Associations among subjective cognitive complaints, perceived social support, and medication adherence in patients with psychotic disorders.

BACKGROUND: Medication adherence is essential for the management of psychotic disorders, yet it remains difficult for many patients to sustain over time. In this study, the focus is on subjective cognitive complaints, defined as patients' self-perceived everyday cognitive difficulties, rather than cognitive biases or objectively measured cognitive deficits. These complaints may be associated with difficulties in remembering medication schedules, organizing treatment routines, and maintaining consistent follow-up. Perceived social support may also be relevant, as supportive family members, frie…

Comorbidity of Scurvy and Psychotic-Spectrum Disorders: Psychiatric Pathways to Vitamin C Deficiency, A Systematic Review.

BACKGROUND: Scurvy is a nutritional deficiency disorder caused by sustained vitamin C depletion and is historically associated with dietary deprivation. OBJECTIVE: To review published cases of scurvy co-occurring with psychotic-spectrum disorders, characterize the recurrent illness-related contexts linked to low ascorbate intake, and summarize the clinical features relevant to recognition in consultation-liaison settings. METHODS: We searched four databases for reports of scurvy with a psychotic-spectrum diagnosis, from inception to May, 2025. RESULTS: Eighteen reports describing nineteen p

Psychological Interventions Targeting Auditory Hallucinations in Persons With Psychotic Disorders: A Systematic Review and Meta-Analysis.

IMPORTANCE: Auditory hallucinations (AHs) are a hallmark characteristic of schizophrenia spectrum disorders (SSDs) and are often associated with severe distress. Previous research lacks comprehensive evidence on the overall efficacy of approaches targeting AHs in SSDs. OBJECTIVE: To evaluate the efficacy and acceptability of all available psychological and psychosocial interventions targeting AHs in SSDs. DATA SOURCES: Embase, MEDLINE, PsycArticles, PsycInfo, PSYNDEX, and the Cochrane library were searched for eligible studies published until August 18, 2025. STUDY SELECTION: Randomized cli

Decoding psychotic and other psychiatric disorders through placental organ: From cytoarchitecture to stress-responsive signaling networks.

Psychosis-defined as a profound disconnection between objective external reality and internal mental representations-represents a critical mental health condition, especially when it emerges for the first time during pregnancy. First-episode psychosis (FEP) in pregnant women (FEP-PW), though rare, presents severe risks for both maternal and fetal health. Despite its clinical significance, the biological mechanisms underlying FEP-PW remain poorly understood. This narrative review investigates the human placenta as a valuable and dynamic organ for decoding psychiatric and psychological disorders

From trauma to treatment: A scoping review of childhood bullying and its role in psychotic disorders.

INTRODUCTION: Childhood adversities are well-established risk factors for the later development of severe mental illness, and experiences of bullying are frequently reported among individuals with psychotic disorders. Bullying may influence symptom presentation, illness trajectory, and treatment responsiveness. It has been hypothesized that patients with psychosis and trauma could benefit from psychotherapeutic interventions. This scoping review aims to synthesize existing literature on the association between childhood bullying and psychotic disorders, including implications for treatment. M…

Inflammation-related pathology in the olfactory epithelium: its impact on the olfactory system in psychotic disorders.

Smell deficits and neurobiological changes in the olfactory bulb (OB) and olfactory epithelium (OE) have been observed in schizophrenia and … t in HCs. Together, our study implies a potential mechanism of the OE-OB pathology in patients with psychotic disorders (schizophrenia and related disorders). We hope that this mechanism may have a cross-disease implication, including COVID-19-elicited mental conditions that include smell deficits.

Polygenic risk for psychotic disorders in relation to cardiac autonomic dysfunction in unmedicated patients with schizophrenia.

Cardiac autonomic dysfunction (CADF), mainly characterized by increased heart rate, decreased heart rate variability, and loss of vagal modulation, has been extensively described in patients with schizophrenia (SCZ) and their healthy first-degree relatives. As such, it represents an apparent physiological link that contributes to the increased cardiovascular mortality in these patients. Common genetic variation is a putative underlying mechanism, along with lifestyle factors and antipsychotic medications. However, the extent to which CADF is associated with genetic factors for SCZ is unknown.

Associations of alcohol and tobacco use with psychotic, depressive and developmental disorders revealed via multimodal neuroimaging.

People affected by psychotic, depressive and developmental disorders are at a higher risk for alcohol and tobacco use. However, the further associations between alcohol/tobacco use and symptoms/cognition in these disorders remain unexplored. We identified multimodal brain networks involving alcohol use (n = 707) and tobacco use (n = 281) via supervised multimodal fusion and evaluated if these networks affected symptoms and cognition in people with psychotic (schizophrenia/schizoaffective disorder/bipolar, n = 178/134/143), depressive (major depressive disorder, n = 260) and developmental (auti

Antipsychotic Medication Continuation vs Taper and Discontinuation in Patients With Schizophrenia and Other Nonaffective Psychotic Disorders.

