Freezing Nitrogen-Ethanol Composite Effectively Eradicates Staphylococcus aureus Biofilm on Prosthetic Surfaces.

BACKGROUND/AIM: Periprosthetic joint infection (PJI) remains one of the most challenging complications after arthroplasty, largely due to biofilm formation on prosthetic surfaces, which protects bacteria from antibiotics and host defenses. This study investigated whether a freezing nitrogen-ethanol composite (FNEC), a semisolid cryogenic material combining liquid nitrogen and ethanol, can effectively eradicate Staphylococcus aureus biofilms on prosthetic components. MATERIALS AND METHODS: Biofilms of S. aureus were established on plastic spacers and metallic knee prostheses. Specimens were tr

Anesthetic Management and Infection Risk in Hip and Knee Arthroplasty: A Narrative Review.

Surgical site infection (SSI) and periprosthetic joint infection (PJI) remain among the most serious complications after total hip and knee arthroplasty, potentially leading to delayed recovery, revision surgery, prolonged antimicrobial treatment, increased healthcare costs, and worse functional outcomes. This narrative review evaluates how perioperative anesthetic management may influence infection risk in patients undergoing hip and knee arthroplasty and summarizes practical measures that anesthesia providers can take to contribute to infection prevention. The review first compares general,

Using biofilm-targeting local antibiotics to treat recalcitrant medical device-associated infections: driveline infection as a model.

Driveline infection, exemplifying medical device-associated infection, is a difficult-to-treat complication of ventricular assist device implantation in which biofilm formation underlies both infection onset and recurrence. Although clinical experience remains limited, treatment success has been reported in cases where local antibiotics at high concentrations were used in combination with surgical debridement and/or device reimplantation. We recently identified the low metabolic state of biofilm cells as the central mechanism underlying the high antimicrobial resistance of staphylococcal biofi

Diagnostic and therapeutic strategies in combating implanted medical device-associated bacterial biofilm infections.

Bacterial biofilms exhibit remarkable resistance against conventional antibiotics and are capable of evading the humoral immune response. They account for nearly 80% of chronic infections in humans. Development of bacterial biofilms on medical implants results in their malfunctioning and subsequently leads to high mortality rates worldwide. Therefore, early and precise diagnosis of bacterial biofilms on implanted medical devices is essential to prevent their failure and associated complications. Culture-based methods are time consuming, more prone to contamination and often exhibit low sensiti

Two-Stage Revision for Pseudomonas-Infected TKA in a Patient With Elephantiasis Nostras Verrucosa: A Rare Case Report.

CASE: A 63-year-old woman presented with an infected left total knee arthroplasty (TKA) having pain, sinus, and cobblestone-like swelling due to elephantiasis nostras verrucosa (ENV). A two-stage revision was performed, with strict asepsis and wound management. At midterm 15-month follow-up, no infection or loosening was noted, and daily knee functions improved. CONCLUSION: No established guidelines exist for revision TKA in ENV. In ENV with structural lymphatic obstruction, infection control and meticulous soft-tissue protocols are essential, and long-term surveillance remains necessary.

Utility of Diagnostic Tests Before Reimplantation in Patients Undergoing 2-Stage Revision Total Joint Arthroplasty: A Systematic Review and Meta-analysis.

INTRODUCTION: Periprosthetic joint infection (PJI) is a devastating complication after total joint arthroplasty (TJA), with treatment failure occurring in 12% to 28% after 2-stage revision. It is vital to identify diagnostic tools indicative of persistent infection or treatment failure after 2-stage revision for PJI. METHODS: The Cochrane Library, PubMed (MEDLINE), and EMBASE were searched for randomized controlled trials and comparative observational studies published before October 3, 2021, which evaluated the utility of serum/plasma biomarkers (erythrocyte sedimentation rate [ESR], C-react

Urine versus stent cultures and clinical UTIs.

PURPOSE: Current American Urological Association guidelines recommend pre-operative antimicrobial therapy based on prior urine cultures (UC); however, the role of stent culture (SC) in urologic practice is unclear. We examined whether UC and SC differed at the time of stent removal, as well as the association, microbiology, and timing of subsequent UTIs as related to SC. METHODS: This was a retrospective review of 159 patients with ureteral stents for indications of urolithiasis, benign stricture, malignant obstruction, and kidney transplant. UC and SC were analyzed at the time of stent remov

Novel strategies to diagnose prosthetic or native bone and joint infections.

INTRODUCTION: Bone and Joint Infections (BJI) are medically important, costly and occur in native and prosthetic joints. Arthroplasties will increase significantly in absolute numbers over time as well as the incidence of Prosthetic Joint Infections (PJI). Diagnosis of BJI and PJI is sub-optimal. The available diagnostic tests have variable effectiveness, are often below standard in sensitivity and/or specificity, and carry significant contamination risks during the collection of clinical samples. Improvement of diagnostics is urgently needed. AREAS COVERED: We provide a narrative review on c

Inoculation of sonicate fluid into blood culture bottles improves microbial detection in patients with orthopedic bone and soft tissue infections of the upper and lower extremities.

