BACKGROUND: Inflammatory sacroiliitis associated with spinal cord injury (SCI) as an unusual cause of elevated erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) level has not been reported previously to our knowledge.
OBJECTIVE: To represent a case of SCI associated with bilateral sacroiliitis causing ESR and CRP level elevation.
METHODS: Case report of a man with T9 paraplegia.
FINDINGS: ESR and CRP levels were high. Pelvic radiography was nearly normal, except for mildly blurred sacroiliac joints with normal margins. A 3-phase bone scan revealed bilateral sacroiliitis and

Respiratory dysfunction is a major cause of morbidity and mortality in spinal cord injury (SCI), which causes impairment of respiratory muscles, reduced vital capacity, ineffective cough, reduction in lung and chest wall compliance, and excess oxygen cost of breathing due to distortion of the respiratory system. Severely affected individuals may require assisted ventilation, which can cause problems with speech production. Appropriate candidates can sometimes be liberated from mechanical ventilation by phrenic-nerve pacing and pacing of the external intercostal muscles. Partial recovery of res

Progress in the care of people with spinal cord injury (SCI) spans every aspect, from research in neuroregeneration to pharmacologic interventions. This article focuses on advances in rehabilitation interventions, which have employed bioengineering, computerization, and advanced therapeutic techniques. These interventions are being applied to functional deficits of the bladder, bowel, upper extremities, and respiratory system, as well as to improvements in ambulation and mobility. Functional electrical stimulation (FES) is being used to augment the function of the lower extremities, the upper

Pregnancies in spinal cord-injured patients present unique clinical challenges to obstetric providers. Spinal cord injury (SCI) alters the function of multiple organ systems, and chronic medical conditions are extremely common in this patient population. Autonomic dysreflexia (ADR) is a potentially life-threatening complication of SCI, usually involving patients with spinal cord lesions at or above the T6 level. Intrapartum care of women with SCI is particularly complicated, and labor is the period during which ADR is most likely to arise. A multidisciplinary team in a unit capable of invasive

PURPOSE: Bladder management programs for patients with spinal cord injury and neurological disease (SCIND) include intermittent catheterization and sphincterotomy with external catheter drainage. These programs depend on maintaining a patent urethra. Once urethral stricture, erosion, diverticulum or urethrocutaneous fistula occurs, the only treatments available are urethral reconstruction and urinary diversion. We evaluate the role of urethral reconstruction in this subset of patients.
MATERIALS AND METHODS: The charts of 18 patients with SCIND (spinal cord injury 16, cerebral palsy 1, mening

BACKGROUND: The number of people in the United States with spinal cord injury (SCI) is estimated at about 200,000. The prevalence of gallbladder disease in this population is about three times as high as in neurally intact people, but the results of surgical treatment have received little attention.
METHODS: A retrospective, population-based study of patients with SCI who later received cholecystectomy for benign gallbladder disease was performed. National computer data sets of all patients receiving medical care in all Department of Veterans Affairs (DVA) medical centers for fiscal years 199

Spinal cord injuries are not as common as many other types of injuries. The victims are often young, the injury debilitating, and the effects devastating and incalculable. The acute management of patients with spinal cord injury can significantly affect the patient's eventual neurologic and functional outcome and ultimately their quality of life. Early interventions are aimed at reestablishing physiologic homeostasis, lessening the amount of secondary injury, and preserving neurologic function.

OBJECTIVE: To observe the changes of endogenous extracellular adenosine after spinal cord injury (SCI) to rats and the effect of exogenous adenosine on extracellular calcium after SCI and post-injury neurological function.
METHODS: A ventral compression injury model of T13 spinal cord was used, and the extracellular fluids were collected consecutively every 20 minutes after injury by using microdialysis. Adenosine in the samples was analyzed using high-pressure liquid chromatography (HPLC) with u.v. detection. The rats received different doses of 2-chloroadenosine (2-CADO), a nonspecific agon

OBJECTIVE: To evaluate the effect of a special calmodulin (CaM) inhibitor-trifluoperizine (TFP) on spinal cord injury (SCI) and the role of CaM in SCI.
METHOD: We observed the effect of TFP on spinal cord blood flow (SCBF), motor evoked potentials (MEP) and motor function in SCI by using hydrogen clearance, inclined plane and electro physiological technique.
RESULT: There was significant improvement of SCBF, MEP and motor function with TFP, when adrenaline maintained the system arterial pressure in SCI.
CONCLUSION: Prevention by TFP of spinal cord function in SCI suggested that CaM plays a

While research on spinal cord injury (SCI) is abundant, few studies focus on women. This population-based study investigates differences in the prevalence of secondary conditions between 128 women and 522 men. Case managers retrospectively interviewed 650 persons regarding medical and psychological conditions secondary to SCI, as well as other life issues. Overall, males and females show more similarities than differences in the ways in which they manage life with SCI. Differences were found, though, regarding etiology of initial injury, insurance coverage, caregiver use, transportation use, m

The treatment of choice for the failure to restore neurogenic bladder dysfunction, managed with clean intermittent catheterization program with incontinence, is anticholinergic medication. The goal is to increase bladder capacity, and decrease intravesical pressure in order to maintain continence between catheterizations. The most commonly used anticholinergic medication in the United States is Oxybutynin. Previous clinical studies have shown that 61% of the patients on oral Oxybutynin report adverse systemic side effects and are noncompliant in taking it. In 48% of the patients, possibly due

BACKGROUND: The risk of heart disease in patients with spinal cord injury is similar to that in the general population. The physiologic derangements raise special problems in patients with SCI having coronary operations.
METHODS: From January 1980 to May 1995, we performed coronary artery bypass procedures on 20 patients with SCI; 4 were tetraplegic and the remainder were paraplegic. The indication for operation was angina: unstable (13), exertional (4), or postinfarctional (3). Bowel and bladder care was given immediately before operation; operating room tables were double padded and a pelvi
