Technical nuances of commonly used vascularised flaps for skull base reconstruction.

BACKGROUND AND METHODS: Reconstruction with a vascularised flap provides the most reliable outcome, with post-operative cerebrospinal fluid leak rates of less than 5 per cent. This article aims to review and summarise the critical technical aspects of the vascularised flaps most commonly used for skull base reconstruction. RESULTS: Vascularised flaps are classified as intranasal or extranasal. The intranasal group includes the Hadad-Bassagaisteguy nasoseptal flap, the Caicedo reverse nasoseptal flap, the nasoseptal rescue flap, the posteriorly or anteriorly based lateral wall flaps, and the m

'Do not touch' lesions of the skull base.

Imaging of the skull base presents many challenges due to its anatomical complexity, numerous normal variants and lack of familiarity to many radiologists. As the skull base is a region which is not amenable to physical examination and as lesions of the skull base are generally difficult to biopsy and even more difficult to operate on, the radiologist plays a major role in directing patient management via accurate image interpretation. Knowledge of the skull base should not be limited to neuroradiologists and head and neck radiologists, however, as the central skull base is routinely included

Intraorbital and intracranial extension of adenoid cystic carcinoma without clinical or radiological lacrimal gland involvement.

Adenoid cystic carcinoma (AdCC) is a rare epithelial neoplasm of the head and neck, most commonly found in the salivary glands. Orbital AdCC is an uncommon clinical entity arising from the lacrimal glands, however primary orbital AdCC has been previously described in a small number of case reports. The exact origin of the neoplasm with uninvolved lacrimal gland in the orbit is unknown, however it may arise from ectopic lacrimal or salivary gland tissue, or extension from nearby epithelial structures. We describe the clinical characteristics, investigations and management of a 55-year-old man p

Congenital supratentorial and infratentorial peripheral neurogenic tumor: a clinical, ultrastructural, and immunohistochemical study.

Congenital neoplasms of the central nervous system are extremely rare, although they have been well documented since the earliest reports from the mid-19th century. Medulloblastoma, other primitive neuroectodermal tumors, and various types of gliomas have comprised the majority of cases. This report describes a highly unusual infratentorial and supratentorial tumor presenting as a scalp mass in a neonate who experienced in utero distress. The supratentorial mass extended through a defect in the skull to the parietooccipital lobe, and the infratentorial aspect involved the 9th and 10th cranial

History and evolution of the far-lateral approach in neurosurgery.

The far-lateral approach (FLA) to the foramen magnum, lower clivus, and craniovertebral junction has become a cornerstone in accessing these regions. Historically, lesions arising at the ventral craniovertebral junction posed significant surgical challenges because of limited visualization, poor exposure or considerable brainstem retraction, and high complication rates with midline posterior and anterior transoral approaches. To overcome these limitations, early pioneers proposed the FLA, which extended the lateral suboccipital exposure and enabled controlled drilling of the occipital condyle,

Extensive Skull Base Epidermoid Cyst Surgery: Cranial Nerve Tractography and Surgical Nuances Through a 2-Dimensional Operative Video.

The surgery of skull base epidermoid cysts can be challenging because of intracerebral extensions and the encasement of cranial nerves and internal carotid artery branches.1-3 Advances in magnetic resonance imaging, particularly tractography, may help to visualize the trajectory of displaced cranial nerves,4,5 and thus reduce the surgical risks. We report here the case of a 59-year-old woman who presented with diplopia, left ataxia, and cognitive impairment, which revealed a large right skull base epidermoid cyst with infra-, supra-, and latero-sellar extensions (Video 1). The optic chiasma wa

Endoscopic Endonasal Resection of Clival Chordomas.

The endoscopic endonasal approach is a major step forward in the treatment of clival chordomas. It has improved the resection rate and in the same time reduced the complications. Chordomas located in the upper and mid-clival regions are well-suited for a purely endoscopic endonasal resection. Although with angulated endoscopes and curved high-speed drills, lateral parts of the tumor can be resected, lower clival lesions often necessitating a combination of both endonasal and transcranial approaches. A major limitation is a caudal tumor extension beyond the dens axis. For more caudally located

Lateral Molar Approach-Driven Transoral Endoscopic Procedure for Benign Infratemporal Fossa Tumor Resection.

