Clinical outcomes of radial probe endobronchial ultrasound using a guide sheath for diagnosis of peripheral lung lesions in patients with pulmonary emphysema.

BACKGROUND: Generally, structural destruction of lung parenchyma, such as pulmonary emphysema, is considered to be related to the low diagnostic yields and high complication rates of lung biopsies of peripheral lung lesions. Currently, little is known about the clinical outcomes of using endobronchial ultrasound with a guide sheath (EBUS-GS) to diagnose peripheral lesions in patients with emphysema. METHODS: This retrospective study was performed to identify the clinical outcomes of EBUS-GS in patients with pulmonary emphysema. This study included 393 consecutive patients who received EBUS-GS…

Positional treatment without mechanical ventilation in a very preterm infant with unilateral pulmonary interstitial emphysema: case report and review of the literature.

BACKGROUND: Pulmonary interstitial emphysema (PIE) in very low birth weight infants is a rare but severe complication. Although most of these air leaks develop in mechanically ventilated infants, they have also been reported in infants exposed only to nasal continuous positive airway pressure (CPAP). The optimal treatment for PIE is still under discussion and includes different approaches such as unilateral intubation, high frequency oscillation ventilation and even surgical lobectomy. However, as yet, there has been no report on complete resolution of unilateral PIE by positioning therapy wit

Ventilation/perfusion SPECT in chronic obstructive pulmonary disease: an evaluation by reference to symptoms, spirometric lung function and emphysema, as assessed with HRCT.

PURPOSE: Chronic obstructive pulmonary disease (COPD) is characterized by airflow limitation which is not fully reversible. Despite the heterogeneity of COPD, its diagnosis and staging is currently based solely on forced expiratory volume in 1 s (FEV(1)). FEV(1) does not explain the underlying pathophysiology of airflow limitation. The relationship between FEV(1), symptoms and emphysema extent is weak. Better diagnostic tools are needed to define COPD. Tomographic lung scintigraphy [ventilation/perfusion single photon emission tomography (V/P SPECT)] visualizes regional V and P. In COPD, relat

The relationships of serum prealbumin levels with parameters that indicate severity of disease and emphysema pattern in patients with stable chronic obstructive pulmonary disease.

OBJECTIVE: Malnutrition, which is a complication frequently observed in chronic obstructive pulmonary disease (COPD) and negatively affects prognosis, has become a parameter that must be monitored. Even though various methods are applied to assess malnutrition, biochemical parameters, especially serum prealbumin levels, are useful. MATERIALS AND METHODS: The relationships between serum prealbumin levels, which we used as an indicator of malnutrition, with the severity of disease and the parameters predicting emphysema in stable COPD patients with no additional health problems were determined

[Role of emphysema in the etiology of functional impairment in patients with severe chronic obstructive pulmonary disease. Study with high resolution computerized tomography].

PURPOSE: To investigate whether high-resolution CT (HRCT) can detect the subjects with massive emphysematous destruction in a group of patients with severe chronic obstructive pulmonary disease (COPD) and therefore be of help in selecting the candidates to surgical lung volume reduction. MATERIAL AND METHODS: We examined 40 former smokers with severe COPD (FEV1, < or = 40% of the predicted value, with no major improvement after inhalation of bronchodilators). Clinico-functional assessment included: a flow/volume loop (mean FEV1 = 28.6% of predicted), arterial blood gas analysis at rest breath

Successful Concurrent Chemoradiotherapy for Limited-Stage Small Cell Lung Cancer in a Patient with Combined Pulmonary Fibrosis and Emphysema: A Case Report.

Interstitial lung disease (ILD) is a major risk factor for severe radiation pneumonitis (RP) during thoracic radiotherapy. We herein report a case of limited-stage small cell lung cancer with coexisting pulmonary fibrosis and emphysema treated with concurrent chemoradiotherapy. The patient exhibited fibrotic lesions consistent with a usual interstitial pneumonia pattern and a mild 18F-fluorodeoxyglucose uptake, indicating an elevated risk of RP. However, high-grade RP did not develop, likely because radiation exposure to nontarget lungs was limited. The irradiation field was spatially separate

Spontaneous Pneumomediastinum, Pneumothorax, Pneumopericardium and Subcutaneous Emphysema-Not So Uncommon Complications in Patients with COVID-19 Pulmonary Infection-A Series of Cases.

