Dysfunctional frontal activation of mismatch negativity in panic disorder: A magnetoencephalographic study.

BACKGROUND: Mismatch negativity (MMN) or its magnetic counterpart (MMNm) is a neurophysiological signal to reflect the automatic change-detection ability. However, MMN studies in patients with panic disorder (PD) showed contrasting results using electroencephalographic (EEG) recordings. The present study attempted to overcome the limitations of EEG methodology by means of a whole-head magnetoencephalography (MEG) combined with the depth-weighted minimum norm estimate method to conduct an in-depth investigation on the MMNm at the cortical level in patients with PD. METHODS: We recruited 22 hea

Sensory neuropathic symptoms in idiopathic Parkinson's disease: prevalence and impact on quality of life.

BACKGROUND: Neuropathic symptoms are commonly reported in Parkinson's disease (PD), but robust data on the epidemiology of such symptoms are lacking. The present study sought to investigate the prevalence and determinants of peripheral sensory neuropathic symptoms (PSNS) in idiopathic PD (IPD) and ascertain the effects of such symptoms on the patients' quality of life (QoL). METHODS: Patients with IPD and age-matched and gender-matched controls were screened for neuropathic symptoms using the Michigan Neuropathy Screening Instrument. The impact of neuropathic symptoms on QoL was investigated

Multiple Sclerosis Patients with Markedly Low Intrathecal Antibody Response in Sri Lanka.

Multiple sclerosis (MS) is a heterogeneous disease which is poorly studied in Asia, where the disease is known to be rare with significant differences in clinical and radiological presentations and intrathecal antibody response. Therefore the objective of this study was to determine clinical presentation, radiological and neurophysiological characteristics, and oligoclonal band status in Sri Lankan MS patients, following careful exclusion of patients with neuromyelitis optica spectrum disorders and other conditions mimicking multiple sclerosis. Sixty-nine MS patients were recruited to the stud

The Effect of Prosthesis Use on Hand Mental Rotation After Unilateral Upper-Limb Amputation.

Amputation of a limb induces changes in the so-called body schema, which might be influenced by the use of prosthetic devices. Changes in the body representation associated with prosthesis use could be investigated using a hand mental rotation task. However, direct neurophysiologic evidence for the effect of prosthesis use on hand mental rotation is still lacking. In this paper, we recruited two groups of unilateral upper-limb amputees, i.e., amputees using a prosthesis or with a history of prosthesis use (Pro group) and amputees without a prosthesis (non-Pro group), as well as a sample of mat

Neutral wrist splinting in carpal tunnel syndrome: a 3- and 6-months clinical and neurophysiologic follow-up evaluation of night-only splint therapy.

AIM: The aim of the study was to evaluate the long-term efficacy of night-only splint wear therapy for carpal tunnel syndrome (CTS). METHODS: We conducted a randomized case-control trial with evaluation after three and six months of follow-up of outpatients with mild, recent onset symptoms of CTS recruited from the department clinic. Fifty patients (50 hands) were enrolled, of which 36 completed the study at 6 months. The case group utilized a thermoplastic neutral wrist splint for night-only wear. Outcome measures were instrumental parameters (sensory and motor nerve conduction velocity), sy

[Myoclonus in the adult: diagnostic approach].

Myoclonus, defined as shock-like involuntary movement, may be physiological or caused by a very wide variety of hereditary and acquired conditions. Because myoclonus can originate from different disorders and lesions affecting quite varied levels of the central and peripheral nervous systems, it represents from many points of view a diagnostic challenge. Moreover, new entities have been recently individualized, such as cortical tremor, which deserve renewed attention. The aim of this review is to propose a rationale for a diagnostic approach based on clinical and electrophysiological grounds.

Correlation of bilateral M1 hand area excitability and overall functional recovery after spinal cord injury: protocol for a prospective cohort study.

