Skull base plasmacytoma with conductive hearing loss and an external auditory canal mass.

Skull base plasmacytomas are rare and difficult to differentiate clinically and radiologically from other tumors of the head and neck. Because of the risk of progression to multiple myeloma, early diagnosis is essential. We report the case of a 65-year-old woman who presented with left-sided conductive hearing loss and an external auditory canal mass. The tumor was removed along with much of the tympanic membrane, and it was found to be a plasmacytoma. The patient was subsequently diagnosed with multiple myeloma and treated with chemoradiation before being lost to follow-up.

Endoscopic multiport resection of a transspatial, retromaxillary tumor.

Tumors of the retromaxillary tissues can grow to involve multiple deep, skull base spaces, including the pterygopalatine fossa, infratemporal fossa, and the parapharyngeal space. Management of these tumors must account for heterogeneous pathology as well as the critical neurovasculature in and surrounding these spaces. Traditionally, these tumors have been approached the open skull base or anterior craniofacial approaches. Modern endoscopic endonasal and multiport approaches have increasingly become favored for certain tumors in the retromaxillary skull base region. In this report, we present

Innovations in surgical approach: lateral cranial base approaches.

The extradural anterior transpetrosal approach possesses several advantages. First, the majority of the dissection is extradural, providing protection to the overlying neural structures. Second, only limited retraction of the temporal lobe is necessary, protecting venous outflow via the vein of Labbe'. Third, transposition of cranial nerves or vascular structures in unnecessary. Fourth, it enjoys natural compatibility with other surgical approaches to enhance exposure of the posterior cavernous sinus and petroclival region. The disadvantages are mostly related to the unfamiliar anatomy. Most s

Advances in Bioprinting Technologies for Craniofacial Reconstruction.

Recent developments in craniofacial reconstruction have shown important advances in both the materials and methods used. While autogenous tissue is still considered to be the gold standard for these reconstructions, the harvesting procedure remains tedious and in many cases causes significant donor site morbidity. These limitations have subsequently led to the development of less invasive techniques such as 3D bioprinting that could offer possibilities to manufacture patient-tailored bioactive tissue constructs for craniofacial reconstruction. Here, we discuss the current technological and (pr

Jugular venography for evaluation of abnormalities of the skull base.

Jugular venography done to evaluate abnormalities at the base of the skull demonstrated three distinctly different patterns depending on whether there is occlusion, invasion, or growth with in the internal jugular vein. Improper technique results in a lack of intracranial dural sinus filling which may masquerade as venous occlusion. This problem is avoided by adequate neck compression along with proper volume and rate of delivery of contrast. Radiographically, an abnormal jugular vein at the base of the skull will show a concave defect in true occlusion, constriction or invasion of the vein by

Impact of salvage surgery with or without reirradiation for skull base meningiomas recurring after prior radiotherapy.

OBJECTIVE: Long-term follow-up of meningiomas has demonstrated recurrence rates ranging from 2.5% to 48% after 10 years, depending on histology grade. There are limited data available to guide the management of recurrent and previously irradiated skull base meningiomas, and challenges related to salvage surgery, reirradiation, and lack of clear systemic therapy strategies remain. In this study, the authors analyzed data from their experience with recurrent and previously irradiated meningiomas to assess the impact of salvage surgery and reirradiation on progression-free survival (PFS). METHOD

Pediatric Cases of Recurrent Skull Giant Osteoma Misdiagnosed as Fibrous Dysplasia.

Osteomas are benign mature bone tumors that typically arise in the skull. Osteomas larger than 3 cm in diameter are considered giant osteomas. Giant osteomas of the skull vault are very rare, especially in children; therefore, only a few cases have been reported in the literature. Although osteomas are usually asymptomatic, a large skull mass can cause headache, as well as esthetic disfigurement of the forehead. it can be misdiagnosed as other conditions, such as fibrous dysplasia, ossifying cephalhematoma, or other malignant bone tumors. Herein, the authors report 2 rare pediatric cases of gi

Surgery of extra-axial tumors of the cerebral base.

