BACKGROUND: Cardiac involvement in systemic sclerosis (SSc) is under-recognized and may present as fulminant myocarditis with cardiogenic shock and poor outcomes.
CASE SUMMARY: A 49-year-old woman with diffuse SSc presented with progressive dyspnea and hypotension. Laboratory testing showed markedly elevated cardiac biomarkers, and echocardiography demonstrated severe biventricular systolic dysfunction. Electrocardiography revealed sinus tachycardia with right bundle branch block. Cardiac magnetic resonance showed diffuse biventricular myocardial inflammation, confirmed by endomyocardial biop

Pulmonary alveolar proteinosis (PAP) is a rare pulmonary disorder characterized by abnormal surfactant accumulation due to defective alveolar macrophage clearance. It often presents with exertional dyspnea and nonspecific imaging findings, commonly leading to diagnostic delays. We present the case of a 56-year-old woman with progressive hypoxia initially misattributed to acute respiratory distress syndrome, noncardiogenic pulmonary edema, and hypersensitivity pneumonitis. Corticosteroids were initiated based on early differential considerations. Further evaluation revealed concurrent methicill

BACKGROUND: Pemphigus vulgaris is an autoimmune blistering disease that may initially present as localized lesions. It rarely remains localized throughout its clinical course.
OBSERVATIONS: A 53-year-old woman with non-progressive pemphigus vulgaris localized to the tongue for 18 years is presented. Clinical examination showed erosions and ulcerations limited to the lateral margins of the tongue. Patient was treated with sublesional triamcinolone-acetonide injections as lesions recurred. Finally, triamcinolone- acetonide injections at three weeks intervals for three months induced a longterm

Cerebellopontine angle masses are most frequently vestibular schwannomas, but may also represent metastatic disease, particularly in patients with a history of malignant melanoma. We describe the case of a 72-year-old woman with a prior history of melanoma and a right cochlear implant who presented with rapidly progressive bilateral hearing loss, visual impairment, imbalance, and right-sided facial weakness. An initial non-contrast CT scan was reported as normal, although a later multidisciplinary review identified bilateral cerebellopontine angle lesions. MRI, the gold standard for characteri

INTRODUCTION: Mucopolysaccharidosis type III (MPS III), also known as Sanfilippo syndrome, is a lysosomal storage disease with progressive neurodegenerative features, predominantly affecting the central nervous system. Diagnosis is based on clinical features, with neurodevelopmental and neuropsychiatric alterations taking precedence, including over phenotype alterations. The disease is confirmed by biochemical analysis to identify the type of glycosaminoglycans present, enzyme assay and molecular genetic studies.
CASE REPORTS: A clinical description was performed for eight patients diagnosed

Advances in laboratory techniques have revolutionized parasitology diagnostics over the past several decades. Widespread implementation of rapid antigen detection tests has greatly expanded access to tests for global parasitic threats such as malaria, while next-generation amplification and sequencing methods allow for sensitive and specific detection of human and animal parasites in complex specimen matrices. Recently, the introduction of multiplex panels for human gastrointestinal infections has enhanced the identification of common intestinal protozoa in feces along with bacterial and viral

BACKGROUND Phosphorous is an essential component of cell structure and physiology, and is required for energy conservation and expenditure. Severe hypophosphatemia can lead to profound dysfunction and injury affecting most organs and can be life-threatening. It can also compromise weaning of mechanically ventilated patients. Long-term assisted ventilatory care in ambulatory or inpatient settings is an expanding medical service for patients with various forms of persistent or progressive incapacitating diseases. Hypophosphatemia, caused by respiratory alkalosis, has been reported in critical-ca

BACKGROUND: Gastric rupture is a rare yet potentially fatal complication that has been reported during cardiopulmonary resuscitation (CPR). This is generally attributed to acute gastric overdistension caused by chest compressions combined with positive-pressure ventilation. Ventilation-induced gastric rupture after planned extubation in the intensive care unit (ICU) is rare.
CASE PRESENTATION: An 80-year-old man developed acute gastric rupture shortly after planned ICU extubation following lumbar spinal surgery. Quantitative neuromuscular monitoring was not performed prior to extubation. Four

IgA nephropathy (IgAN) is a very common glomerulonephritis worldwide. In this review, we discuss therapeutic options in four clinical scenarios encountered in patients with IgAN: first is the patient with minor urinary abnormalities where the mainstay of treatment is long-term, regular follow-up to detect renal progression and hypertension. Second is the typical patient presenting with microhematuria, significant but non-nephrotic proteinuria, hypertension, and variable degrees of renal failure. Here the mainstay of treatment is optimized supportive care. If this does not lower proteinuria bel

The life-threatening DRESS (drug rash with eosinophilia and systemic symptoms) syndrome is characterized by the presence of at least three of the following findings: fever, exanthema, eosinophilia, atypical circulating lymphocytes, lymphadenopathy, and hepatitis. This syndrome is difficult to diagnose, as many of its clinical features mimic those found with other serious systemic disorders. This idiosyncratic reaction occurs most commonly after exposure to drugs such as allopurinol, sulfonamides, and aromatic anticonvulsants such as phenytoin, phenobarbital, and carbamazepine. We describe a 44

