Thirty-six patients with primary Ewing's sarcoma of the spine were diagnosed at the Mayo Clinic between 1951 and 1988. The mean age was 17 years (range, 5-40 years). Neurologic symptoms and signs were seen in 58% of the patients. Forty-seven percent of all patients had an open biopsy of the lesion and underwent a decompressive laminectomy. Three of the four patients with thoracic or thoracolumbar involvement had progressive kyphosis after laminectomy. All patients received radiation therapy in various dosages. Sixteen of the patients were registered in the Intergroup Ewing's Sarcoma Study. Int…

Spinal operation via an anterior thoracic approach is becoming increasingly common, and the thoracic surgeon is now being called upon to provide exposure for orthopedic and neurosurgical colleagues. We report experience with 126 such patients from 1982 through 1993. There were 61 male and 65 female patients (mean age, 39.0 years; range, 14 to 77 years). Indications were trauma in 45 patients (36%), spinal deformity in 42 (33%), cancer in 15 (12%), disc disease in 12 (10%), and infection in 12 (10%). Operative incisions included 22 (17%) right and 14 (11%) left thoracotomies, 33 (26%) right and

Aneurysmal bone cysts are uncommon bony lesions of the spine. Two such patients are reported in the study. The first patient had involvement of first and second thoracic vertebrae, and a preoperative diagnosis was made possible due to the typical magnetic resonance imaging appearances. A total removal was achieved in two stages. In the second patient the C2 spinous process was involved, and this patient presented as atantoaxial dislocation. Removal of the abnormal tissue and posterior fusion was done. Both the patients showed neurologic improvement. The relevant literature is reviewed.

Synovial cyst herniations, typically observed in the lumbar region in conjunction with degenerative changes of the facet joints, only rarely present as space occupying lesions in the cervical spine. The case of a 47-year-old white man with a C7-T1 synovial cyst impinging on the spinal cord and right C8 nerve root is presented.

Low back pain is a large and increasing cause of workers compensation claims in all industrialized nations. In the United States, 23% of all compensation claims involved back pain. Almost 90% of these were self limiting sprains and strains. It has been estimated that the remaining 10% account for approximately 75% of lost work days, medical costs and permanent disability payments. It is the goal of the spine surgeon to provide the patient with relief from his symptoms and return him or her to the work place. Advances in lumbar skeletal fixation have increased the fusion rate, decreased hospita…

When surgery for thoracic spine lesions is being considered, surgical indications should be strongly influenced by the presence of myelopathy; evidence of instability, particularly kyphosis; and the general medical and, particularly, the pulmonary health status of the patient. Numerous surgical options are available and should be utilized by considering the location, resectability, and the relationship of the lesion to the spinal cord blood supply. Options in stabilization technique and bone grafting permit adequate stabilization in most patients. As with instability syndromes in the cervical …

A 34-year-old female fell 7 meters onto her lower back and side, and sustained a nondislocated fracture of the 7th thoracic vertebra, a complex pelvic fracture with symphysiolysis and a left acetabular fracture in combination with a bilateral comminuted sacral fracture and downward intrusion of the lumbosacral spine. There was also a cauda equina-syndrome. Laparotomy with exploration of the lumbosacral area was terminated early because of hemorrhage. Later internal fixation of the fractures was performed by an anterior approach with complete reduction of the bilateral sacral fracture and the l…

Benign tumors of the cervical spine are relatively infrequent but have a number of common characteristics that aid in the evaluation and treatment of these lesions. The tumors are most common in the first and second decades of life, presenting as pain, neck stiffness and torticollis. In approximately 70%, the lesions are visible on plain roentgenograms and the remainder are well visualized on bone scan and computed tomographic scan. The majority are present in the posterior elements and may be treated adequately with excisional biopsy by curettage. Stage 3 lesions are best treated by marginal …

Developmental disorders of the pediatric spine are a group of congenital malformations commonly referred to as spinal dysraphism. These malformations are characterized by incomplete or absent fusion of midline mesenchymal, bony, and neural structures. They can be classified according to the presence or absence of an associated back mass. A detailed analysis of the embryology, clinical findings, and radiologic characteristics of the common forms of spinal dysraphism is outlined.

In modern competitive sports the vertebral column is increasingly subjected to maximum stress. This has led to lasting damage especially in young competitive sportsmen in whom the vertebral column had not yet developed to full maturity. Back pain can be more successfully analysed in respect of its causes by means of the additional use of the knowledge and examination methods of manual medicine. Emphasis is placed in this article on the importance of such pain as an early symptom of ligamentous instability. Therapeutic recommendations are given.

A general description and discussion of the normal head and spine, and major fetal anomalies has been presented. It is important to remember that there can be wide variability in the structural findings noted in the fetus with ventricular dilatation or abnormal cerebellar development. If an abnormality of the fetal head and spine is seen, it is important to carefully assess other areas of fetal anatomy because of the increased likelihood of coexisting anomalies. Because of the increased incidence of chromosomal abnormalities, evaluation by percutaneous umbilical blood sampling, amniocentesis o…

The vertebral column, consisting of individual Junghann's movement segments (smallest functional units), can show functional impairment (malfunction) producing peripheral pain mostly without an organic cause. In the ENT area there are many syndromes caused by such malfunction, mostly in the upper cervical spine. Spondylogenous disorders can perhaps trigger additional nociceptive reactions. Knowledge of the functional pathology of the vertebral column and its exact diagnosis are the basis of a comprehensive treatment strategy.

The role of magnetic resonance imaging (MRI) for the evaluation of the spine is expanding rapidly. In addition to being noninvasive, MRI offers high soft-tissue contrast and multiplanar imaging capability. MRI examinations of the spine usually include a T1-weighted spin-echo and a T2-weighted spin-echo and/or a gradient-echo sequence. As in other parts of the body, the use of surface coils results in higher-quality examinations. Various methods are employed to reduce the motion artifacts that are particularly troublesome in spine imaging. Paramagnetic contrast agents are used to enhance soft-t…

Having studied 470 case histories of patients with injuries of the spine the author found traumas of the 5th lumbar vertebra in 23 cases (4.9%). The characteristic injuries were compression wedge-shaped fractures, multiple fractures of the vertebrae, comminuted and traumatic spondylolistheses and dislocation fractures. A typical mechanism of the injury was a blow in the lumbar region or in the back with subsequent bending of the trunk. The tactics of surgical treatment of such lesions is presented, the main tasks of which are the reconstruction of the shape and the volume of the vertebral cana…

The authors analyze the achondroplastic lumbar spine in the pre-puberal and post-puberal periods until adulthood by means of standard X-rays, CT scan, MRI, and myelography. The small spinal canal in achondroplasts must be considered the result of developmental and congenital conditions. Orthopaedic and surgical possibilities are reviewed with the goal of reducing to a minimum the developmental factors.

The first part of this report is devoted to the functional anatomy of the lumbar spine, and reminds of the morphology, structure and proper physiology of each lumbar vertebral component, and then of each flexible element linking the lumbar vertebrae with one another. In this tripod system, the column of vertebral bodies and discs play an essential role, while the two columns of posterior articular processes have a supplementary, but nevertheless effective role in spinal stability and mobility. The intervertebral disc, because of the strong osmotic pressure of the nucleus and the dual direction…