BACKGROUND: The usefulness of sonicate fluid culture for biological tissues in orthopedic bone and soft tissue infections have not been reported. We assessed whether inoculating the sonicate fluid of bone and soft tissue into a blood culture bottle could increase the diagnostic accuracy for biofilm-related orthopedic infections. METHODS: Twenty consecutive patients with infections (9 purulent arthritis, 4 osteomyelitis, 4 purulent tenosynovitis, 2 purulent bursitis, and 1 cellulitis) and 10 non-infected cases (6 carpal tunnel syndrome, 3 Dupuytren contracture, and 1 soft tissue tumor) between

Biofilm-host immunometabolic crosstalk in Staphylococcus aureus orthopedic implant infection: mechanisms and translational constraints.

Orthopedic implant biofilms are not only antibiotic-tolerant bacterial communities but metabolically active niches that reshape host immunity. Compared with planktonic infection, mature biofilms impose spatial and metabolic constraints that can uncouple leukocyte recruitment from sterilizing activity. Neutrophils can penetrate and phagocytose portions of Staphylococcus aureus biofilms, but clearance becomes incomplete as biofilms mature and may coexist with degranulation and neutrophil extracellular trap-associated tissue injury. Macrophages show impaired access and bactericidal programming, a

Antimicrobial-loaded PMMA bone cement: time for good practice recommendations.

Antimicrobial-loaded PMMA bone cement has been used since the 1970s as both a fixation material and a local antimicrobial reservoir. Its clinical roles differ between prophylaxis in primary arthroplasty and treatment of established infection, particularly during revision surgery and through the use of temporary spacers. Commercially manufactured products offer more predictable handling, but bespoke admixing may be considered when resistant or unusual organisms are encountered. This Viewpoint argues that such practice requires clearer governance. Cement formulation, antimicrobial choice, dose a

Fighting fire with fire: using infectious agents to treat persistent infection.

Infectious diseases lead to significant morbidity and mortality. Often, resolution of the acute stage of the disease leads to microbial persistence, resulting in chronic debilitating disease. Management of persistent infections frequently requires lifelong therapy with antimicrobial agents. These infections could be chronic viral infections like HIV, hepatitis B or chronic bacterial persistent infections like prosthetic joint infections caused by multi-drug resistant organisms. Bacteriophages have been designed specifically to target recalcitrant bacterial infections, such as prosthetic joint

Effect of urinary tract infection on the risk of prosthetic joint infection: A systematic review and meta-analysis.

PURPOSE: Urinary tract infection (UTI) are very common in the general population, however it is unclear whether UTI is a risk factor of prosthetic joint infection (PJI). Our purposes were: (1) To determine whether UTI is a risk factor of PJI after joint replacement, and (2) to determine whether the microorganisms causing PJI and UTI are the same. METHODS: PubMed, Web of Science, the Cochrane Library, and EMBASE were searched systematically for studies. The effect sizes of RR were calculated for included studies that reported raw counts with 95% CIs. The aim 1 of the study is a meta-analysis;

Clostridium septicum-infected aortic aneurysm or graft is a deadly diagnosis.

BACKGROUND: Clostridium septicum is an anaerobic, motile, spore-forming, toxin-producing gram-positive bacillus that can lead to rapidly progressive gas gangrene due to the release of alpha toxin. Aortic aneurysm secondary to C. septicum infection is a rare condition with 60 cases reported in the literature; however, we have recently treated several patients with the condition in our large tertiary care and aortic center. METHODS: Blood and tissue culture results collected between January 2005 and January 2018 and maintained in the microbiology laboratory database at the Cleveland Clinic were

Novel Insights into Staphylococcus aureus Deep Bone Infections: the Involvement of Osteocytes.

Periprosthetic joint infection (PJI) is a potentially devastating complication of orthopedic joint replacement surgery. PJI with associated osteomyelitis is particularly problematic and difficult to cure. Whether viable osteocytes, the predominant cell type in mineralized bone tissue, have a role in these infections is not clear, although their involvement might contribute to the difficulty in detecting and clearing PJI. Here, using Staphylococcus aureus, the most common pathogen in PJI, we demonstrate intracellular infection of human-osteocyte-like cells in vitro and S. aureus adaptation by f

Algorithm for the management of antibiotic prophylaxis in onychocryptosis surgery.

BACKGROUND: Antibiotic prophylaxis in nail surgery is not clearly established, and there is scant scientific evidence regarding the need for its use in preventing surgical site infection, hematogenous total joint infection, and infective endocarditis. OBJECTIVES: To propose an algorithm based on the evidence for the management of antibiotic prophylaxis in onychocryptosis surgery. METHODS: A literature review was performed in Medline, Pubmed, Cochrane database and Scopus and recent prospective studies were examined.