Inferior vena cava (IVC) thrombosis is a rare but serious complication. We report a case of delayed presentation of abdominal compartment syndrome (ACS) secondary to IVC thrombosis, which contributed to progressive multi-organ dysfunction syndrome. Increased intra-abdominal pressure further resulted in venous stasis and thrombus formation, exacerbating systemic deterioration. Despite supportive care and attempted interventions, the patient developed refractory organ failure and expired. This case underscores the critical importance of early diagnosis and timely management of ACS to prevent vas

Severe coronavirus disease 2019 can be associated with progressive respiratory failure. In addition to respiratory support and other supportive care, use of corticosteroids has shown to improve outcome. Despite the use of steroids, a significant proportion of patients progressively worsen. Adjunct immunomodulators have been studied in addition to steroids in these patients. Here we present a successful use of tofacitinib, a Janus Kinase inhibitor, in conjunction with dexamethasone for a patient with rapid worsening of respiratory status and with high level of serum inflammatory biomarkers.

Progressive glomerular damage can occur as a result of various etiologic factors including infections, medications, diseases, and autoimmune disorders. This article discusses the clinical management of the leading conditions associated with glomerular disease, including glomerulosclerosis, diabetic nephropathy, focal segmental glomerulosclerosis, and membranous nephropathy. Glomerular damage and disease progression may lead to end stage renal disease. Clinical management is individualized, as based on causative factors and clinical manifestations, with the overall goal of limiting glomerular d

EVALUATION: A 63-year-old female Jehovah's Witness with familial idiopathic pulmonary fibrosis underwent elective bilateral lung transplantation at a private quaternary hospital in southeastern Brazil. Intraoperatively, Cell Saver technology was used to minimize blood loss. Postoperatively, she developed severe anemia (Hb < 7 g/dL), primary graft dysfunction (grade 2), and septic shock. Due to her religious beliefs, she refused allogeneic blood transfusion, creating unique challenges for the intensive care team.
DIAGNOSIS: Nursing diagnoses included risk of ineffective tissue perfusion relate

Despite advances in the detection, pathological diagnosis and therapeutics of lung cancer, many patients still develop advanced, incurable and progressively fatal disease. As physicians, the duties to cure sometimes, relieve often and comfort always should be a constant reminder to us of the needs that must be met when caring for a patient with lung cancer. Four key areas of end-of-life care in advanced lung cancer begin with first recognizing 'when a patient is approaching the end of life'. The clinician should be able to recognize when the focus of care needs to shift from an aggressive life

Systemic sclerosis (SSc), the most lethal of rheumatologic conditions, is the cause of death in >50% of SSc cases, led by pulmonary fibrosis followed by pulmonary hypertension and then scleroderma renal crisis (SRC). Multiple other preventable and treatable SSc-related vascular, cardiac, gastrointestinal, nutritional and musculoskeletal complications can lead to disability and death. Vascular injury with subsequent inflammation transforming to irreversible fibrosis and permanent damage characterizes SSc. Organ involvement is often present early in the disease course of SSc, but requires carefu

BACKGROUND: Huntington's disease is a complex neurodegenerative hereditary disease with symptoms in all domains of a person's functioning. It begins after a healthy start in life and leads through the relentless progression over many years to complete care dependency and finally death. To date, the disease is incurable. The long progressive complex nature of the disease demands multiple disciplines for treatment and care of patient and family. These health care providers need inter- and multidisciplinary collaboration to persevere and be efficacious in this devastating disease trajectory.

BACKGROUND: Huntington's disease (HD) is a rare, genetic, neurodegenerative and ultimately fatal disease with no cure or progression-delaying treatment currently available. HD is characterized by a triad of cognitive, behavioural and motor symptoms. Evidence on epidemiology and management of HD is limited, especially for Germany. This study aims to estimate the incidence and prevalence of HD and analyze the current routine care based on German claims data.
METHODS: The source of data was a sample of the Institute for Applied Health Research Berlin (InGef) Research Database, comprising data of

Multiple system atrophy (MSA) is a rare, rapidly progressive neurodegenerative disorder characterized by varying combinations of Parkinsonian features, cerebellar ataxia, and autonomic dysfunction. The disease is classified into two major subtypes based on predominant symptoms: The Parkinsonian variant (MSA-P) and the cerebellar variant (MSA-C). MSA-C, in particular, presents significant diagnostic challenges due to its overlapping clinical features with other neurodegenerative disorders, such as cerebellar ataxias and atypical Parkinsonian syndromes. Early stages of the disease may not show c

INTRODUCTION: The alpine pit viper, Trimeresurus gracilis, is an endemic species in Taiwan. The incidence of human envenoming is rare.
CASE SUMMARIES: We present three events in two patients bitten by Trimeresurus gracilis. In the first patient, envenoming inflicted pain, local bleeding, hemorrhagic bulla, and progressive swelling, leading to necrosis of the bite wound. In the second patient, the two snakebites caused pain and progressive swelling. There were no systemic effects such as organ damage or neurological deficits observed.
