events may have a trig

Cogan's syndrome in a patient with familial Mediterranean fever.

Cerebrospinal fluid (Csf) leak is the leakage of CSF from subarachnoid space into the nasal cavity.Csf rhinorrhoea is classified as traumatic and non traumatic causes (Ommaya et al. J Neurol Neurosurg Psychiatry 31(3):214, 1968). Spontaneous csf leaks occur due to increased intracranial pressure. Early diagnosis of spontaneous csf leaks may prevent complications like meningitis. This was a cross sectional prospective study done in a tertiary care hospital. Group A-spontaneous csf leak patients and Group B- Non csf leak matched controls. Haematological toxicity of drugs used in psychiatry.

Almost all classes of psychotropic agents have been reported to cause blood dyscrasias. Mechanisms include direct toxic effects upon the bone marrow, the formation of antibodies against haematopoietic precursors or involve peripheral destruction of cells. Agranulocytosis is probably the most important drug-related blood dyscrasia. The mortality from drug-induced agranulocytosis is 5-10% in Western countries. The manifestations of agranulocytosis are secondary to infection. Aggressive treatment with intravenous broad-spectrum antimicrobials and bone marrow stimulants may be required. Of drugs e

Rumination-focused cognitive behaviour therapy for residual depression: a case series.

The treatment of chronic and recurrent depression is a priority for the development of new interventions. The maintenance of residual symptoms following acute treatment for depression is a risk factor for both chronic depression and further relapse/recurrence. This open case series provides the first data on a cognitive-behavioural treatment for residual depression that explicitly targets depressive rumination. Rumination has been identified as a key factor in the onset and maintenance of depression, which is found to remain elevated following remission from depression. Fourteen consecutively

Tobacco smoking within psychiatric inpatient settings: biopsychosocial perspective.

Tobacco smoking remains a neglected issue within general psychiatry despite high rates of associated morbidity and mortality. While there has been a coordinated community campaign to reduce tobacco smoking within the general population, mental health facilities have been reluctant to adopt such regulatory controls, and rarely target smoking prevention or treatment. This paper summarizes and discusses evidence relating to the clinical impact of tobacco smoking within inpatient psychiatric settings. A selective review of psychiatric and psychological research on smoking within inpatient settings

Women's reproductive health and depression: a community survey in the Gambia, West Africa.

BACKGROUND: Depression is the commonest mental illness in developing countries and impoverished women are most at risk. Formal mental health services in these situations are rare. Depression commonly co-presents with physical symptoms or else is unspectacular, so the condition often goes unrecognised. To strengthen the prevention and management of depression, information is required on easily recognisable correlates of depression. This study explored associations between depression and reproductive health conditions in rural African women of reproductive age. METHODS: A community-based reprod

The relationship between eruption and length of mandibular incisors in young rats.

OBJECTIVE: To investigate whether women raised in a family with substance abuse constitute a particularly vulnerable group of patients in an infant psychiatry setting and to identify the risk factors of suspected parental malfunctioning in women referred to treatment in an infant and toddler intervention program. BACKGROUND: A history of family substance abuse can severely disrupt the caretaking abilities of parents in ways that can have far-reaching consequences, and children growing up with insufficient parental care may incorporate this deficiency into their own parental behavior. METHODS

[How much treatment does a person need? Addiction, spontaneous remission and "family" as biographical as leitmotiv].

Referring to theoretical and empirical examples taken from psychiatry, psychology and sociology, this paper first clarifies the dimensions and application of concepts such as 'natural recovery' and 'spontaneous remission'. In order to illustrate the general development of research in the area of therapy free drug remission from a purely quantitative approach to a qualitative, ethnographic one, two studies are referred to: the classic Vietnam veteran study done by Robins and a longitudinal analysis of a network of cocaine users. This is also supplemented by the latest results from Germany. Next

Children and adolescents at risk for seclusion and restraint in inpatient psychiatric treatment: a case control study.

UNLABELLED: To reduce coercion in acute inpatient child and adolescent psychiatric units, a better understanding of individuals at risk for seclusion and/or restraint (S/R) is needed. We report data on the proportion of patients secluded/restrained and factors associated with higher risk of S/R. Identifying preventative mechanisms through risk stratification upon inpatient admission can aid the training of mental health professionals, and support shaping specific workflows for at-risk populations for example by joint crisis plans or post-coercion review sessions. METHODS: A case-control study

The investigation of factors related to suicide attempts in Southeastern Turkey.