Because of the complex structure of the cranial base and its close proximity to cranial nerves and vessels, surgery in this area is associated with considerable risk of morbidity and mortality. Multiple approaches to each part of the cranial base have been developed over the past few decades, ranging from small modifications of more traditional approaches to complex and sophisticated new techniques. However, experience has shown that optimal outcome is achieved if the selected approach is not associated with significant approach-related morbidity. Furthermore, not all cranial base tumors can b

Predicting epilepsy in patients diagnosed with intracranial meningiomas: A systematic review and meta-analysis of clinical and anatomical risk factors.

INTRODUCTION: Meningiomas comprise 40% of all central nervous system neoplasms, with peak prevalence occurring in the fifth and sixth decades of life. Presenting symptoms vary between patients and are location-dependent, but new-onset seizures bring a subset of patients to clinical attention. OBJECTIVE: The present meta-analysis aims to identify the clinical features and tumor characteristics most associated with seizures at diagnosis of intracranial meningioma. METHODS: The PubMed, Scopus, and Embase databases were queried for all primary retrospectives (case-control studies, cohort studies

Demographic and Diagnostic Spectrum of Neurosurgical Biopsies: Initial Experience From a Re-established Neurosurgical Unit in a Tertiary Hospital in North Central Nigeria.

Introduction Neurosurgical biopsies are obtained from lesions of the central nervous system, comprising the skull, brain, spine, spinal cord, and nerves. Neurosurgery practice is a highly specialized field with wide disparities related to access to care, especially in developing countries where there are few specialists and poor support care for patients. After over 20 years of redundancy, the neurosurgical unit in Jos University Teaching Hospital (JUTH), Jos, Plateau State, Nigeria, was re-established to meet the needs of patients in the area of neurosurgery. The aim of the study is to docume

Evidence for increased clinical severity of familial and sporadic Paget's disease of bone in Campania, southern Italy.

UNLABELLED: The analysis of 236 Italian patients with Paget's bone disease showed higher clinical severity and greater frequency of neoplastic degeneration among patients who live or descend from individuals living in the Campania region (southern Italy). A prevalent involvement of the spine and the skull, the sites preferentially involved in giant cell tumors complicating Paget's disease, was also shown in familial cases from this geographical region. INTRODUCTION: The Campania region in southern Italy has been recently indicated as a high prevalence area for Paget's disease of bone (PDB), a

Is the skull view necessary in routine gamma camera bone scanning?

Bone scans were reviewed in 478 patients, 237 with carcinoma of the breast, 47 with carcinoma of the lung, 43 with carcinoma of the prostate, 56 with other malignancies and 95 in whom scans were performed for basic screening purposes. In only 11 of the scans reviewed were abnormalities of the skull alone observed and in only one of the 11 scans was the finding of an abnormality thought to be clinically useful. This study suggests that in patients with known malignant disease, the skull view adds little useful information. However, when performing bone scans for screening purposes, the skull vi

A clinical-radiographic classification of skull base lesions.

Lesions involving the base of the skull can be divided into primary and secondary disease processes. The primary group consists of bone lesions more often seen elsewhere in the skeletal system while the secondary group consist of those lesions extending to the base of the skull by direct extension from adjacent intracranial or extracranial sites. The purpose of this paper will be to arrive at a classification of skull base lesions based on the roentgen localization of the disease process. The base of the skull because of its anatomically variable curvature can be difficult to evaluate on good

Sellar, suprasellar, and parasellar masses: Imaging features and neurosurgical approaches.

The sellar, suprasellar, and parasellar space contain a vast array of pathologies, including neoplastic, congenital, vascular, inflammatory, and infectious etiologies. Symptoms, if present, include a combination of headache, eye pain, ophthalmoplegia, visual field deficits, cranial neuropathy, and endocrine manifestations. A special focus is paid to key features on CT and MRI that can help in differentiating different pathologies. While most lesions ultimately require histopathologic evaluation, expert knowledge of skull base anatomy in combination with awareness of key imaging features can be
