AIM: We report the outcome of photodynamic therapy (PDT) in 4 patients with rosacea.

BACKGROUND: Dilatation of vascular vessels in rosacea has generally been attributed to yielding to deranged connective tissue. In contrast, in a previous study a degrading effect of insufficient vascular vessels in connective tissue has been demonstrated in connection with diabetic microangiopathy. We present an 84-year-old white male patient who, during a 2-year period, developed successively tumor infiltrations of an angiosarcoma in his face. The tumor started at the right lower eyelid, later appeared on the nose, the front, the cheeks and the earlobes. The first clinical impression was that of a rosacea. The tumors where successfully treated by soft X-rays, the total doses ranging from 4,800 to 5,600 cGy.

One percent metronidazole in an emollient cream base was compared with 250 mg oral tetracycline taken twice daily for the treatment of seventy-five patients with rosacea. After eight weeks of treatment there was no statistically significant difference between the results of the two treatments. Tetracycline did have a more rapid onset of effect on papules and pustules. Both treatments were well tolerated.

Flushing in rosacea has been investigated by means of (a) pharmacological inhibition of some possible chemical mediators and (b) titration of bradykinin as a possible effector directly in the blood. Clonidine-inhibited flushing was seen in all patients (mean 45%), other drugs had poorer results. Bradykinin increased in all patients at the climax of flushing (mean 60%). These findings support the hypothesis that epinephrine promotes a bradykinin release responsible for vasodilation.
