In vitro endothelial hyperpermeability occurs early following traumatic hemorrhagic shock.

BACKGROUND: Endothelial hyperpermeability is suggested to play a role in the development of microcirculatory perfusion disturbances and organ failure following hemorrhagic shock, but evidence is limited. OBJECTIVE: To study the effect of plasma from traumatic hemorrhagic shock patients on in vitro endothelial barrier function. METHODS: Plasma from traumatic hemorrhagic shock patients was obtained at the emergency department (ED), the intensive care unit (ICU), 24 h after ICU admission and from controls (n = 8). Sublingual microcirculatory perfusion was measured using incident dark field vide

Pharmacologic resuscitation for hemorrhagic shock combined with traumatic brain injury.

BACKGROUND: We have previously demonstrated that valproic acid (VPA), a histone deacetylase inhibitor, can improve survival after hemorrhagic shock (HS), protect neurons from hypoxia-induced apoptosis, and attenuate the inflammatory response. We have also shown that administration of 6% hetastarch (Hextend [Hex]) after traumatic brain injury (TBI) decreases brain swelling, without affecting size of the lesion. This study was performed to determine whether addition of VPA to Hex would decrease the lesion size in a clinically relevant large animal model of TBI + HS. METHODS: Yorkshire swine (42

[Acute traumatic haemorrhagic shock and transfusion: what's new in 2009?].

In spite of continuous progress in surgery and in interventional radiology, massive haemorrhage remains a leading cause of death in traumatology. The transfusional strategy appears a key step in the treatment of haemorrhagic shock. In the light of new insights into the pathophysiology of coagulopathies associated with traumatic shock it seems reasonable to transfuse patients with haemorrhagic shock earlier than previously recommended.

Shell shock and mild traumatic brain injury: a historical review.

Mild traumatic brain injury is now claimed to be the signature injury of the Iraq and Afghanistan conflicts. During World War I, shell shock came to occupy a similar position of prominence, and postconcussional syndrome assumed some importance in World War II. In this article, the nature of shell shock, its clinical presentation, the military context, hypotheses of causation, and issues of management are explored to discover whether there are contemporary relevancies to the current issue of mild traumatic brain injury. When shell shock was first postulated, it was assumed to be the product of

Disturbances in hormonal regulation of vascular tone during traumatic shock.

Changes in hormonal regulation of the vascular tone in Wistar rats were studied on Cannon model of traumatic shock. The pressor response to angiotensin II decreased by 30-40% 3 h after the incidence of trauma. The reaction to vasopressin remained unchanged. However, phenylephrine in medium and high doses produced a more pronounced pressor response under these conditions. One day after trauma we revealed a decrease in vascular sensitivity not only to angiotensin II, but also to vasopressin and alpha1-adrenoceptor agonist phenylephrine. The vascular response was observed only after treatment wit

Noninvasive muscle oxygenation to guide fluid resuscitation after traumatic shock.

BACKGROUND: Three different protocols tested the hypothesis that hind limb muscle tissue O(2) saturation (StO(2)), measured noninvasively with near-infrared spectroscopy (NIRS), is as reliable as invasive systemic oxygenation indices to guide fluid resuscitation. METHODS: In series 1, swine (n=30) were hemorrhaged, then received either no fluid, a fixed volume of colloid (15 mL/kg), or shed blood plus lactated Ringer's (LR) titrated to MAP >60 mm Hg. In series 2, swine (n=16) received a penetrating femur injury, a 47% to 55% hemorrhage to determine a median lethal dose (LD(50)) then shed bloo

Blood rheological abnormalities in the microcirculation during experimental traumatic shock.

OBJECTIVE: To find whether the principal indices of blood rheological disorders related to the microcirculation undergo drastic changes during the traumatic shock. METHODS: RBC aggregability and deformability, as well as the systemic hematocrit, were assessed in white laboratory rats with the techniques that provided us with direct and quantitative data. RESULTS: We found that in the experimental animals the RBC aggregability was sharply increased, the RBC deformability significantly decreased, while the systemic hematocrit underwent considerable lowering during the traumatic shock. CONCLUS

Beneficial effects of prostaglandin E1 infusion in experimental traumatic shock.

The effects of prostaglandin E1 (PGE1) were studied in a standardized model of traumatic shock in rats. Pentobarbital anesthetized rats were subjected to standardized drum trauma of 525 revolutions in a Noble-Collip drum. These traumatized rats were characterized by a survival time of 108 +/- 19 min, a 12-fold increase in plasma cathepsin D activity, and a three-fold increase in plasma myocardial depressant factor (MDF) activity. PGE1 (1.2 micrograms/kg X min) significantly improved survival time during traumatic shock (191 +/- 29 vs. 108 +/- 19 min), drug vs. vehicle, respectively (p less tha

Cellular mechanisms of heparinase III protection in rat traumatic shock.

Pentobarbital-anesthetized rats subjected to traumatic shock developed a shock state characterized by marked hypotension to 65-70 mmHg, a survival time of 88 +/- 13 min, significant increases in ileal myeloperoxidase activity (P < 0.01), and severe endothelial dysfunction as evidenced by a significant (P < 0.01) decrease in vasorelaxation to endothelium-dependent dilators. Treatment with heparinase III (45 microg . kg-1 . min-1) 10 min posttrauma prolonged survival time to 223 +/- 19 min (P < 0.001), significantly attenuated ileal myeloperoxidase activity (P < 0.01), and significantly preserve

Beneficial effects of C1 esterase inhibitor in murine traumatic shock.

Activation of the complement system is an integral part of the initiation and maintenance of the inflammatory process such as that occurring in traumatic shock, and is considered responsible for much of the trauma-induced microvascular injury. We investigated the effects of early complement blockade induced by a C1 esterase inhibitor (C1 INH) in a rat model of traumatic shock. Pentobarbital-anesthetized rats subjected to Noble-Collip drum trauma developed a shock state characterized by marked hypotension and a 83.3% mortality rate with a mean survival time of 157.5 +/- 26 min. Accompanying the

[Anaphylactic shock after traumatic rupture of a splenic echinococcal cyst].

Anaphylactic shock as a result of trauma is very rare. We describe a 20-year-old Druze soldier who presented with anaphylactic shock due to rupture of a splenic echinococcal cyst induced by blunt trauma to the left chest wall and upper abdomen. The main clinical manifestations, which developed within minutes of the trauma, were high fever, pruritus, edema of the lips and eyelids, dyspnea, stridor and rhinorrhea. Eosinophilia was not present on admission but appeared 4 days later. Surgery revealed an intact echinococcal cyst in the left lobe of the liver and another in the spleen. The splenic c

[Leucostasis in the lung during hypovolemic-traumatic shock (author's transl)].

The cause of the posttraumatic respiratory distress syndrome is not yet well known and cannot be proved with certainty. The early stage of posttraumatic lung failure has been examined both from a morphological and biochemical aspect. Lung biopsies taken during hypovolemic-traumatic shock show besides endothelial damage an accumulation of leucocytes with free granules, similar to lysosomes. Based on these results further tests have been carried out to achieve a similar reaction in the hypovolemic-traumatic shock model in the dog, enabling us to undertake more extensive examinations and to use n
