Stroke in patients with dengue.

BACKGROUND: Central nervous system infections are one of the common causes of stroke in developing countries. Stroke after dengue is uncommon. CASE REPORTS: A total of 1148 dengue cases were seen in a tertiary referral hospital during the epidemic from September to November 2008. We present 3 patients who had stroke caused by dengue. The first patient was a 45-year-old woman with dengue hemorrhagic fever who developed sudden right arm and leg weakness. Magnetic resonance imaging (MRI) revealed multiple hemorrhagic foci in the left parietal and temporal lobes. The second patient was a 35-year-…

[Stroke and the genetics of hyperhomocysteinemia].

The T allele of the methylenetetrahydrofolate reductase (MTHFR) C677T polymorphism has been shown to be a risk factor for stroke. Previous meta-analyses have shown that the individuals with the TT genotype have 1.26-1.37 times the risk for stroke as compared to those with the CC genotype. We performed a meta-analysis of all 5 Japanese studies that investigated the relationship between the MTHFR 677T allele and stroke. The risk of stroke was found to increase in a dose-dependent manner (OR for CT genotype: 1.35, 95% CI; 1.07-1.31, OR for TT genotype: 2.05, 95%CI; 1.51-2.78). This estimate was a…

An update on cardioembolic stroke.

About one-fifth of all ischaemic strokes are cardioembolic. Recent years have seen considerable progress in our understanding of atrial fibrillation, the most important cause of cardioembolism, and evidence-based treatment strategies have emerged. Progress in relation to other cardioembolic disorders has been more limited and here stroke prevention strategies remain less certain and subject to debate. This article briefly reviews the methods currently used to identify and investigate cardioembolic stroke and then provides an update on stroke prevention in relation to atrial fibrillation, valvu…

[Pseudogout as a complication of stroke].

We studied the incidence and clinical characteristics of pseudogout complicating stroke and the clinical characteristics of pseudogout occurring after stroke. Ten of 181 patients with stroke developed pseudogout. Compared with the patients of other disease, the incidence of pseudogout in the stroke patients was significantly high. Age at onset was 75 to 90, with a mean of 82. The time interval from stroke to onset of pseudogout ranged from 3 to 31 days, with a median of 5.5 days. The most common initial symptom was fever (90%), whereas subjective arthralgia was not highly frequent (50%). The a…

The burden of stroke in Nigeria.

Stroke is a leading cause of morbidity and mortality worldwide, and it is likely to worsen in developing countries over the next two decades based on the projections by the World Health Organization (WHO). With the current scourge of HIV/AIDS and the battle against other communicable diseases like multi-drug resistant malaria and tuberculosis; Nigeria, the most populous black nation in the world, stands to risk the further straining of its resources as a result of the increasing prevalence of stroke and other cardiovascular diseases due to epidemiological transition. The current prevalence of …

Pathophysiology and classification of stroke.

There are a wide variety of causes of stroke and as many different forms of presentation, depending on the area of the brain affected. This article describes how different types of stroke present and outlines the likely outcome for patients for each type.

Is this patient having a stroke?

CONTEXT: Patients suspected of having a stroke or transient ischemic attack require accurate assessment for appropriate acute treatment and use of secondary preventive interventions. OBJECTIVE: To update a 1994 systematic review of the accuracy and reliability of symptoms and findings on neurological examination for the evaluation of patients with suspected stroke or transient ischemic attack. DATA SOURCES: We identified potential articles dated between 1994 and 2005 by multiple search strategies of the MEDLINE database and review of article and textbook bibliographies along with private col…

Update in the management of stroke.

Recent advances have improved the outlook for patients with stroke, Singapore's fourth leading cause of death. Stroke unit care reduces death, dependency and institutionalisation, independent of age, gender and stroke severity. Neuroimaging is essential prior to initiation of specific therapies. While computed tomography (CT) Head remains the most widely used modality, magnetic resonance imaging (MRI), particularly diffusion weighted imaging (DWI) has enhanced the positive diagnosis of ischaemic stroke. General medical measures include close monitoring, adequate oxygenation, avoidance of exces…

The relationship of migraine and stroke.

Migraine, a common disorder of uncertain pathogenesis, is linked to ischemia in a variety of ways. In some cases the relationship is coincidental. In others, migraine may be causally related to stroke, although the mechanism of migrainous stroke, if not due to arterial dissection, is unclear. In young women, additional risk factors for stroke such as cigarette smoking, use of combined oral contraceptives and anticardiolipin antibody immunoreactivity may potentiate migraine, especially migraine with aura, as a stroke risk factor. The complexity of the relationship is highlighted in certain gene…

Profile of stroke in Bahrain.

OBJECTIVE: As no report on the pattern of stroke in Bahrain is available, we sought to determine the stroke types, the annual incidence rate, and the risk factors associated with different types of stroke in Bahrain nationals and expatriates in Bahrain. METHODS: Case records of patients admitted with stroke from January 1995 to December 1995 in a tertiary level hospital, the only one with a well established neurology service in Bahrain were reviewed. Diagnosis of stroke and its type were confirmed by clinical and computerized tomography of brain. Risk factor analysis was made based on clinica…

The management of dysphagia in stroke.

People who have strokes suffer a variety of problems, depending on the site and severity of brain damage. A particularly unpleasant and dangerous one is dysphagia which, if not properly assessed and managed, can prevent the patient receiving adequate nutrition to enable him or her to participate fully in a rigorous rehabilitation programme, or which may even cause his or her death through aspiration of food. The authors describe an appropriate nursing response to the problem, and advocate a multidisciplinary approach to the management of nutrition in the care of patients with strokes.

A review of brainstem strokes.

Stroke is the fifth leading cause of death and a common serious neurological problem. The location of a presumed ischemic lesion can be inferred by relating the observed signs and symptoms to the functional anatomy of the cerebral vessels. It is essential to be able to determine whether a lesion lies in the distribution of either the carotid or the vertebrobasilar arterial systems. The focus of this paper will be limited to a discussion of strokes occurring within the distribution of the vertebrobasilar system. Although nursing does not make medical diagnoses, a clear understanding of the clin…

The CHAT classification of stroke.

Current terminology for clinical episodes relating to stroke is inconsistent and unclear, does not permit inclusion of data regarding the location and magnitude of extracranial and intracerebral arterial disease, does not coincide with existing classifications in Europe, and characterizes a hemispheric entity only, as opposed to a global description including prior symptoms in both hemispheres. A new classification system (CHAT) has been designed to deal with these problems, including the current clinical presentation, historical clinical episodes, the site and pathologic type of arterial dise…