(1) Background: Spontaneous pneumomediastinum (PM), pneumothorax (PT), and pneumopericardium (PP) were recently reported as rare complications in patients with severe COVID-19 pneumonia, and our study aims to follow the evolution of these involvements in 11 cases. The presumed pathophysiological mechanism is air leak due to extensive diffuse alveolar damage followed by alveolar rupture. (2) Methods: We followed the occurrence of PM, PN, PP, and subcutaneous emphysema (SE) in 1648 patients hospitalized during the second outbreak of COVID-19 (October 2020-January 2021) in the main hospital of in

Nebulized Potassium-Conjugated Linoleic Acid (K-CLA) Therapy in a Patient With Chronic Obstructive Pulmonary Disease and Centrilobular Emphysema: A Case Report.

Chronic obstructive pulmonary disease (COPD) is a progressive respiratory disorder characterized by persistent airflow limitation that commonly causes dyspnea and reduced exercise capacity. Emphysema, a major component of COPD, involves destruction of alveolar tissue and loss of elastic recoil, resulting in impaired gas exchange and air trapping. We report a 60-year-old female with COPD and centrilobular emphysema who underwent nebulized potassium-conjugated linoleic acid (K-CLA) therapy for persistent dyspnea and exercise intolerance despite standard inhaled therapy. Following two months of t

Immune checkpoint inhibitor-induced interstitial lung disease in non-small cell lung cancer patients with preexisting pulmonary emphysema or interstitial lung abnormalities.

BACKGROUND: Immune checkpoint inhibitor-induced interstitial lung disease (ICI-ILD) is a serious adverse event. While interstitial lung abnormalities (ILA) are known risk factors, the role of preexisting pulmonary emphysema remains controversial. PATIENTS AND METHODS: We enrolled patients with advanced or recurrent NSCLC who received anti-PD-1 antibody at our institution between October 2020 and October 2022. Clinical characteristics, radiographic features, time of onset, and outcomes of ICI-ILD were compared between patients with preexisting emphysema and/or ILA and those without these condi…

Mycobacterium abscessus pulmonary disease presenting with spontaneous pneumomediastinum and subcutaneous emphysema in childhood acute lymphoblastic leukemia: a case report and literature review.

INTRODUCTION: Mycobacterium abscessus is a rapidly growing mycobacterium commonly identified in adults with underlying pulmonary diseases but is rarely observed in children. A better understanding of this pathogen in children is essential. CASE PRESENTATION: We report the case of a 49-month-old female child without previous underlying pulmonary diseases but with acute lymphoblastic leukemia (ALL). The patient was complicated with pneumonia during chemotherapy, which was primarily characterized by spontaneous pneumomediastinum and subcutaneous emphysema on chest computed tomography (CT). M. ab

Extensive Surgical Emphysema and Pneumomediastinum Following Chest Drain Insertion in a Patient With Bullous Chronic Obstructive Pulmonary Disease (COPD): A Case Report.

Subcutaneous (surgical) emphysema and pneumomediastinum are recognized complications of thoracic procedures. They are often self-limiting but may occasionally indicate serious underlying pathology or precipitate life-threatening deterioration, particularly in patients with chronic obstructive pulmonary disease (COPD) and bullous lung disease. We present the case of an 87-year-old male patient with known COPD who was admitted with worsening dyspnoea and a productive cough. Imaging revealed a large left-sided pneumothorax, and emergency needle decompression followed by intercostal chest drain in

Impact of radiomics features, pulmonary emphysema score and muscle mass on the rate of pneumothorax and chest tube insertion in CT-guided lung biopsies.

Iatrogenic pneumothorax is a relevant complication of computed tomography (CT)-guided percutaneous lung biopsy. The aim of the present study was to analyze the prognostic significance of texture analysis, emphysema score and muscle mass derived from CT-imaging to predict postinterventional pneumothorax after CT-guided lung biopsy. Consecutive patients undergoing CT-guided percutaneous lung biopsy between 2012 and 2021 were analyzed. Multivariate logistic regression analysis included clinical risk factors and CT-imaging features to detect associations with pneumothorax development. Overall, 479

Presence of lung cancer and high gender, age, and physiology score as predictors of acute exacerbation in combined pulmonary fibrosis and emphysema: A retrospective study.

