Anterior Canal benign paroxysmal positional Vertigo following surgical Management of superior canal dehiscence.

OBJECTIVE: Benign paroxysmal positional vertigo (BPPV) is a common post-surgical finding in patients managed for superior semicircular canal dehiscence (SSCD). The posterior semicircular canal has been reported as the involved canal in the majority of cases of post-surgical BPPV, with only two cases reported of lateral canal involvement. The objective of this report is to present a case in which an anterior semicircular canal BPPV response was identified in a patient following surgical management for SSCD. METHOD: This case report presents an adult with residual dizziness following surgical m

Canal dehiscence.

PURPOSE OF REVIEW: The aim is to review canal dehiscence involving the superior, lateral, and posterior semicircular canals. The main focus will be on superior semicircular canal dehiscence. RECENT FINDINGS: Canal dehiscence involving the superior, lateral, and posterior semicircular canal can have different etiologies, including developmental abnormality, congenital defect, chronic otitis media with cholesteatoma, and high-riding jugular bulb. However, their clinical presentation can be very similar, with patients complaining of vertigo, oscillopsia, and sometimes hearing loss. Canal dehisce…

Endolymphatic hydrops in perilymphatic fistula.

Cochlear hydrops was produced either by injecting artificial perilymph into the subarachnoid space or by sucking perilymph through the round window membrane. The animals were either vitally fixed immediately or kept alive for 1 to 3 months before fixation. Conventional celloidin embedding method was used for serial sectioning. Animals with cochlear hydrops were selected from among 56 guinea pigs; 13 animals were used for the present observation. The cochleas of these animals showed slight to moderate hydrops, while the vestibule and semicircular canals demonstrated either a normal or collapsed…

Simple methods for determining the affected side and performing otolith repositioning in cases of lateral semicircular canal-benign paroxysmal positional vertigo-canalolithiasis: Healthy-ear-down 135° maneuver and affected-ear-down 135° maneuver.

OBJECTIVE: To develop simple and effective methods for determining the affected side and promoting early symptom resolution in lateral semicircular canal-benign paroxysmal positional vertigo (L-BPPV) due to canalolithiasis, including its geotropic (L-BPPV-can-geo) and apogeotropic (L-BPPV-can-apo) variants. METHODS: A total of 146 cases of paroxysmal geotropic positioning nystagmus (PGPN, L-BPPV-can-geo) and 285 cases of persistent apogeotropic positional nystagmus (PAPN) due to peripheral vestibular disorders were analyzed. Positional and positioning nystagmus were observed and recorded usin

Heat exchange in cryosurgery of Meniere's disease. Experimental and clinical studies.

The course of the temperature in the lateral semicircular canal and the heat exchange were measured on temporal-bone specimens when cooling with a cryoprobe on the lateral semicircular canal was performed. The measurements showed a correlation between the heat exchange, the cooling time and the temperature in the lateral semicircular canal. In 15 patients with unilateral Meniere's disease, this was utlized to achieve a controlled temperature course during cryosurgery on the lateral semicircular canal. The effect of the treatment on the attacks of vertigo and on the hearing in these patients is

Chronic Symptoms After Vestibular Neuritis and the High-Velocity Vestibulo-Ocular Reflex.

HYPOTHESIS: As the anterior and posterior semicircular canals are vital to the regulation of gaze stability, particularly during locomotion or vehicular travel, we tested whether the high-velocity vestibulo-ocular reflex (VOR) of the three ipsilesional semicircular canals elicited by the modified Head Impulse Test would correlate with subjective dizziness or vertigo scores after vestibular neuritis (VN). BACKGROUND: Recovery after acute VN varies with around half reporting persistent symptoms long after the acute episode. However, an unanswered question is whether chronic symptoms are associa…

Inner ear blood flow in the rat after unilateral arterial occlusion in the vertebrobasilar arterial system.

It is generally accepted that certain kinds of vertigo and hearing disturbances are caused by blood flow insufficiency in the vertebrobasilar arterial system. Using the microsphere method we investigated whether unilateral vertebral artery or unilateral posterior inferior cerebellar artery occlusion could cause an imbalance between right and left inner ear blood flow in rats. We also studied the differential vulnerability between blood flow in the cochlea and in the ampullae of the three semicircular canals. We counted the numbers of microspheres distributed to the cochlea (CO) and microsphere…

Topography and reciprocal activity of cerebellar Purkinje cells in the uvula-nodulus modulated by vestibular stimulation.

In the rabbit uvula-nodulus, vestibular and optokinetic information is mapped onto parasagittal zones by climbing fibers. These zones are related functionally to different pairs of vertical semicircular canals, otolithic inputs and horizontal optokinetic inputs. Vestibular stimulation restricted to one of these zones modulates climbing fiber responses (CFRs). Within each of these zones, simple spikes (SSs) are modulated reciprocally with CFRs. In rabbits anesthetized with chloralose-urethan, we have used vestibular and optokinetic stimulation to evoke CFRs within a parasagittal zone while reco…

Extended middle fossa and retrolabyrinthine approaches in acoustic neuroma surgery: case reports.

Hearing conservation surgery for small acoustic neuromas is well accepted. At present, two approaches are primarily used: the suboccipital and the middle fossa. The middle fossa approach to the internal auditory canal has the advantage of using bony landmarks to identify and protect the facial nerve. Because of anatomic constraints presented by the superior semicircular canal however, its uses are limited to intracanalicular tumors or tumors protruding only slightly into the cerebellopontine angle. By extending the approach through the superior semicircular canal, a wide access to the cerebell

Benign Paroxysmal Positional Vertigo: Is It Really an Otolith Disease?

The current theory in physiopathology of benign paroxysmal positional vertigo is the mechanical theory, namely the cupulolithiasis-canalolit … smal positional vertigo physiopathology has been questioned. Studies suggest that the debris in the semicircular canals is caused by degeneration due to aging and may not be responsible for the symptoms in benign paroxysmal positional vertigo. Some patients with debris in semicircular canals do not have benign paroxysmal positional vertigo symptomatology, while some …

High-frequency vestibulo-ocular reflex (VOR) evaluation in patients with spontaneous downbeat nystagmus: a case-control study.

INTRODUCTION: Spontaneous downbeat nystagmus (DBN) is generally attributed to vestibulocerebellar dysfunction, although its exact pathophysiological mechanism remains unclear. The video head impulse test (vHIT) quantifies vestibulo-ocular reflex (VOR) gain in semicircular canals, allowing evaluation of VOR imbalance. OBJECTIVE: To elucidate the role of VOR in spontaneous DBN of central origin we measured gain across semicircular canals and gain asymmetry between coplanar canals, compared with matched healthy subjects. METHODS: Nested case-control study within a retrospective cohort. We inclu…

The Evidence for Selective Loss of Otolithic Function.

Recent advances in vestibular testing now permit functional testing of all peripheral vestibular sense organs (all three semicircular canals, utricle, and saccule). This makes it possible to identify patients with isolated dysfunction of the utricle or saccule, even though parallel pathways for vestibular information are ultimately integrated centrally. Selective, isolated unilateral loss of utricular function as measured by ocular vestibular-evoked myogenic potentials (VEMPs) has been observed in patients with normal semicircular canal function as measured by the video head impulse test of al…