Benign infratemporal fossa tumors necessitate complete resection while preserving neurovascular integrity. Conventional open approaches risk delayed bone healing, occlusal dysfunction, severe facial scarring, and iatrogenic neurovascular injury. We propose an endoscopic-assisted plasma ablation technique via a lateral molar transoral approach to address these limitations. A 55-year-old female with a 2.5 × 2 cm right infratemporal fossa tumor underwent the novel procedure. A 4-5 cm mucosal incision lateral to the molar established an osteotomy-free corridor along the mandibular ramus. The techn

Jugular Foramen Paragangliomas.

Paragangliomas are the most common tumors at jugular foramen and pose a great surgical challenge. Careful clinical history and physical examination must be performed to adequately evaluate neurological deficits and its chronologic evolution, also to delineate an overview of the patient performance status. Complete imaging evaluation including MRI and CT scans should be performed, and angiography is a must to depict tumor blood supply and sigmoid sinus/internal jugular vein patency. Screening for multifocal paragangliomas is advisable, with a whole-body imaging. Laboratory investigation of endo

[Transoral approach in surgery for chordomas extending into craniovertebral junction: a systematic review of the literature].

UNLABELLED: To date, treatment of chordomas involves maximal tumor resection followed by proton therapy. Various approaches are used depending on location of tumor (transcranial and through natural anatomical openings (nose, mouth), as well as their combinations). Although transoral approach has been introduced into neurosurgical practice for a long time, it is routinely used in patients with chordoma only in certain hospitals in the world. OBJECTIVE: To analyze postoperative outcomes in patients with chordomas of skull base and craniovertebral joint after transoral surgery. MATERIAL AND MET

Prognosis and histology of sporadic synchronous and metachronous meningiomas and comparative analyses with singular lesions.

Synchronous or metachronous growth of multiple tumors (≥ 2) is found in up to 20% of meningioma patients. However, biological as well as histological features and prognosis are largely unexplored. Clinical and histological characteristics were retrospectively investigated in 95 patients harboring 226 multiple meningiomas (MMs) and compared with 135 cases of singular meningiomas (SM) using uni- and multivariate analyses. In MM, tumors occurred synchronously and metachronously in 62% and 38%, respectively. WHO grade was intra-individually constant in all but two MMs, and histological subtype var

Validation of the anterior skull base questionnaire 35 in endoscopic pituitary adenoma surgery.

Patient quality of life (QOL) is an important metric of surgical success. To guide therapeutic advances in pituitary adenoma surgery, a validated, comprehensive instrument to quantify QOL is required. We aim to evaluate the validity of the 35 item anterior skull base questionnaire (ASBQ-35) in patients undergoing pituitary adenoma surgery. A total of 168 patients undergoing endoscopic resection of pituitary adenomas underwent longitudinal QOL assessment using the ASBQ-35 and the 22-item Sinonasal Outcomes Test (SNOT-22) over the first postoperative year. Validity of the ASBQ-35 was assessed by

Bony landmarks in the endoscopic endonasal transoculomotor approach.

The endoscopic endonasal transoculomotor approach (EETA) has been recently described as a doorway to access the parapeduncular space and treat pituitary adenomas with oculomotor extension. Intraoperative identification of the oculomotor triangle endonasally is challenging and dissection can put the internal carotid artery (ICA) at risk. The aim of the present study is to find reliable landmarks that identify the oculomotor triangle (OCMT) during the EETA and protect the ICA from injury. Several lines were defined for calculations. Among them, one oblique line that extends from the inferior mar

Complications of resection of malignant tumours of the skull base: outcome and solution.

The results of surgical resection of malignant tumours located at the skull base have improved significantly over the years. There are however still some complications associated with these procedures. For central skull base lesions, the anterolateral approach exposes the region adequately for an oncological resection to be carried out. The development of palatal fistula in irradiated patient has been a nuisance. With modification of the palatal incision, this problem is now eliminated. For the management of osteoradronecrosis of the skull base, an uncommon late complication, a microvascular f