Combined pulmonary fibrosis and emphysema (CPFE) patients visit hospitals frequently due to acute exacerbations (AEs); however, the predictors of CPFE AE have not been comprehensively described in literature. Thus, we investigated the predicting factors of AE in CPFE patients. We retrospectively reviewed medical records from the past 12 years at Korea University Guro Hospital. We selected CPFE patients by computed tomography findings. Rapid deterioration (RD) was defined as acute worsening of dyspnea requiring hospitalization and the presence of newly developed radiologic abnormalities. AE was

Cadmium-chloride-induced air-space enlargement with interstitial pulmonary fibrosis is not associated with destruction of lung elastin. Implications for the pathogenesis of human emphysema.

To determine whether lung elastin is lost during the evolution of cadmium-induced air-space enlargement with pulmonary fibrosis, the lung elastin of 5- to 7-day-old golden Syrian hamster pups was radiolabeled by giving [3H]valine. At maturity, a single intratracheal instillation of 0.5 ml of 0.025% CdCl2 solution was given. Lung mechanics, histologic examination, and biochemistry were studied 5, 10, 21, 42, 105, and 180 days after the cadmium treatment. The animals developed fibrosis and air-space enlargement with decreased lung volumes, compliance, and forced expiratory flow; their functional

Successful Combined Autologous Blood and Minocycline Pleurodesis for Intractable Bilateral Pneumothorax in an Elderly Patient With Combined Pulmonary Fibrosis and Emphysema: A Case Report.

Bilateral pneumothorax in elderly patients with combined pulmonary fibrosis and emphysema (CPFE) is rare and often precludes surgery. We report an 86-year-old man with CPFE and a persistent air leak unresponsive to tube drainage. Combined pleurodesis via the chest tube using 50 mL autologous blood and 100 mg minocycline diluted in 50 mL saline achieved cessation after two sessions 3 days apart. Two months later, contralateral pneumothorax occurred and was successfully treated using the same protocol. No procedure-related complications were observed; no recurrence was seen at 6-month follow-up.

Outcome and safety 90 days after combined airway valve treatment of the right upper and middle lobes in patients with severe pulmonary emphysema.

BACKGROUND: In COPD patients with severe right-sided emphysema, complete major and incomplete minor fissure, implantation of one-way valves in both the right upper (RUL) and middle lobes (ML) is a possible approach for endoscopic lung volume reduction. The aim of this retrospective analysis was to evaluate the response to therapy and the complication rate at 90 days (90d-FU) after combined RUL-ML valve implantation. METHODS: This retrospective, monocentric study included all patients from the Thoraxklinik Heidelberg who underwent RUL-ML valve treatment between 2012 and 2023 with available fol

A rare case of symptomatic congenital lobar emphysema in an adolescent female who underwent expectant management of a congenital pulmonary malformation: a case report.

BACKGROUND: Asymptomatic congenital pulmonary airway malformations (CPAMs) and congenital lobar emphysema (CLE) may safely be observed, though little is known about how many patients later require surgery. The management of these lesions remains controversial, as limited research exists on which patients later require surgery. Notably, there are few reported cases where patients become symptomatic and require definitive treatment beyond childhood. CASE DESCRIPTION: We present the rare case of a 17-year-old female who developed dyspnea on exertion associated with pre-syncope, following diagnos

Fatal Presumed Re-expansion Pulmonary Edema Following Suction-Assisted Drainage of a Complete Spontaneous Pneumothorax in a Patient With Advanced Emphysema: A Case Report.

Re-expansion pulmonary edema (RPE) is a rare but potentially life-threatening complication of pleural decompression. Although most reported cases are self-limiting, severe presentations may rapidly progress to respiratory failure and death. We report a fatal case of presumed unilateral RPE following drainage of a complete spontaneous pneumothorax in a patient with advanced emphysema. A 51-year-old man with chronic obstructive pulmonary disease presented to the emergency department with progressive dyspnea of approximately one week's duration. On admission, he was ambulatory, hemodynamically st