Schizophrenia is a major mental illness that is managed with long-term antipsychotic medication as a standard of care. Antipsychotic medications, however, are associated with many subjective and objective adverse effects. These adverse effects have driven the study of risk-mitigation strategies such as targeted intermittent therapy and dose reduction and drug discontinuation. Randomized controlled trials (RCTs) of these strategies have been synthesized in meta-analysis; both strategies have been associated with no functional benefits and with an increased risk of relapse. The RCTs, however, ha

Common genetic and environmental risk for personality disorders and psychotic-like experiences in young adult twins.

INTRODUCTION: Psychotic-like experiences (PLE) have been associated with the subsequent emergence of psychotic disorders as well as several other domains of psychopathology. In this twin study, we estimated the genetic and environmental correlations between PLE and 10 personality disorders (PD). METHODS: Diagnoses of 10 PDs according to the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) and PLE from the Composite International Diagnostic Interview (CIDI) were retrieved for 2793 young adult twins from the Norwegian Twin Registry. Risk for having a PD and PLEs was modeled using …

Chronic Psychotic Disorders and Correctional Involvement: A Population-Based Matched Case-Control Study in Ontario, Canada.

OBJECTIVE: Individuals with chronic psychotic disorders are overrepresented in correctional facilities, but little is known about factors that increase the risk of correctional involvement. The objective of this study was to compare individuals with chronic psychotic disorders who were released from correctional facilities in Ontario to individuals with chronic psychotic disorders but no correctional involvement on sociodemographic, clinical, and prior mental health-related health service utilization characteristics. METHOD: All individuals with chronic psychotic disorders who were released f…

Clinical phenotypes of five patients with psychotic disorders carrying rare schizophrenia-associated loss-of-function variants.

The Schizophrenia Exome Meta-Analysis (SCHEMA) consortium identified 10 genes in which loss-of-function (LoF) variants are highly associated with schizophrenia (SZ). In a well-characterized sample of 988 patients with psychotic disorders, we investigated whether patients bearing a SCHEMA variant presented with unusual or unique signs, symptoms, or course of illness. We identified 5 patients who carried a LoF variant in a SCHEMA gene, each in a different gene. None of the patients with a SCHEMA variant had unique symptoms. However, compared to the average of patients in the sample, all of the p…

Seasonal Effects on Hospitalizations Due to Mood and Psychotic Disorders: A Nationwide 31-Year Register Study.

PURPOSE: To examine seasonal patterns of hospital admissions due to mood and psychotic disorders and to investigate whether the admission rates show variation according to the seasonal daylength (photoperiods). PATIENTS AND METHODS: A retrospective nationwide register-based cohort of all psychiatric admissions (N=978,079) during 1987-2017 in Finland was utilized. The smoothed time-series of adjusted ratio of observed and expected (O/E) daily counts were estimated to examine seasonal variation. The mean O/E with 95% confidence intervals (CI) was used to study the admission rates by photoperiod…

Clinical characterization of brief psychotic disorders triggered by the COVID-19 pandemic: a multicenter observational study.

This study aimed to characterize the clinical profile of patients with brief psychotic disorders (BPD) triggered by the psychosocial distress derived from the COVID-19 crisis. A multicenter study was conducted from March 14 to May 14, 2020 (the peak weeks of the pandemic in Europe). All consecutive patients presenting non-affective psychotic episodes with a duration of untreated psychosis of less than 1 month and whose onset was related to the COVID-19 crisis were recruited, but only those patients meeting Diagnostic Statistical Manual 5th edition (DSM-5) criteria for "BPD with marked stressor…

Alterations in intrinsic fronto-thalamo-parietal connectivity are associated with cognitive control deficits in psychotic disorders.

Despite a growing number of reports about alterations in intrinsic/resting brain activity observed in patients with psychotic disorders, their relevance to well-established cognitive control deficits in this patient group is not well understood. Totally 88 clinically stabilized patients with a psychotic disorder and 50 healthy controls participated in a resting-state magnetic resonance imaging study (rs-MRI) and performed an antisaccade task in the laboratory to assess voluntary inhibitory control ability. Deficits on this task are a well-established biomarker across psychotic disorders as we …

Risk of dementia associated with psychotic disorders in later life: the health in men study (HIMS).

BACKGROUND: Recent research has identified several potentially modifiable risk factors for dementia, including mental disorders. Psychotic disorders, such as schizophrenia and delusional disorder, have also been associated with increased risk of cognitive impairment and dementia, but currently available data difficult to generalise because of bias and confounding. We designed the present study to investigate if the presence of a psychotic disorder increased the risk of incident dementia in later life. METHODS: Prospective cohort study of a community-representative sample of 37 770 men aged 65…