BACKGROUND: After spinal cord injury (SCI), a large number of survivors suffer from severe motor dysfunction (MD). Although the injury site is in the spinal cord, excitability significantly decreases in the primary motor cortex (M1), especially in the lower extremity (LE) area. Unfortunately, M1 LE area-targeted repetitive transcranial magnetic stimulation (rTMS) has not achieved significant motor improvement in individuals with SCI. A recent study reported that the M1 hand area in individuals with SCl contains a compositional code (the movement-coding component of neural activity) that links

Dysfunction of the Human Mirror Neuron System in Ideomotor Apraxia: Evidence from Mu Suppression.

Stroke patients with ideomotor apraxia (IMA) have difficulties controlling voluntary motor actions, as clearly seen when asked to imitate simple gestures performed by the examiner. Despite extensive research, the neurophysiological mechanisms underlying failure to imitate gestures in IMA remain controversial. The aim of the current study was to explore the relationship between imitation failure in IMA and mirror neuron system (MNS) functioning. Mirror neurons were found to play a crucial role in movement imitation and in imitation-based motor learning. Their recruitment during movement observa

Fostering Infant Responsivity and Reciprocity - Support to Thrive (FIRRST): a multisite randomized control trial of a pre-emptive telehealth intervention for infants with early signs of Autism Spectrum Disorders in Italy.

BACKGROUND: Early prodromal signs of Autism Spectrum Disorder (ASD) can be detected within the first year of life, a period of heightened neuroplasticity. Pre-emptive, parent-mediated interventions delivered during this window may improve developmental outcomes and reduce ASD symptom severity, but evidence from adequately powered randomized trials in Europe remains limited. Protocol for a pragmatic randomized controlled trial evaluating a care pathway incorporating ultrasound-guided platelet-rich plasma injection plus physiotherapy versus physiotherapy alone in patients with L5/S1 lumbar disc herniation.

BACKGROUND: Lumbar disc herniation (LDH) causes low back pain and lower-limb neurological symptoms. Conservative and surgical treatments have similar outcomes and recurrence rates within two years. Anti-inflammatory drugs or opioids reduce inflammation but are unsuitable for long-term use. Physiotherapy improves mobility and function, but has limited evidence for reversing disc herniation. Platelet-rich plasma (PRP) is emerging as a regenerative therapy for LDH. Combining ultrasound-guided PRP with McKenzie therapy may enhance disc repositioning, tissue repair, and sustained recovery. Integrative AI-based multiomic and neurophysiological profiling of chronic pain in rheumatoid arthritis: study protocol for the prospective, observational, case-control RA-PAIN-AI study.

INTRODUCTION: Rheumatoid arthritis (RA) is a chronic systemic autoimmune disease in which pain remains the most prominent and burdensome symptom from the patient's perspective. Despite effective control of peripheral inflammation with biological and targeted synthetic disease-modifying antirheumatic drugs, an estimated 20-30% of patients continue to experience persistent moderate-to-severe pain after achieving clinical remission. This therapeutic gap reflects the evolution of pain mechanisms from peripheral nociception towards central sensitisation and nociplastic pain, creating a need for int

Using cortical auditory-evoked potential responses to verify the objective benefit of aural rehabilitation in adult cochlear implant users.

INTRODUCTION: Cortical auditory-evoked potential (CAEP) responses are used clinically to assess auditory processing and support cochlear implant (CI) mapping procedures post-implantation. This study aims to examine CAEP responses as an objective measure of neurophysiological changes following a structured Aural Rehabilitation Program (ARP) in long-term adult CI users, with poor baseline speech perception performance. Advancing our understanding of the mechanisms underlying improved post-implantation outcomes in poor performing CI users may inform the development of targeted interventions and r

Nerve ultrasound, neuronopathy and cough predict sensory neuropathy patients with RFC1 expansions.

The finding of biallelic pathogenic pentanucleotide RFC1 expansions has extended the spectrum of disease in cerebellar ataxia, neuropathy and vestibular areflexia syndrome. It is clear that for many, a sensory neuropathy is an early feature and raises the question of how to identify which patients with this common neurophysiological presentation should be tested genetically for the condition. We identified patients with idiopathic, sensory predominant neuropathies who had attended the Neurophysiology Department of Auckland Hospital for nerve conduction studies. We undertook a systematic clinic

Improving localization and measurements of M-waves using high-density surface electromyography.

