Intra- and early (first week) post-operative haemorrhages are the most common complications in stapled hemorrhoidectomy PPH (Procedure for Prolapse and Hemorrhoids) and in circumferential resection of the rectal prolapse STARR (Stapled Trans Anal Rectal Resection). Performing PPH and STARR we employed a gelatin based haemostatic sealant with thrombin component (FloSeal) to control intra-operative bleeding and to reduce post-operative bleeding avoiding haemostatic stitches on suture line. We report the preliminary results on 197 PPH and 64 STARR; 44 PPH (22.4%) and 27 STARR (42.2%) were treated

This review focused on an examination of the literature on the surgical treatment of low-back pain in patients with DDD or low-grade degenerative spondylolisthesis treated with PLF, with or without the use of pedicle screw fixation. All Class I and the majority of Class II and Class III medical evidence on this topic indicates that the addition of pedicle screw fixation to PLF increases fusion success rates when assessed based on plain x-ray films with dynamic imaging. Although there does appear to be a positive relationship between radiographic fusion and clinical outcome, no convincing corre

BACKGROUND: Fluoroscopically guided lumbar epidural steroid injections (ESIs) are commonly performed to treat lumbar radicular pain. Intraprocedural pain is an important patient centered aspect of the procedural experience and may influence satisfaction, anxiety, and willingness to undergo repeat interventions. Although prior studies have suggested that interlaminar approaches may be less painful than transforaminal injections, the patient and procedure related factors associated with intraprocedural pain in routine clinical practice remain incompletely characterized.
OBJECTIVE: 1. To evaluat…

Originally published in Pain Management, this article is a summary of a study performed to look at the benefit, if any, of more than one epidural steroid injection in the spine before the mild® Procedure. Minimally invasive lumbar decompression (commonly known as the mild Procedure) and epidural steroid injections are both common treatment options for lumbar spinal stenosis (commonly referred to as LSS), a condition that causes chronic lower back pain in older adults. To determine how to best treat LSS patients, healthcare professionals use a guide to help with the decision-making process (cal

BACKGROUND: The Nuss procedure corrects pectus excavatum by forceful displacement of the sternum with metal bars. Optimal pain management remains a challenge. Mutimodal anesthesia alleviates pain through blockade of multiple nociceptive and inflammatory pain receptors.
METHODS: A retrospective chart review of anesthetic and postoperative mangement of 125 children undergoing the Nuss procedure was conducted. Anesthetic mangement strategies were analyzed in four groups: opioid during general anesthesia (GA), epidural with general anesthesia (Epidural), multimodal anesthesia (MM), and multimodal

STUDY OBJECTIVE: Minimally invasive repair of pectus excavatum (Nuss procedure) is associated with significant postoperative pain and high-dose opioid consumption. Liposomal bupivacaine (LB), as an ultra-long-acting local anesthetic, has been extensively studied for perioperative analgesia. However, data regarding the effects of LB on postoperative pain in children undergoing the Nuss procedure remain limited. The aim of this study was to evaluate the ability of LB to reduce postoperative opioid use among children undergoing the Nuss procedure.
DESIGN: Randomized controlled trial.

Establishing an appropriate treatment strategy for patients presenting with low-back pain, in the absence of stenosis or spondylolisthesis, remains a controversial subject. Inherent to this situation is often an inability to adequately identify the source of low-back pain to justify various treatment recommendations, such as lumbar fusion. The current evidence does not identify a single best treatment alternative for these patients. Based on a number of prospective, randomized trials, comparable outcomes, for patients presenting with 1- or 2-level degenerative disc disease, have been demonstra

BACKGROUND: In complex anal fistulas, treatment should achieve healing while preserving continence and quality of life. However, direct comparative evidence across sphincter-preserving, sphincter-dividing/reconstructive, and seton-based approaches remains limited. We compared these techniques for postoperative fecal incontinence, pain, quality of life, and recurrence.
METHODS: We followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses 2020 statement and the Preferred Reporting Items for Systematic Reviews and Meta-Analyses-Network Meta-Analysis checklist to report our

BACKGROUND: Lumbar spinal stenosis (LSS) is a serious degenerative condition of the spine that can cause significant functional disability. Therapies for these patients generally begin with conservative management, since more invasive interventions such as open surgery and spinal implants are associated with higher complication rates. Early in the treatment algorithm for LSS patients, multiple conventional medical management (CMM) therapies are often combined as an initial low-risk treatment strategy. This composite first-line treatment plan may include conservative care together with early in

UNLABELLED: Streptococcus pneumoniae remains a leading bacterial pathogen causing pneumonia, meningitis, septicemia, and post-viral respiratory infections such as COVID-19. Despite licensed vaccines, efficacy is limited by serotype dependency, cost, and restricted coverage, highlighting the need for broad-spectrum alternatives. Protein-based vaccines with conserved antigens represent a promising strategy. This study aimed to design and evaluate an efficient peptide-based vaccine against pneumococcal virulence factors using immunoinformatics tools. Two major pneumococcal surface proteins were s

BACKGROUND: Management strategies for the recurrence of trigeminal neuralgia after microvascular decompression include repeat procedures, medical management or no further therapy. No consensus exists as to which strategy is best for pain relief. The aim of this study was to determine the characteristics of patients with recurrences after microvascular decompression in the cohort, and to compare long-term pain relief between different management strategies.
MATERIALS AND METHODS: A historical cohort of patients who underwent microvascular decompression at a neurosurgical institution between 19

OBJECTIVE: Complex regional pain syndrome (CRPS) describes a broad spectrum of symptoms that predominantly localize to the extremities. Although limb fracture is one of the most frequently reported triggering events, few large-scale studies have shown the occurrence of and factors associated with CRPS following limb fracture. This study aimed to show the occurrence and identify of those factors.
METHODS: Using the Japanese Diagnosis Procedure Combination database, we identified 39 patients diagnosed with CRPS immediately after open reduction and internal fixation (ORIF) for limb fracture from

OBJECT: Gamma Knife surgery (GKS) is a treatment option for patients with refractory typical trigeminal neuralgia (TN), TN with atypical features, and atypical types of facial pain. The Gamma Knife's 201 60Co sources decay with a half-life of 5.26 years. The authors examined whether the decrease in dose rate over 4.6 years between Co source replacements affected the control rates of facial pain in patients undergoing GKS.
METHODS: The authors collected complete follow-up data on 239 of 326 GKS procedures performed in patients with facial pain. Patients were classified by their type of pain. T

BACKGROUND AND IMPORTANCE: Although various strategies have been examined to mitigate discomfort during sampling, arterial blood gas (ABG) is a common and often painful procedure in emergency departments (EDs). Hypnotic communication, characterized by positive language/suggestions, may help reduce perceived procedural pain. Conversely, the traditional use of negative language may increase discomfort through a 'nocebo' effect.
OBJECTIVE: To assess whether hypnotic communication reduces procedural pain during ABG sampling compared with neutral or nocebo communication, when delivered by emergenc…
