Postpartum complications can be broadly divided into 4 categories: postpartum hemorrhage, obstetrical trauma, thromboembolic complications, and puerperal infections. Postpartum hemorrhage is most commonly caused by uterine atony, abnormal placentation, or genital tract trauma. Secondary causes of hemorrhage include retained products of conception and, rarely, subinvolution of the placental implantation site. Uterine dehiscence or rupture may be occult on ultrasound examination and may be better visualized on sagittal computed tomography or magnetic resonance imaging. Obstetric trauma during pr

Cauda equina syndrome is rarely associated with pregnancy, with few cases reported in the literature. The majority of cases describe antenatal presentations, with only one case manifesting post-partum, three weeks after a normal vaginal delivery. We outline a case of cauda equina syndrome following caesarean section in a patient with known lumbar disc disease, and discuss the contribution of the mode of delivery and anaesthesia as precipitants of disc herniation causing cauda equina compression. We conclude that vigilance, particularly in the post-partum setting, be attributed towards the neur

The Brugada syndrome is an important cause of sudden cardiac death primarily among males without structural heart disease. Although not widely known, it may be unmasked by an acute febrile illness. An association between mastitis and Brugada syndrome has not been previously reported. We describe an 18-year-old postpartum female who developed mastitis, fever, and syncope. Electrocardiography revealed type 1 Brugada pattern, which disappeared once her fever resolved. Although the role of pregnancy in this syndrome is unknown, clinicians should be aware that fever may precipitate Brugada syndrome

The authors present an unusual case of a true left-sided posterolateral (Bochdalek) hernia containing stomach, transverse colon and spleen within a hernial sac in a 25-year-old female patient. They presume this voluminous herniation took place due to a rupture of congenitally preformed hernial ostium on account of increased intraabdominal pressure during repeated pregnancy. They describe an effective surgical transabdominal laparoscopic treatment.

A 37-year-old woman who was six months postpartum presented with a painless lump in her left breast for three months. Mammograms showed a mixed fat and soft density mass on the craniocaudal and mediolateral views. A fat-fluid level was seen within the mass, typical of a galactocoele. Needle aspiration yielded milky fluid. The differential diagnosis of breast masses presenting in women with pregnancy or lactation is briefly discussed. The other causes of fat containing breast masses, such as oil cyst, hamartoma, lipoma and intramammary lymph nodes, are illustrated with additional examples.

We present two cases of late postpartum eclampsia. Both patients presented with a chief complaint of headache, and were diagnosed with eclampsia after the onset of seizures. Neither patient had proteinuria or edema. Further evaluation did not yield another diagnosis for the seizures, and treatment with i.v. magnesium sulfate was successful in stopping the seizures. No further seizure activity occurred in either patient.

BACKGROUND: Placenta accreta is the abnormal attachment of the placenta to the uterus. It occurs when the decidua basalis is either absent or incomplete. It is uncommon and most often presents with a retained placenta and hemorrhage.
CASE: A 29-year-old gravida 1 had an uncomplicated antenatal course and delivery. The third stage of labor was complicated by a retained placenta necessitating manual removal. Her postpartum course was complicated by a persistent endometritis. The diagnosis of placenta accreta was made with the help of sonohysterography.
CONCLUSION: Sonohysterography is a useful

PURPOSE: To underline the crucial role of urgent echocardiography in the differential diagnosis of acute respiratory and/or circulatory failure in the postpartum period.
CLINICAL FEATURES: A 24-yr-old woman was admitted to the intensive care unit (ICU) with a preliminary diagnosis of pulmonary embolism (PE) one week after Cesarean section. Neither computerized tomography nor Doppler sonography showed any signs of deep venous thrombosis or PE. In the ICU she required intubation and ventilatory support for acute respiratory and circulatory failure. Bedside echocardiography revealed features of

A 23-year old female patient diagnosed as HELLP syndrome (H-hemolysis, EL-elevated liver enzymes, LP-low platelets) was admitted during her 34th week of first pregnancy. After caesarean section she received hypotensive and anticoagulant treatment and blood transfusion. Her general condition was improved. The ocular manifestations (i.e. blurred vision, retinal oedema nad hemorrhages) were seen on the 10th postpartum day in both eyes. The ocular changes disappeared 4 month later.

Secondary postpartum haemorrhage after Caesarean section due to non-union of the uterine incision is an unusual complication. Two women who had had lower segment Caesarean section for placenta praevia presented after 2 and 3 weeks respectively with shock due to massive postpartum haemorrhage. The uterine scars had not healed in both of these women who were managed by emergency subtotal hysterectomy.

Severe pre-eclampsia, associated with HELLP syndrome, can occur after a normal delivery and birth in a woman whose blood pressure has remained normal throughout the antenatal period. Although rare, the syndrome can lead to pulmonary oedema or renal failure, and should be borne in mind when a woman develops epigastric or right upper-quadrant pain after a normal pregnancy and delivery. A significant fall in the platelet count in the antenatal period may be a useful indicator of risk.

Transient blindness of cortical origin associated with gestational (pregnancy induced) hypertension is a terrifying and unusual experience in obstetric practice. Two cases encountered in our centre at the University of Ilorin Teaching Hospital (UITH) Ilorin, Nigeria are presented. One case occurred in pregnancy and the other in early puerperium. The two patients were Nigerians. The possible aetiology and pathophysiology of this very rare condition are discussed.