Surface electromyography (sEMG) is useful for studying muscle function and controlling prosthetics, but cross talk from nearby muscles often limits its effectiveness. High-density surface EMG (HD-sEMG) improves spatial resolution, allowing for the isolation of M-waves in the densely packed forearm muscles. This study assessed HD-sEMG for localizing M-waves and evaluated the impact of spatial filters on cross talk reduction. We administered peripheral nerve stimulation to activate forearm muscles in five participants. We analyzed cross talk by correlating the shape of M-waves between electrodes

Prospective multi-phase observational study evaluating local field potentials to guide deep brain stimulation programming in dystonia at a UK Tertiary Neurosciences Centre (LFP-DYT): a protocol.

INTRODUCTION: Deep brain stimulation (DBS) for dystonia is effective but programming optimisation can take months. Local field potentials (LFPs) recorded by the Medtronic Percept device may provide biomarkers to guide stimulation. This study will prospectively evaluate whether chronic LFP profiles correlate with clinical outcomes and can inform DBS programming strategies. METHODS AND ANALYSIS: LFP-DYT is a single-centre, multi-phase observational study at Newcastle upon Tyne National Health Service (NHS) Foundation Trust. An internal pilot (Cohort 1) will refine recording workflows, followed

Modeling Negative Symptom Domain Neurobiology: Protocol for an Observational, Transdiagnostic, Translational Study.

BACKGROUND: Negative symptoms (NS) such as anhedonia (reduced pleasure), avolition (reduced motivation), asociality (social withdrawal), blunted affect (diminished emotional expression), and alogia (poverty of speech) are associated with poor functional outcomes in psychiatric and neurological disorders and are an unmet treatment need. Current medication primarily targets positive or affective symptoms, leaving NS neurobiology unaddressed. A critical research gap exists in understanding whether these symptoms share a common biological architecture across different diagnoses or whether they eme

Neural mechanisms of uncertain decision-making in anxious adolescents: an EEG study based on the balloon analogue risk task.

BACKGROUND: Adolescence is marked by heightened vulnerability to anxiety which can impair decision-making under uncertainty. The underlying neural oscillatory network-level mechanisms in anxious adolescents require further elucidation. METHODS: We recruited 40 adolescents (20 adolescents with clinically diagnosed anxiety disorders, 20 matched controls). Participants performed a gain/loss version of the Balloon Analogue Risk Task (BART) during EEG recording. Behavioral data, time-frequency EEG power in beta (13-30 Hz) and theta (4-7 Hz) bands, and phase-based functional connectivity were analy

Higher disease burden and greater small fibre impairment in women with painful diabetic neuropathy.

OBJECTIVE: Although the incidence of Painful-Diabetic Peripheral Neuropathy (Painful-DPN) is higher in women, the underlying reasons for this are not known. This study investigated sex differences in pain and neurological phenotypes in 2 cohorts of patients with Painful-DPN. METHODS: Two cohort analyses were performed in this retrospective analysis of participants with Painful-DPN recruited to cross-sectional studies: (1) Patient-Reported Outcomes Measures Cohort analysis (n = 184), which assessed neuropathic pain intensity (Numeric Rating Score), neuropathic symptoms (Neuropathic Pain Scale)

Electromyographic features for quantitative assessment of lower limb spasticity after stroke: a feasibility study.

Assessment of lower limb spasticity is crucial for enhancing both functional recovery and rehabilitation effectiveness of stroke patients. Traditional methods like the Modified Ashworth Scale (MAS) are often hampered by subjective bias, leading to inconsistent assessments among clinicians. In contrast, neurophysiological methods, mainly surface Electromyography (sEMG) data analysis, offer a more objective and sensitive alternative. Most of the existing studies based on sEMG focus more on upper limb applications, while research on the assessment of lower limb spasms is still under exploration.
