INTRODUCTION: dislocations of carpal bones without associated fractures are considered a rare injury, the most common mechanism of injury being axial loading with wrist in extension plus ulnar deviation. The literature reports a wide variety of complex carpal injuries, even so, it is possible to identify previously undescribed injuries.
OBJECTIVE: to present an atypical case of a patient with an injury to the midcarpal stabilizing mechanism and the stabilizing mechanism of the proximal row of the wrist following trauma to the hand that required carpectomy as definitive surgical treatment.

BACKGROUND: Percutaneous scaphoid screw fixation is a popular treatment for acute scaphoid fractures with no or minimal displacement. For treating scaphoid nonunions, however, open reduction and internal fixation with bone grafting is still the most popular treatment. Percutaneous fixation with bone grafting through the screw insertion hole has received little attention, although it minimizes damage to the surrounding tissues. We report excellent results of six scaphoid nonunions treated by retrograde percutaneous fixation with curettage and bone grafting through the distal insertion hole of a

Purpose We evaluated clinical and radiographic outcome of percutaneous transtrapezial fixation of the scaphoid delayed union or nonunion using a headless bone screw without bone grafting. Methods Sixteen patients with delayed union or nonunion of the scaphoid were included in this retrospective study between 2006 and 2011. All patients had a delayed presentation of scaphoid fracture, and none of them was treated conservatively elsewhere. Patients with bone graft, sclerotic bone debridement, or displacement of the fragment at the nonunion site were excluded. A percutaneous transtrapezial fixa

INTRODUCTION: Scaphoid fracture is the most common fracture among the bones of the wrist. Plain radiography has a sensitivity of 59 to 79 percent for the diagnosis of scaphoid fracture after injury. Casting is done to avoid complications in patients with suspected fractures with normal radiography. Magnetic resonance imaging (MRI) has a sensitivity of 100% and a specificity of 99%. To avoid unnecessary casting and high costs, we decided to offer a way more efficient than radiography and less costly than MRI.
MATERIAL AND METHODS: In the present study, 175 patients with suspected scaphoid frac

BACKGROUND AND AIM: Scaphoid nonunion (SN) is a challenging state after scaphoid fracture. The posterior interosseous nerve neurectomy (PINN) is often performed adjunctively with scaphoid reconstruction using autologous bone grafts. However, it remains unclear whether PINN has a prophylactic or therapeutic value, and thus it results superior to patients with SN without PINN.
METHODS: A retrospective study was performed to evaluate patients with SN who were treated with autologous bone grafts with (cohort 1) or without PINN (cohort 2) between 1996 and 2010. Clinical outcomes, Mayo-wrist score,

PURPOSE: Within a prospective, multicenter cohort study we investigated whether operative treatment of scaphoid bone fractures leads to earlier return to previous activity levels.
METHODS: Only isolated, acute, complete, stable and non-displaced fractures of the mid-third of the scaphoid bone were included. A total of 94 patients with the same number of fractures were recruited. In the operative group, fractures were fixed with a cannulated screw and had postoperative splint immobilization for a maximum of 1 week. In the conservative group a short arm cast was applied until fracture union was…

BACKGROUND: Although magnetic resonance imaging (MRI) is now considered the gold standard in second-line imaging of patients with suspected scaphoid fracture and negative radiographs, bone scintigraphy can be used in patients with pacemakers, metallic implants, or other contraindications to MRI. Bone scintigraphy is highly sensitive for the detection of fractures, but exact localization of scintigraphic lesions may be difficult and can negatively affect diagnostic accuracy.
PURPOSE: To investigate the influence of image fusion of planar bone scintigraphy and radiographs on image interpretatio

OBJECTIVES: The authors report their results on the treatment of scaphoid non-unions without osteoarthritis or necrosis, by anterior nonvascularized bone graft.
PATIENTS AND METHODS: Forty-seven patients treated between 1988 and 2003 were analysed with a mean follow-up of 74 months. There were 19 non-unions without carpal instability (IIA) and 28 with carpal instability (IIB). All patients were treated by an anterior approach with corticocancellous bone graft and osteosynthesis using two pins. Clinical results were analysed on pain, strength and mobility. Radiographic analysis was of union, c

