Non-infectious uveitis and scleritis are significant causes of visual impairment, and currently available targeted and effective therapeutic options remain limited. Multiple immunologic pathways are currently under investigation and targeted therapeutics have the potential to change the treatment landscape and improve visual outcomes. This review synthesizes potential upcoming local and systemic treatments for non-infectious uveitis and scleritis with a focus on therapeutics with human data, noting those with the most promising evidence, and areas of gaps in research.

INTRODUCTION: Cogan's syndrome is a rare autoimmune vasculitis characterized by ocular inflammation and audiovestibular dysfunction, with the atypical form presenting with inflammatory ocular disease other than interstitial keratitis. Typical Cogan's syndrome manifests with interstitial keratitis and audiovestibular symptoms occurring within a 2-year interval, whereas atypical Cogan's syndrome involves inflammatory ocular disease without interstitial keratitis (such as scleritis, episcleritis, uveitis, or choroiditis), audiovestibular impairment, and systemic manifestations with delays longer …

This case report describes a 39-year-old male presenting with serous retinal detachment (SRD) who was initially misdiagnosed with acute central serous chorioretinopathy (CSC). CSC is typically a self-limiting condition, but recurrence occurs in approximately 30-50 % of patients depending on risk factors and disease chronicity. He received antibiotics for suspected infection, but his symptoms persisted without corticosteroid use. After referral to our hospital, posterior scleritis was confirmed through inflammation signs, multimodal imaging (showing optic disc edema, extending SRD, thickened ey…

OBJECTIVE: Describe the proportion of patients with wide-field fluorescein angiographic (WFFA) findings in patients with active anterior scleritis.
METHODS: An observational, descriptive, cross-sectional study of the WFFA findings of patients with active anterior scleritis including nodular, diffuse, or necrotizing involvement was performed. Studies were performed with the Heidelberg Spectralis module (102º). Images were saved and assessed by two masked co-authors.
RESULTS: Seventy-nine eyes from 39 patients, 31 (79%) females with a mean age of 50.5 years ± 13.98. Mean best-corrected visual …

OBJECTIVES: Scleritis represents a severe extra-articular manifestation of rheumatoid arthritis (RA). Recent clinical observations suggest a decreasing incidence of scleritis in RA, attributed to improved treatment options. Our study reports on the incidence and clinical characteristics of scleritis in RA observed in the biological era and reflects on our results in a historical perspective.
METHODS: We performed a retrospective evaluation of all 1623 consecutive patients with RA diagnosed at the department of rheumatology between 2011 and 2021 at the Erasmus Medical Center to investigate the…

We retrospectively reviewed literature reports on pathogens, clinical features, diagnosis, treatment, and clinical and visual outcomes in patients with bacterial scleritis over the past decade. Eye surgery and trauma are the most common causes of bacterial infections. Subtenon triamcinolone acetonide injection, intravitreal ranibizumab, and wearing contact lenses are also causes of bacterial scleritis. Pseudomonas aeruginosa is the most common pathogenic microorganism causing bacterial scleritis. Mycobacterium tuberculosis ranks second. The main symptoms of bacterial scleritis are red and pain…

KEY CLINICAL MESSAGE: Though COVID-19 vaccination saved many lives all around the world, it has had many adverse effects including ophthalmologic side effects. It is important to report such adverse effects to provide timely diagnosis and management.
ABSTRACT: Since the COVID-19 global outbreak, many types of vaccines have been introduced. These vaccines have been associated with some adverse effects including ocular manifestations. Herein, we report a case of a patient who developed nodular scleritis shortly after receiving the first and second doses of the inactivated COVID-19 vaccine, Sino…

Suprachoroidal triamcinolone acetonide (SCS-TA) injections were recently approved to treat macular edema secondary to noninfectious uveitis. However, its use in managing posterior scleritis has not been reported. We report the first case of SCS-TA used in the treatment of posterior scleritis. A 67-year-old woman with posterior scleritis complicated by exudative retinal detachment, diagnosed by spectral-domain optical coherence tomography scan (OCT), presented with pain, decreased vision, and redness in the left eye for 8 months. She was previously prescribed topical prednisolone and oral predn…

Surgically induced necrotizing scleritis (SINS) is an uncommon but devastating complication that may occur days to years after ocular surgery. We report the case of a 32-year-old man who underwent uncomplicated strabismus surgery for large-angle exotropia and developed SINS characterized by painless scleral inflammation, choroidal exposure, and globe ectasia within days of surgery. Work-up revealed no associated infectious process or underlying systemic inflammatory condition. Clinical resolution occurred with oral immunosuppression alone, without need for graft.

INTRODUCTION: Coronavirus disease (COVID-19) vaccines have been reported to have ocular side effects including scleritis and episcleritis.
PURPOSE: To report scleritis and episcleritis within a month following administration of COVID-19 vaccine.
METHODS: Retrospective case series.
RESULTS: 15 eyes of 12 consecutive patients with scleritis and episcleritis from March 2021 to September 2021 were included. The mean time of onset of symptoms in patients with scleritis was 15.7 days (range, 4-30) and for episcleritis it was 13.2 days (range 2-30). Patients received COVISHIELD™ (10 patients) and …

BACKGROUND: Chikungunya is a matter of grave concern in Brazil. This case report describes a rare ocular manifestation in a patient with chi … apering every three days, and nimesulide 100 mg every 12 hours for 7 days. One week later, however, scleritis was worse and the medial sclera was elevated, particularly in the right eye. Intraocular pressure remained normal, and hyperemia increased to 4+/4+ in the right eye. Supplementary tests revealed positive serology for chikungunya IgG and IgM antibodi …

A 65-year-old male presented with redness and pain associated with active necrotizing scleritis in the left eye. He was started on mycophenolate mofetil and oral corticosteroids, to which there was no response detected after 4 weeks. A rheumatology opinion was sought and he was started on tofacitinib, after which there was dramatic clinical improvement. Patients refractory to conventional immunosuppressive therapy can benefit from the new class of immunosuppressive agents, JAK/STAT kinase inhibitors.

Scleritis is an idiopathic condition that may sometimes be associated with systemic immunological like disorders rheumatoid arthritis, Wegener's granulomatosis and systemic lupus erythematosus. A variety of foreign material has been reported to result in granuloma formation in various parts of the body. We report a case of inflamed episcleral granulomas mimicking severe nodular anterior scleritis in a healthy Caucasian woman who underwent strabismus surgery in her childhood. Foreign body reaction on the episcleral/scleral surface is rare. It is extremely unusual for a non-absorbable suture tha…