BACKGROUND: Suicide is an important health problem in Turkey as it is in all regions of the world. Suicidal behavior has multiple causes, which are broadly divided into those related to proximal stressors and those due to predisposition. Suicide statistics may be associated with mental health disorders, which are among the foremost predictors of suicide attempts. More than 90% of patients who commit suicide have a diagnosable psychiatric disorder, usually a major depressive disorder. Other major risk factors for suicide attempts are history of suicide attempts in the family, stressful life eve

The Vilayam Framework: a conceptual model of bidirectional human development from devolution to flourishing.

Contemporary developmental psychology, positive psychology, prevention science, and contemplative research have substantially advanced understanding of human growth, resilience, flourishing, and the factors associated with psychological and behavioural outcomes across the lifespan. However, comparatively less theoretical attention has been devoted to whether the developmental processes that precede observable manifestations constitute a distinct domain of scientific inquiry. This article introduces the Vilayam Framework, a conceptual model proposing that human development may be understood as

Impact of Nitrous Oxide on Neuropsychiatric Impairment in Adolescents: A Scoping Review.

Nitrous oxide (N₂O) misuse represents a growing public health issue, particularly among adolescents and young adults, who use it recreationally. This scoping review investigates recent and foundational studies on the long-term neuropsychiatric effects associated with prolonged and frequent use of this volatile gas. Various clinical databases and journal archives, such as those of the Journal of Addiction Medicine, Journal of Child Psychiatry, Journal of Anesthesiology, Science Direct, and National Survey of Drug Use and Health, were queried for clinical trials, case reports, and public health

Risky sexual behavior and associated factors among people with severe mental illness in Africa: A systematic review and meta-analysis.

BACKGROUND: Individuals, who suffer from severe mental illnesses (SMI), such as bipolar disorder (BD), schizophrenia (SCZ), and major depressive disorder (MDD), are more likely to engage in risky sexual behavior. A severe mental illness can lead to a decrease in impulse control, a reduction in cognitive function and memory, and a psychosocial impairment that increases risky sexual behavior. Risky sexual behavior (RSB) can lead to health problems such as sexually transmitted infections, including HIV/AIDS, early pregnancy, and unplanned pregnancies. The purpose of this systematic review and met

Psychodynamic accessibility: a testable framework for supported agency in social psychiatry and psychiatric rehabilitation.

Recovery-oriented social psychiatry and psychiatric rehabilitation increasingly emphasize rights, participation, community-based care, supported decision-making, supported employment, and prevention-oriented early intervention. Yet formal access to services, information, rehabilitation, legal-administrative procedures, or participation formats does not by itself ensure that people can use these resources when shame, mistrust, affective overload, fatigue, cognitive narrowing, or structural vulnerability are already present. This Hypothesis and Theory article introduces psychodynamic accessibili

The Bipolar Illness Onset study: research protocol for the BIO cohort study.

INTRODUCTION: Bipolar disorder is an often disabling mental illness with a lifetime prevalence of 1%-2%, a high risk of recurrence of manic and depressive episodes, a lifelong elevated risk of suicide and a substantial heritability. The course of illness is frequently characterised by progressive shortening of interepisode intervals with each recurrence and increasing cognitive dysfunction in a subset of individuals with this condition. Clinically, diagnostic boundaries between bipolar disorder and other psychiatric disorders such as unipolar depression are unclear although pharmacological and

Patterns of multimorbidity and their association with edentulism: the moderating role of health literacy in the Lifelines Cohort.

Multimorbidity may affect oral health, especially in individuals with limited health literacy (HL), or specific multimorbidity patterns; however, evidence remains scarce. Understanding these associations could improve early detection and prevention, both in clinical care and public health strategies. This study aimed to assess the association of multimorbidity and multimorbidity patterns with complete tooth loss, i.e. edentulism, and whether HL moderates this association. A total of 42 357 participants from the prospective multigenerational Dutch Lifelines Cohort Study were included. Multimorb
