Surgical interventions for vertical strabismus in superior oblique palsy.

BACKGROUND: Superior oblique palsy is a common cause of vertical strabismus in adults and children. Patients may be symptomatic from binocular vertical diplopia or compensatory head tilt required to maintain single vision. Most patients who are symptomatic elect to undergo strabismus surgery, but the optimal surgical treatment for vertical strabismus in people with superior oblique palsy is unknown. OBJECTIVES: To assess the relative effects of surgical treatments compared with another surgical intervention, non-surgical intervention, or observation for vertical strabismus in people with supe…

Crouzon Syndrome: Relationship of Eye Movements to Pattern Strabismus.

PURPOSE: To characterize conjugate eye movements in Crouzon syndrome (CS) patients with and without strabismus. METHODS: Smooth pursuit, saccades, horizontal optokinetic nystagmus (OKN), and horizontal vestibulo-ocular reflex (VOR) were recorded using binocular video-oculography (VOG) in 10 children with CS (5 orthotropic, 5 strabismic) and 12 age-matched controls. Hess-Lancaster plots were generated from Orbit 1.8 using rectus muscle pulley locations from computed tomography imaging. Two-dimensional eye scan paths from VOG recordings were compared with the Hess-Lancaster plots. RESULTS: Tar…

Loop suture technique for optional adjustment in strabismus surgery.

PURPOSE: To describe a loop suture technique that allows intraoperative conjunctival closure and later optional suture adjustment in strabismus surgery in uncooperative patients. MATERIALS AND METHODS: This retrospective case series comprised 25 patients. After a recessed or resected horizontal muscle was secured to the sclera with a primary suture suspended back 2 mm, a second loop suture was passed through the body of the muscle and under the primary suture knot. The loop suture could be removed later while the patient was awake, or it could be tied to advance the muscle. Success was define…

Minimally invasive strabismus surgery (MISS) for rectus muscle transpositions.

AIMS: To present two similar, minimally invasive strabismus surgery (MISS) techniques for rectus muscle transpositions. METHODS: The technique and results of 10 consecutive MISS muscle transpositions performed in eight patients by applying only small radial cuts between the insertion of the weak rectus muscle and the adjacent rectus muscles are presented. RESULTS: In nine eyes a minimal and in one eye a moderate lid and conjunctival swelling could be seen on the first postoperative day. The preoperative deviation at distance of 41.7 (SD 21.6) pdpt decreased to 2.3 (4.4) pdpt at 6 months (p<0…

Combined eyelid and strabismus surgery: examining conventional surgical wisdom.

PURPOSE: To determine whether satisfactory results of both eyelid surgery and strabismus surgery can be obtained when these procedures are performed in the same setting in selected patients. METHODS: Nine patients (16 eyelids) who had undergone surgery in the same setting for eyelid malposition and strabismus of the horizontal rectus muscles, with or without concurrent vertical rectus muscle surgery, were retrospectively reviewed. Eyelid malposition surgery success was defined as a postoperative margin reflex distance (MRD1) within 1 mm of the target. Strabismus surgery success was defined as…

Minimally invasive strabismus surgery for horizontal rectus muscle reoperations.

AIMS: To study if minimally invasive strabismus surgery (MISS) is suitable for rectus muscle reoperations. METHODS: The study presents a series of consecutive patients operated on by the same surgeon at Kantonsspital St Gallen, Switzerland with a novel MISS rectus muscle reoperation technique. Surgery is done by applying two small radial cuts along the muscle insertion. Through the tunnel obtained after muscle separation from surrounding tissue, a recession, advancement or plication is performed. RESULTS: In 62 eyes of 51 patients (age 35.4 (SD 16.3) years) a total of 86 horizontal rectus mu…

Bilateral necrotizing scleritis following strabismus surgery for thyroid ophthalmopathy.

A case of a bilateral necrotizing scleritis following strabismus surgery for thyroid ophthalmopathy is described. A 71-year-old man was operated on for esotropia due to prior and equilibrated Grave's disease. Two weeks after surgery, he had a scleral necrosis that was located on the surgery sites (insertion of the two medial rectus muscles). There was no infectious or autoimmune etiology. The pathogenic mechanism was ischemic and inflammatory, doubtless facilitated by the thyroid ophthalmopathy. The outcome was favorable after 3 weeks of antibiotic and anti-inflammatory treatment. To the autho…

Anterior necrotizing scleritis after strabismus surgery in a child.

Anterior necrotizing scleritis is a rare but potentially devastating complication of ocular surgery that most often occurs after cataract surgery in elderly patients who may have an underlying systemic autoimmune condition(1) or, less likely, an infectious cause.(2) We describe the management and outcome of a case of bilateral anterior necrotizing scleritis after postoperative infection in a 19-month-old girl who had recently undergone strabismus surgery.

Animal models for visual deprivation-induced strabismus and nystagmus.

The development of gaze-stabilizing systems depends on normal vision during infancy. Monkeys reared with binocular lid suture (BLS) for the first 25-40 days of life have strabismus, optokinetic nystagmus deficits, latent nystagmus, and decreased binocular cells in the visual cortex and nucleus of the optic tract. When BLS is extended to 55 days, pendular and congenital nystagmus also occurs. Eyelids in infant monkeys are hairless and thin, but BLS still degrades sensory fusion, motion, and form perception. To determine to what extent these visual properties are critical in the development of n…

Conjunctival closure in strabismus surgery: Vicryl versus fibrin glue.

Six patients undergoing bilateral symmetric strabismus surgery were enrolled in this study. For each subject, incisions were closed with Vicryl in one eye and glue in the other. The conjunctival closure with the Vicryl suture resulted in increased discomfort and inflammation during the early postoperative period compared with fibrin glue. However, the difference between the Vicryl and the glue disappeared after 14 days.

[Correction of post-operative strabismus after scleral encircling procedures].

The scleral encircling or explant procedures for treatment of retinal detachment usually lead to post-operative strabismus, due to traumatic muscular adhesion to the globe, explant interference with muscular function, nerve damage and muscular paralysis, excessive or prolonged stretching of the muscle and surgical muscular reattachment at an improper site. In addition, there may be muscular atrophy or cicatrizing transformation due to compression. In 3 patients, the authors made use of the cicatricial tissue in place of the original muscle with or without surgical adjustment of the other ocula…

Endophthalmitis after strabismus surgery with a good visual result.

A 78-year-old woman underwent recession-resection surgery for longstanding exotropia. A scleral perforation occurred at the time of surgery and was treated with transscleral retinocryopexy. Three days after surgery the patient experienced light perception vision, pain, and hypopyon. She underwent a trans pars plana vitrectomy with intravitreal and periocular antibiotics. Three months after vitrectomy, visual acuity returned to the preoperative level of 20/50. Six months after vitrectomy she underwent an uneventful cataract extraction with posterior chamber lens implant. Visual acuity 5 months

Botulinum toxin injection of eye muscles to correct strabismus.

One hundred thirty-two doses of botulinum A toxin were injected into 42 humans. The effect on horizontal strabismus was uniformly beneficial, and effect lasting up to 411 days since the last injection was documented. The effect in vertical strabismus and lid retraction was beneficial, but less strongly so. No systemic effect or local complications were encountered except for effect on adjacent muscles. The drug appears to be a safe and useful therapy for strabismus.