BACKGROUND: Surgically, it is difficult to achieve union of a scaphoid nonunion that is associated with osteonecrosis of the proximal pole, and those with carpal collapse are especially difficult to treat. A variety of vascularized bone grafts can be used. The purpose of this study was to compare the effectiveness of two types of vascularized bone graft--a distal radial pedicle graft and a free vascularized medial femoral condyle graft--in the treatment of scaphoid waist nonunions associated with proximal pole osteonecrosis and carpal collapse.
METHODS: A retrospective review was conducted at

Pedicled vascularized bone grafts (Zaidemberg's technique) were used to treat 22 established scaphoid fracture nonunions, 16 of which were found to have avascular proximal poles at surgery. After a follow-up of 1-3 years, only six (27%) of the 22 fracture nonunions had united. Only two of the 16 nonunions with avascular proximal poles united, compared with four of the six nonunions with vascular proximal poles. We conclude that this technique of pedicled vascularized bone grafting may not improve the union rate for scaphoid fracture nonunions with avascular proximal pole fragments.

BACKGROUND: Malreduced or neglected unstable carpal scaphoid fractures may lead to nonunion, which is often difficult to treat and carries the risk of carpal instability and degenerative arthrosis. Here, we describe a new "sandwich" method, which takes advantage of minimal dissection and has promising results.
METHODS: From 1989 through 1992, we treated 39 patients with scaphoid nonunion by using the sandwich method that consisted of a wedge corticocancellous strut graft and numerous cancellous bone chips. Divergent Kirschner wires were used for fixation of reduction. We retrospectively revie

Degenerative arthritis of the wrist as a consequence of chronic scapho-lunate instability or chronic scaphoid non-union follows very specific patterns. Joint destruction occurs primarily in the radio-scaphoid and in the luno-capitate joints while the radio-lunar joint is preserved. Treatment by partial carpal fusion between capitate, scaphoid and lunate aims at reconstruction of correct length of the central column and restoration of normal carpal height by reducing the dorsiflexed lunate bone, thereby decompressing the painful radio-scaphoid joint. We have carried out the procedure in 21 case

INTRODUCTION: The exact mechanisms of bone remodelling after scaphoid fractures are not fully understood. Blood supply may lead to delayed consolidation and non-unions as challenging long-term problems. The aim of this study was to follow-up the microstructure during the scaphoid bone remodelling process using High Resolution peripheral Quantitative Computed Tomography (HR-pQCT) and compare the results with clinical and laboratory data.
PATIENTS AND METHODS: In this monocentric, prospective, controlled, clinical trial 39 patients with an unilateral conservatively treated scaphoid fracture at …

INTRODUCTION: Scaphoid nonunion after failed primary treatment remains challenging particularly when entailed by bone loss, avascular necrosis or deformity. We describe a scaphoid augmentation and fixation technique for cases of recalcitrant nonunion after screw placement by autologous press fit corticocancellous dowel. This study aims to provide reliable data on clinical and radiological outcomes and to contextualize in the face of other treatment options.
MATERIAL AND METHODS: The study included 16 patients with recalcitrant nonunion of the scaphoid. All patients received screw removal and

Eighteen patients who underwent revision non-vascularized bone grafting and internal fixation after failed surgery for scaphoid nonunion were reviewed after a minimum of 8.2 years. Eleven of the nonunions were located in the middle and seven in the proximal third of the scaphoid. The mean interval between injury and the revision procedure was 6 years. Sixteen of the 18 nonunions healed, two after a third attempt. Three patients with healed nonunions and one patient with persistent nonunion required salvage procedures for progressive radiocarpal arthrosis. In the remaining 14 cases, the mean lo
